Severidade de cárie em crianças e relação com aspectos sociais e comportamentais

OBJECTIVE To identify social and behavioral conditions that could act as risk factors to the severity of dental caries in 12-year-old children. METHODS Based on the oral health survey data obtained in Florianópolis, Brazil, in 1995, social and behavior conditions were compared between two 12-year-old children groups with dental caries with different severity: high and very high severity, and very low severity. In the interview, there were questions about each family were part of the interview, besides social-economic conditions and behavior aspects. RESULTS/CONCLUSIONS The multivariate logistic regression analysis showed that the risk factors for dental caries with high severity were candy intake and family income. Children who consumes cariogenic products 2 or 3 times a day on a daily basis has 4.41 more chance of having dental caries with high severity when compared to children who consumes these same products only once a day - CI (OR) = [1.18; 16.43] ). Family income was the most important socialeconomic factor. Children whose family income is lower than five minimum wages has 4.18 more chance of having high severity dental caries when compared to children whose family income is higher than five minimum wages - CI (OR) = [1.16; 15.03]. The purpose of this study was to have a a better knowledge of dental caries occurrence in 12-year-old children who, in most cases, have a complete permanent dentition that showed the illness history.OBJECTIVE: To identify social and behavioral conditions that could act as risk factors to the severity of dental caries in 12-year-old children. Diferenças entre autopercepção e critérios normativos na identificação das oclusopatias

OBJECTIVE To evaluate the impact of objectively defined orthodontic needs (normative criteria) on the external aspect and mastigations satisfaction, and to compare these needs to self-perceived ones (subjective criteria) among adolescents. METHODS A cross-sectional study was carried out among all 315 students aged 14-18 years who attended a highschool in Florianópolis, Brazil. A single dentist collected clinical data about malocclusion (Dental Aesthetic Index) and a questionnaire was applied to assess perceived aesthetic and masticatory satisfaction and need for orthodontic treatment. Data analysis included frequency distribution calculation and multiple logistic regression modeling. RESULTS There were high participation of 95% (n=300) and intra-examiner agreement (Kappa 0.6-1.0). The prevalence of one form of malocclusion was 71.3%. Crowding in one or two incisal segments [OR=2.8 (1.6-4.9)] and overjet [OR=2,4 (1.4-4.3)] were risk factors associated with perceived aesthetic dissatisfaction. Adolescents who had anterior mandibular irregularity [OR=3.3 (1.6-6.9)], overjet [OR=1.7 (1.1-3.0)] and anterior diastema [OR=3.1 (1.4-6.9)] revealed the highest self-perceived need for orthodontic treatment. CONCLUSIONS There are different degrees of objectively defined occlusal problems acceptable in the population which interfere with their decision to seek treatment and have a direct impact on the need of care. Subjective measures should be incorporated to clinical criteria currently used.

Impacto da cárie e da fluorose dentária na satisfação com a aparência e com a mastigação de crianças de 12 anos de idade

It has been suggested that psychosocial dimensions be used to assess the impact of oral health on peoples quality of life. The purpose of this study was to measure the impact of dental caries and dental fluorosis on childrens self-perception of appearance and mastication. A dental caries and dental fluorosis cross-sectional study was conducted with twelve-year-old schoolchildren (n = 695) in Chapec , Santa Catarina, Southern Brazil. A questionnaire to assess dental care and dental habits was applied. Risk factors were identified using unconditional multiple logistic regression analysis. None of the variables investigated was associated with dissatisfaction in relation to self-perceived appearance. Independent risk factors for dissatisfaction with self-perceived mastication included enrollment in a public school, caries in an anterior tooth and at least two posterior teeth, and female gender. By way of conclusion, dental fluorosis had no impact on childrens self-perception of appearance. Increased severity of dental caries was associated with greater self-perception of impact on chewing. This impact was greater in public schoolchildren, suggesting that there are different values and perceptions between socially different groups.

Determinantes sociais e biológicos da cárie dentária em crianças de 6 anos de idade: um estudo transversal aninhado numa coorte de nascidos vivos no Sul do Brasil

OBJETIVO: O objetivo deste estudo foi investigar os determinantes sociais e biologicos, medidos no periodo perinatal e na infância, sobre a ocorrencia de carie dentaria em criancas de 6 anos de idade. METODOS: Foi realizado um estudo transversal de carie dentaria em 1999, em uma subamostra de 400 criancas pertencentes a uma coorte de nascidos vivos iniciada em 1993, em Pelotas, RS. Os indices e criterios de diagnostico adotados foram os preconizados pela Organizacao Mundial da Saude (1997). Os resultados do estudo de carie foram inseridos no banco de dados da coorte, que continha informacoes sobre as condicoes sociais e de saude coletadas ao nascimento, no primeiro, terceiro, sexto e decimo segundo meses e no sexto ano de vida da crianca. O indice de carie (ceo-d = 0 ou > 1) foi a variavel dependente. Foram realizadas analises univariada e de regressao logistica multipla nao condicional, tendo como base um modelo teorico hierarquico de determinacao. RESULTADOS: Escolaridade materna abaixo ou igual a 8 anos, renda familiar menor que 6 salarios minimos, nao frequentar pre-escola e consumo de doces pelo menos uma vez ao dia aos 6 anos de idade foram fatores de risco a carie. CONCLUSOES: Os fatores de risco sociais, como baixa escolaridade materna e baixa renda familiar, nao frequentar a pre-escola e dieta inadequada sao comuns a carie dentaria e outras doencas e agravos infantis, sugerindo que medidas de intervencao dirigidas a estes fatores seriam mais adequadas a prevencao da carie do que medidas especificas.
