Acute hamstring injuries are the most prevalent muscle injuries reported in sport. Despite a thorough and concentrated effort to prevent and rehabilitate hamstring injuries, injury occurrence and re-injury rates have not improved over the last three decades. This failure is most likely due to the following: (1) a lack of studies with high level of evidence into the identification and prevention of hamstring injuries and (2) a reductionist approach of the current literature. The objectives of this article are to review and critique the current literature regarding isolated risk factors, and introduce a new concept for a more comprehensive scientific understanding of how multiple risk factors contribute to hamstring strain injury. The authors hope that this new conceptual model can serve as a foundation for future evidence-based research and aid in the development of new prevention methods to decrease the high incidence of this type of injury.

AbstractPurpose The purpose of this study was to report a comprehensive literature review on the risk factors for anterior cruciate ligament (ACL) injuries in male athletes.Methods All abstracts were read and articles of potential interest were reviewed in detail to determine on inclusion status for systematic review. Information regarding risk factors for ACL injuries in male athletes was extracted from all included studies in systematic fashion and classified as environmental, anatomical, hormonal, neuromuscular, or biomechanical. Data extraction involved general characteristics of the included studies (type of study, characteristics of the sample, type of sport), methodological aspects (for quality assessment), and the principal results for each type of risk factor.ResultsThe principal findings of this systematic review related to the risk factors for ACL injury in male athletes are: (1) most of the evidence is related to environmental and anatomical risk factors; (2) dry weather conditions may increase the risk of non-contact ACL injuries in male athletes; (3) artificial turf may increase the risk of non-contact ACL injuries in male athletes; (4) higher posterior tibial slope of the lateral tibial plateau may increase the risk of non-contact ACL injuries in male athletes.ConclusionAnterior cruciate ligament injury in male athletes likely has a multi-factorial aetiology. There is a lack of evidence regarding neuromuscular and biomechanical risk factors for ACL injury in male athletes. Future research in male populations is warranted to provide adequate prevention strategies aimed to decrease the risk of this serious injury in these populations.Level of evidenceSystematic review on level I–IV studies, Level IV.

OBJECTIVE To assess the effects of eccentric exercise on optimum lengths of the knee flexors and extensors during the preseason in professional soccer. DESIGN Twenty-eight athletes from a professional Spanish soccer team (Division II) were randomly assigned to an eccentric exercise intervention group (EG) or a control group (CG). Over the four-week period two athletes from the control group suffered RF injuries and two athletes were contracted by other clubs. After these exclusions, both groups (EG, n=13; and CG, n=11) performed regular soccer training during the four-week preseason period. RESULTS After the four weeks, the optimum lengths of the knee flexors were significantly (P<0.05) increased by 2.3 degrees in the CG and by 4.0 degrees in the EG. The change in the EG was significantly (P<0.05) greater than that of the CG. The optimum lengths of the knee extensors were significantly increased only in the EG by 6.5 degrees . Peak torque levels and ratios of quadriceps to hamstring (Q/H ratios) were not significantly altered throughout the study for either group. CONCLUSION Eccentric exercise can increase the optimum lengths of both the knee extensors and knee extensors flexors during the preseason in professional soccer.

J Strength Cond Res 24(9): 2539-2544, 2010-Contralateral leg deficits between lower limbs during athletic movements are thought to increase the risk of injury and compromise performance. The purpose of this study was to quantify the magnitude of leg deficits during running in noninjured and previously injured Australian Rules football (ARF) players. The players included a group of noninjured ARF players (n = 11) and a group of previously injured ARF players (n = 11; hamstring injuries only). The players in the injured group (IG) had at least 1 acute hamstring injury in the previous 2 years. The legs of the noninjured players (NIG) were classified as right and left, whereas the legs of the injured players were classified as injured or noninjured. The players ran on a nonmotorized force treadmill at approximately 80% of their maximum velocity (Vmax). For the NIG, there were no significant differences between right and left legs for any of the variables. For the IG, the only variable that was significantly (p < 0.001) different between the injured and noninjured leg was horizontal force (175 ± 30 vs. 326 ± 44 N). Furthermore, horizontal force was significantly greater in the noninjured leg (IG) in comparison with either legs in the NIG (19.2% and 20.5%) and significantly less in the injured leg (IG) in comparison with either legs of the NIG (31.5% and 32.7%). In the present study, athletes with previous hamstring injuries had contralateral leg deficits in horizontal but not vertical force during running at submaximal velocities.

Hamstring strain injuries are the most prevalent muscle injuries in track and field (TF). These injuries often cause prolonged symptoms and a high risk of re-injury. Strengthening of the hamstring muscles has been recommended for injury prevention. The authors review the possible role of eccentric training in TF hamstring injury prevention and introduce exercise classification criteria to guide clinicians in designing strengthening programmes adapted to TF. The principles exposed may serve as a foundation for future development and application of new eccentric programmes to decrease the high incidence of this type of injury in other sports.

PurposeTo synthesize the results of systematic literature review focused on the effectiveness of anterior cruciate ligament (ACL) injury prevention programmes in male athletes.MethodsAll abstracts and articles of potential interest identified through the systematic literature search were reviewed in detail to determine on inclusion status. Information regarding prevention programmes to reduce ACL injuries or to modify risk factors for ACL injuries in male athletes was systematically extracted and included intervention and study design, characteristics of participants, sport and level of competition, characteristics of prevention programmes, results, and conclusions. All studies were evaluated for methodological quality to assess the risk of bias.ResultsThe principal findings of this systematic review are as follows: (1) most of the studies applied prevention programmes that utilized risk factors as outcomes of interest as opposed to ACL injury incidence (5 and 2 studies, respectively); (2) the effectiveness of prevention programmes to reduce ACL injuries in male athletes is equivocal (1 in favour, 1 against) and only refers to soccer players; (3) the effectiveness of prevention programmes to modify risk factors for ACL injuries in male athletes is controversial (2 in favour, 3 against) and outcome data are limited to cutting manoeuvres.ConclusionData regarding the effectiveness of prevention programmes to reduce ACL injuries or to modify risk factors for ACL injuries in male athletes are scarce and not conclusive. Future research to better determine the most effective approaches to optimize the effectiveness of prevention programmes targeted to reduce ACL injuries in male athletes is warranted.Level of evidenceSystematic review on level I–II evidence studies, Level II.

The objectives of this study were to examine the consequences of an acute hamstring injury on performance and mechanical properties of sprint-running at the time of returning to sports and after the subsequent ~2 months of regular soccer training after return. 28 semi-professional male soccer players, 14 with a recent history of unilateral hamstring injury and 14 without prior injury, participated in the study. All players performed two 50-m maximal sprints when cleared to return to play (Test 1), and 11 injured players performed the same sprint test about 2 months after returning to play (Test 2). Sprint performance (i. e., speed) was measured via a radar gun and used to derive linear horizontal force-velocity relationships from which the following variables obtained: theoretical maximal velocity (V(0)), horizontal force (F(H0)) and horizontal power (Pmax). Upon returning to sports the injured players were moderately slower compared to the uninjured players. F H0 and Pmax were also substantially lower in the injured players. At Test 2, the injured players showed a very likely increase in F(H0) and Pmax concomitant with improvements in early acceleration performance. Practitioners should consider assessing and training horizontal force production during sprint running after acute hamstring injuries in soccer players before they return to sports.

PURPOSE The aim of this study was to compare the effects of 2 strength and conditioning programs involving either purely vertically oriented or combining vertically and horizontally oriented exercises on soccer-relevant performance variables (ie, acceleration, jumping ability, peak power, and endurance). METHODS Twenty-two professional male soccer players were randomly assigned to 2 training groups: vertical strength (VS, n = 11) and vertical and horizontal strength (VHS, n = 11). Players trained 2 times per week during all the preseason (5 wk) and 3 weeks of the competitive season. The effect of the training protocols was assessed using double-and single-leg vertical countermovement jumps (CMJ), half-squat peak power (PP), sprint performance over 5 and 15 m, and blood lactate concentration at selected running speeds. RESULTS Both groups obtained significant improvements in PP (P < .05; ES = 0.87 and 0.80 for VS and VHS, respectively) and small practical improvements in 5-m- (P < .05; ES = 0.27 and 0.25 for VS and VHS, respectively) and 15-m-sprint time (P < .05; ES = 0.19 and 0.24 for VS and VHS, respectively). The CMJ performance showed a small improvement (P < .05, ES = 0.34) only in the VHS group. Submaximal aerobic-fitness changes were similar in both groups (P < .05; ES = 1.89 and 0 .71 for VS and VHS, respectively). CONCLUSION This study provided a small amount of practical evidence for the consideration of preseason training protocols that combine exercises for vertical- and horizontal-axis strength development in professional male soccer players. Further studies using more aggressive training protocols involving horizontally oriented conditioning exercises are warranted.

Recent literature supports the importance of horizontal ground reaction force (GRF) production for sprint acceleration performance. Modeling and clinical studies have shown that the hip extensors are very likely contributors to sprint acceleration performance. We experimentally tested the role of the hip extensors in horizontal GRF production during short, maximal, treadmill sprint accelerations. Torque capabilities of the knee and hip extensors and flexors were assessed using an isokinetic dynamometer in 14 males familiar with sprint running. Then, during 6-s sprints on an instrumented motorized treadmill, horizontal and vertical GRF were synchronized with electromyographic (EMG) activity of the vastus lateralis, rectus femoris, biceps femoris, and gluteus maximus averaged over the first half of support, entire support, entire swing and end-of-swing phases. No significant correlations were found between isokinetic or EMG variables and horizontal GRF. Multiple linear regression analysis showed a significant relationship (P = 0.024) between horizontal GRF and the combination of biceps femoris EMG activity during the end of the swing and the knee flexors eccentric peak torque. In conclusion, subjects who produced the greatest amount of horizontal force were both able to highly activate their hamstring muscles just before ground contact and present high eccentric hamstring peak torque capability.

Quadriceps muscle strains frequently occur in sports that require repetitive kicking and sprinting, and are common in football in its different forms around the world. This paper is a review of aetiology, mechanism of injury and the natural history of rectus femoris injury. Investigating the mechanism and risk factors for rectus femoris muscle injury aims to allow the development of a framework for future initiatives to prevent quadriceps injury in football players.
