Comparison of functional results in non-traumatic and traumatic spinal cord injury.

Purpose. This study was conducted primarily to be a descriptive study about non-traumatic (NT) spinal cord injury (SCI) patients, in terms of their demographic and neurological features, and to investigate the complications and efficient results of rehabilitation in this group. The second aim of the study was to conduct a comparison between non-traumatic SCI patients and traumatic SCI patients. Materials and methods. The design was a retrospective study at a Physical Medicine and Rehabilitation Training and Research center. The study was carried out with 194 patients, of whom 63 had non-traumatic SCI and 131 had traumatic SCI. Main outcome measures were: Demographics, etiology, Functional Independent Measure score, and level of injury. Results. A total of 32.47% (n = 63) of the patients in the study had a non-traumatic SCI. In traumatic SCI group, the proportion of male patients were significantly more than the proportion of female patients (p = 0.002). Admission FIM score of non-traumatic SCI group was better than the traumatic SCI group (p = 0.004). The comparison of discharge FIM scores between traumatic SCI and non-traumatic SCI groups did not yield any significant results (p = 0.303). However, the comparison of FIM gain between the two groups showed a significant difference (p = 0.03). The most common complication in non-traumatic SCI group was urinary tract infection and this was observed in 25 patients (39.68%). Conclusions. The present study showed that there are certain differences between NT and traumatic SCI patients. Therefore, not all SCI patients should be considered to be falling under a single category, and should be divided into groups on the basis of the etiology of the injury. It is important to consider this information while developing the targets and planning of the rehabilitation program.

Comparision of Ultrasound Therapy of Various Durations in the Treatment of Subacromial Impingement Syndrome

[Purpose] A prospective, randomized, single-blind study was performed in order to compare the efficacy of ultrasound treatments of various durations for patients with subacromial impingement syndrome. [Subjects and Methods] One hundred patients who had been diagnosed with subacromial impingement syndrome by clinical examination and magnetic resonance imaging were included in this study. Patients were randomly divided into two groups of fifty patients per group. The first group received 15 sessions of therapeutic ultrasound (4 minutes), superficial heat and transcutaneous electrical stimulation therapy combined with exercise. The second group received the same treatment except that each of the 15 ultrasound sessions were eight minutes in length. The patients were evaluated before and after the treatment. A visual analog scale (VAS) was used to assess pain, the University of California at Los Angeles (UCLA) and Constant Scale were used to assess shoulder function and the Beck Depression Inventory (BDI) was used to quantify depressive symptoms. [Results] There were no statistically significant differences between the groups in age, time since the onset of pain, sex, education and depression levels prior to the treatment. The post-treatment evaluation of patients VAS, UCLA, Constant, and BDI scores showed statistically significant within group improvements. When the two groups were compared, we found no statistically significant differences in the Constant activities of daily living, Constant external rotation, Constant force and BDI scores. However, the second group scored better than the first group in all the remaining parameters. [Conclusion] Ultrasound therapy was found to have beneficial effects on pain and functional status in the treatment of subacromial impingement syndrome. Eight minutes of ultrasound treatment was shown to be more effective than 4 minutes of ultrasound treatment.

Low Back Pain Prevalence and Characteristics in Caregivers of Stroke Patients: A Pilot Study

Abstract Purpose: The aim of this study was to determine the frequency and features of low back pain (LBP) among stroke caregivers. Method: Participants included 64 caregivers of stroke survivors in our inpatient clinic. Age, gender, body weight, FIM™, and Brunnstrom Scale of patients and low back pain history, Short Form-36 (SF-36), Beck Depression Measure (BDM), and Oswestry Disability Scale (ODS) of caregivers were reported. Descriptive statistics, Spearman correlation, and Mann-Whitney U test were used. Results: Fifty-three (82.8 %) of caregivers had LBP. Stroke survivors of caregivers with LBP (group 1) had significantly lower FIM™ scores when compared with stroke survivors of caregivers without LBP (group 2) (P < .05). All SF-36 subscores and total score of group 1 except mental health were significantly lower than SF-36 scores of group 2. Discussion: The lifetime, 12-month period, and point prevalence of LBP were 44.1%, 34.0%, and 19.7%, respectively, in our country. Caregivers had a higher frequency of LBP in this study. However it was a study with a small number of participants. There are many studies about stroke caregivers’ depression and life quality. LBP should also be investigated.

Validity and reliability of a Turkish Brief Pain Inventory Short Form when used to evaluate musculoskeletal pain

BACKGROUND The Brief Pain Inventory (BPI) is both a questionnaire and an outcome measure that is used widely in clinical trials to assess pain associated with many conditions. The BPI Short Form has been extensively translated into foreign languages. The aim of this study was to assess the validity and reliability of a Turkish Brief Pain Inventory Short Form (BPI-TR) to evaluate musculoskeletal pain. METHODS In total, 297 patients with musculoskeletal pain participated in the study. Demographic characteristics and brief medical histories were recorded. Pain intensity was assessed using a visual analogue scale (VAS) and quality-of-life was assessed using the Short Form 36 (SF-36). Pain was evaluated using the BPI-TR in all patients. Internal consistency and test-retest analysis were used to assess reliability. The internal consistency of the scale items was assessed by calculating Cronbachs α value, which was expected to be > 0.7. The criterion validity of the BPI-TR was assessed by correlation with VAS scores. RESULTS Pain intensity, pain interference, and other components of the Turkish version were consistent with validity thereof. Cronbachs α was 0.84 for pain intensity and 0.89 for pain interference. The extent of BPI-TR and VAS correlation was statistically significant. CONCLUSIONS The BPI-TR may be used for assessment of musculoskeletal pain.

Comparison of therapeutic duration of therapeutic ultrasound in patients with knee osteoarthritis

[Purpose] The aim of study was to compare different durations of ultrasound in patients with knee osteoarthritis. [Subjects and Methods] One hundred patients diagnosed with bilateral knee osteoarthritis (OA) were enrolled in this study. Patients were divided into two groups. The first group (G1) received 4 minutes of ultrasound. The second group (G2) received the exact same treatment, but the duration of ultrasound was longer at 8 minutes. Patients in both groups underwent a total of 10 ultrasound over 2 weeks. Following treatment, all patients provided self-evaluations of pain via the Visual Analog Scale (VAS), overall physical function with WOMAC, disability via the Lequesne index (Leq), and depressive symptoms with the Beck Depression Index (BDI). [Results] There were no significant differences in VAS, WOMAC Leq, and BDI values between groups 1 and 2. After treatment, VAS, WOMAC, Leq, and BDI values improved for both treatment groups. However, following treatment, G2 had significantly greater values for WOMAC functional and total scores than G1. No statistically significant differences were observed for VAS scores while inactive, WOMAC pain and stiffness scores, and BDI values after treatment between both groups. VAS pain scores while active and Leq index values were significantly lower in G1 than G2. [Conclusion] Patients in both groups demonstrated improved functionality, pain and psychological status following a consistent, 2-week regimen of 4-minute or 8-minute treatments with ultrasound. Yet, patients that experienced longer treatment durations of 8 minutes demonstrated better outcomes in pain and the ability to carry out activities of daily living.

The effects of orthotic intervention on nerve conduction and functional outcome in Carpal Tunnel Syndrome: A prospective follow-up study

AIM To evaluate the effects of using night orthosis for 6 weeks in patients with mild to moderate carpal tunnel syndrome (CTS), including a follow up after 3 months using electrophysiological and functional outcome measurements. STUDY DESIGN 12 week follow-up prospective study. METHODS Twenty-two patients with a total of 36 hands diagnosed as CTS were included. Subjects were informed about using a night orthosis for 6 weeks and were evaluated at the baseline, 6th and 12th week. RESULTS Median motor distal latency was significantly decreased and median motor compound muscle action potential was significantly increased at the 6th week. Median sensory velocity was significantly increased at the 12th week. No significant difference was found in terms of functional outcome measurements. CONCLUSION Electrophysiological follow-up findings support the positive effects of using a wrist orthosis on median nerve conduction for CTS patients.

The Effects of the Pilates Exercises on Rectus Abdominis Thickness and Balance in Healthy Female Health Worker: Prospective, Single Blind Study with Controlled Group

Pilate’s exercises were originally designed for and used by performance artists, but have enjoyed wider popularity in recent years [1]. Joseph H. Pilates developed a comprehensive program known as the Pilates method in the 1920’s. Over the last decade, the popularity of this set of exercises has grown and Pilates is now used as a form of fitness and holistic health [2]. The Pilates method can offer significant improvement in personal autonomy, static balance and quality of life in elderly females [3]. The abdominal muscles flex the lumbar spine, where rectus abdominis is particularly powerful muscle. Acting collectively, the abdominal muscles not only increase intra-abdominal pressure but also affect forced expiration, sneezing and coughing [4]. Rectus abdominis muscle is one of the anterior abdominal muscles. This muscle is originated from pubic crest and pubic symphysis and inserted in xiphoid process and costal cartilages 5-7. It is innervated by intercostal nerve (T7-T11) and subcostal nerve (T12). Function of rectus abdominis is to depress ribs and flex trunk [5].
