Randomized, controlled study on adjuvant immunochemotherapy with PSK® in curatively resected colorectal cancer

A randomized, controlled trial of adjuvant immunochemotherapy with PSK®(Kureha Chemical Industry Co., Tokyo, Japan) in curatively resected colorectal cancer was studied in 35 institutions in the Kanagawa prefecture. From March 1985 to February 1987, 462 patients were registered. Four hundred forty-eight of those patients (97.0 percent) satisfied the eligibility criteria. The control group received mitomycin C intravenously on the day of and the day after surgery, followed by oral 5-fluorouracil (5-FU) administration for over six months. The PSK®group received PSK®orally for over three years, in addition to mitomycin C and 5-FU as in the control group. At the end of February 1990, the median follow-up time for this study was four years (range, three to five years). The disease-free survival curve and the survival curve of the PSK®group were better than those of the control group, and differences between the two groups were statistically significant (disease-free survival,P=0.013; survival,P=0.013). These results indicate that adjuvant immunochemotherapy with PSK®was beneficial for curatively resected colorectal cancer.

Relationship between adrenergic fibers and the production of experimental gastric ulcer

SummaryTen rats per group, when injected with reserpine or 6—hydroxydopamine (6-OHDA), all developed gastric erosion. A study of the development of the lesion by a fluorescent histochemical method revealed that the injection of reserpine was associated with the prolonged exhaustion of the adrenergic amines, decrease of serotonin, ischemia of gastric mucosa and increase of gastric acid secretion. This suggests the possibility that following treatment with reserpine occurrs first ischemia of the gastric mucosa, due to the release of adrenergic amines within the tissues, then the gastric secretion increases as a result of the exhaustion of the function of adrenergic fibers; and finally, erosion ensues. Next, 6-OHDA, which is said to cause degeneration of sympathetic nerve terminals, was injected into rats. Fluorescence in the adrenergic fibers decreased, but serotonin fluorescence failed to decrease; nevertheless erosion appeared in the rat’s stomach. From these results it can be concluded that both reserpine ulcer and 6-OHDA ulcer are produced by disordered amine balance (setoronin, adrenergic amines, etc.) within gastric tissues and by the effect of the released biogenic amines.

Gallbladder contractility and gallstone formation in the Richardson Ground Squirrel

SummaryThe pathogenesis of cholesterol cholelithiasis in man is probably multifactorial and the mechanism by which gallbladder stasis occours in gallstone patients has not been studied in detail. In the present study, time-dependent changes in gallbladder motility of the Richardson Ground Squirrels were investigated. 100 of animals were examined for their contractile responses of gallbladders to CCK-OP and Ach in vitro. Also, biochemical changes of serum and bile were investigated. There were no significant differences in motility of gallbladders to CCK-OP or Ach between control and cholesterol-fed animals. The results of this study indicate that gallbladder muscle contractility remains unchanged during the dietary induction of cholesterol gallstones in the Richardson Ground Squirrel.

ConclusionAbove mentioned results indicated that, that is, the patients with gastric ulcer, as having non-specific abdominal pain without meal, larger in size, upper part from pyloric ring in location, and senile ulcer might be judgde to get the application or adaptability of surgical treatment for gastric ulcer.

On the minute gastric cancer accompanied with duodenal ulcer

From January, 1965 to June, 1968 513 cases with gastric lesions were operated including 241 cases with gastric and/or duodenal ulcers and 218 cases with gastr ic cancers (42.5%). For three years (from January, 1965 to December, 1967) 10 cases of gastr ic cancers were found among 9723 subjects by the gastric-mass-survey, out of them there were 5 cases (50%) proved early gastric cancers (superficial carcinomas of the stomach), while by means other than the gastric-mass-survey the superficial carcinoma of the stomach was observed in only 8.2% of the gastric cancers. The rate of resectabili ty of gastric cancer was 65.6% (143 cases) including total gastrectomy. Thirty lesions (27 cases) of early gastric cancer were presented: IIc and I Ic+II I types were the most (60.0%), I and IIa types were 36.7%. Besides, 4 cases with gastr ic double cancers, 3 cases of gastric cancer with gastric ulcer, one case of gastric cancer with polyps and 10 cases of gastric polyps (18 polyps) were reported. The relationship between bradykinin-like substance and dumping syndrom

Forty-two pat ients with duodenal ulcer were operated by th is procedure. The operative morta l i ty is zero. There is no major complication af ter the surgery. This procedure is charactal ized by the less operative s t ress and safety on patients, technical ease of the operation, and good postoperat ive function of the gas t ro in tes t ina l physiology. The cases include two uncontrollable bleeding ulcer occured a f te r b ra in injury by traffic accident. Two cases were also uncontrollable bleeding ulcer occured on recipient a f te r the kidney homotransplanta t ion . Four cases had panper i toni t i s with perfora ted ulcer. Seven cases had massive hemorrhage. All these pat ients are well now af ter th is simple operation. Four of these showed sl ight dumping syndrom, however, these were t rans ient . Two pat ients show hear tburn , but much improved than before the surgery. Due to the simplici ty of this procedure, one pat ient received th is operat ion with hysterectomy and other received wi th oophorectomy. Pa t ien t s with lung tuberculoses, diabetes mell i tus, hyperthyroidismus, rheumat i smus also received this operation wi thout any danger. No recurrence has been mentioned up to date of postoperative two years and six months.

Serotonin determination in urine and blood by gastric disease, especially the relation of serotonin to the dumping syndrome

was cooled as low as 15~ as tempera ture of an te r ior wall of body (measured with a needle thermis ter) . The other par t s of s tomach were es t imated to range within 15_+3~ as indicated by a pre l iminary study. Wi th this gastr ic tempera ture , cooling was carr ied out for 60 minu tes and following observations were performed. (1) Gastric specimens (27 in total) were obtained before and 24 hrs , 4, 7 and 10 days af ter cooling. Histologically these specimens had edema, thrombosis, hyperemia and round cell infiltration, the most f requent change being edema af ter 24 hrs (24%) and round cell infi l trat ion af te r 4 to 10 days (80 to 33%). The parietal cell count was noted decreased (20 to 25%) in specimens af ter 24 hrs , 3 and 7 days and re turned to the control level af ter 10 days. The chief cell count was noted decreased in all kind specimens. (2) Free Hydrochloric acidity (1 case) was noted to be profoundly decreased (83%)a f t e r 24 hrs and to be same as control af ter 7 days and above the control (40%) af ter 14 days. (3) Gastric e lec t romyography revealed an excited moti l i ty on inser t ion of the balloon. With cooling started, frequency of motion decreased markedly and this decrease re turned to the control level along the recovery of normal gas t r ic tempera ture (4) E. C. G. (9 cases) general ly showed only bradycardia, and one case T wave became reversed. (5) Portal pressure rose as much as 40 mmHzO only with inflation of the balloon and approximated to the control when balloon was deflated. Cooling itself did not cause noticeable change. In conclusion, there was no grave irreversible ha rms caused by gast r ic cooling with p resen t magnitude, but it is also worth an at tent ion tha t gastr ic cooling has disadvantages as well as advantages f rom the view point of controll ing hemorrhage or its recurrence .
