Is emotional dissonance more prevalent in oncology care? Emotion work, burnout and coping.

Objectives: Emotional burden on oncology care workers is considerable. These workers develop confidential relationship with the patient through interpersonal communication, which entails managing their own emotions as well as the emotions displayed by their patients, and it involves a great deal of emotion work. The objectives in our study were to assess the prevalence of burnout and emotional dissonance and to investigate the interrelationship among burnout, emotion work and coping in oncology care.

The attitude and knowledge of intensive care physicians and nurses regarding organ donation in Hungary--it needs to be changed.

BACKGROUND The education of intensive care professionals can influence the number of transplantable organs. The aim of this cross-sectional study is to estimate the attitude and knowledge of intensive care staff as about organ donation. MATERIAL/METHODS The self-completed questionnaire was completed at the Congress of the Hungarian Society of Anesthesiology and Intensive Therapy in 2011. Data, including attitudes about donation, attendance in an organ donation course, donation activity, self-reported knowledge of donor management, legislation, transplantation, and aftercare were collected from intensive care specialists (n=179) and nurses (n=103). RESULTS An organ donation course was attended by 53.6% of physicians and 16.7% of nurses (p=0.000); the 59% of doctors and 64.7% of nurses who did not participate in education were not willing to do so. Older staff were more likely to attend the course (p<0.01). Organ donation activity was not influenced by age or type of staff (physician or nurse), but it was higher among staff who attended training (p<0.01). Independently from accepting the presumed consent legislation (91.1%), 66% of intensive care professionals supported the practice of requesting the consent of family for organ retrieval. Self-reported knowledge regarding the Eurotransplant, donor management, the law and ethics of donation, transplantation, and after care for transplanted patients was influenced by age, donation activity, education, type of staff (p<0.01). CONCLUSIONS Education, including knowledge concerning brain death, donor management and communication with family, needs to be part of the specialist training of intensive care professionals, with a refresher course every fifth year.

Attitudes toward death in Hungary using the multidimensional fear of death scale

Abstract Background The aim of this study was to investigate the relationship between the attitudes displayed towards death by the youth and their mental health in the Hungarian population and to compare the results to data published in the literature. Our goal was to examine changes in the way youth conceived death, furthermore to examine the way youth perceived their own death within a small generational comparison study. Methods In order to measure the degree and possible dimensions of the fear of death in Hungarian young the Multidimensional Fear of Death Scale (MFODS) was used. The samples came from different socio-economic groups in Budapest, the capital of Hungary. Results The two highest anxiety factors were Fear for Significant Others and Fear of the Dying Process in all the examined groups. A significant difference in the overall fear of death was detected between the young and middle aged groups (p = 0.000) but the most characteristic difference was found between the young and older age groups....

Health behavior of medical students. Introduction of mental hygienic skills in the medical education in Hungary

UNLABELLED HuMania Training Programme is a voluntary peer training course. AIM to measure the efficiency of HuMania Training Programme and to analyse social relations of medical students. METHODS WHO Well-being Index, Bergen Social Relationship Scale, Lazaruss Ways of Coping Questionnaire and the Maslach Burnout Inventory for undergraduate students were used. The results of HuMania Training Programme established in 2009 were compared with those obtained from controls and HuMania Training Programme of the forthcoming year. RESULTS At the beginning, participants of the HuMania Training Programme had fewer well-being scores (p = 0.01), whereas their social anxiety (p = 0.01) and emotion-based coping (p = 0.05) were greater as compared to controls. After the HuMania Training Programme course, well-being showed an increasing tendency (p = 0.072), and burnout-level decreased (p = 0.040). Fifty percent of medical students had a minimum of three stressors, and 57% of them said that there was a person in their environment, who needed help, but they cannot provide it. CONCLUSIONS Medical students display poor mental hygienic state. and HuMania Training Programme tries to fight this situation.

Bevezetes: A halaltol valo felelem es a foglalkozas kozotti kapcsolatrol viszonylag keves szakirodalmi adat all rendelkezesre. Celkitűzes: A szerzők a foglalkozas es a halalfelelem osszefuggeseinek vizsgalatat tűztek ki celul. Modszer: Orvosok, orvostanhallgatok es egyeb egeszsegugyi dolgozok, lelkeszek, pszichologusok es egyeb, nem egeszsegugyi dolgozok (N = 1062) halallal kapcsolatos attitűdjeit elemeztek a Multidimenzionalis Halalfelelem Skala segitsegevel. Eredmenyek: Szignifikans kulonbsegeket talaltak az osszesitett halalfelelem-pontszam tekinteteben, valamint egyes vizsgalt csoportok eseteben is a halalfelelem kulonboző faktorait illetően. A halalfelelem a lelkeszek koreben volt a legalacsonyabb. A holttesttől valo felelem a pszichologusoknal es a nem egeszsegugyi dolgozoknal volt a legmagasabb, akiknek nincs kozvetlen tapasztalatuk holttesttel, illetve haldoklokkal. A kommunikáció nehézségei daganatos gyermekek szüleivel a palliatív ellátásra történő áttérés során

INTRODUCTION Adequate communication by medical personnel is especially important at certain points during the treatment of childhood cancer patients. AIM To investigate the timing and manner of communication with parents concerning the introduction of palliative care in pediatric oncology. METHOD Structured interviews, containing 14 questions, were carried out with physicians working in pediatric oncology (n = 22). Codes were generated inductively with the aid of Atlas.ti 6.0 software. RESULTS Interviews show a tendency of a one-step transition to palliative care following curative therapy. Another expert is usually involved in communication, most likely a psychologist. Regarding communication, there are expressions utilized or avoided, such as expressing clarity, self-defense and empathy. The communication of death and dying was the most contradictory. CONCLUSION This was the first investigation regarding communication in pediatric palliative care in Hungary. Our results show that a modern perspective of palliative communication is present, but necessitates more time to become entrenched. A túlélők mindennapjai. A hospice-ellátásban dolgozók életminősége

Introduction Each year, the number of hospice services and terminally ill patients increase while the number of hospice workers is falling. The intensification of the physical and mental burdens of the latter can lead to burnout and the fluctuation of the workers. Aim The aim of the authors was to survey the physical and mental state of hospice workers, as well as the risk of burnout and coping strategies. Method A questionnaire survey in hospice experts and volunteers (n ≈ 1500) based on the Hungarostudy survey was performed. Results Those who filled in the questionnaire (n = 195) had on average 1.86 workplaces and 45.8% of them reported working 12 or more hours a day. Most often, they suffered from lack of energy (65.1%), disturbing physical pain (46.9%), sleeping (56.9%) and digestion (35%), they considered themselves overweight (56.9%) and they were occupied with work problems even at bedtime (72.8%). Conclusions Hospice workers are overloaded, they have physical and mental symptoms and they are characterized by compassion fatigue. At the same time, they are also characterized by compassion satisfaction as an ability to accept appreciation for the caretaking.

[The current state, the possibilities and difficulties of palliative and hospice care in Hungary].

Hospice-palliative care has existed in Hungary for more than 20 years but physicians know very little about it. The objective of the study is to give detailed practical information about the possibilities and the reasonability of hospice care and the process of how to have access to it. The authors review and analyze the database of the national Hospice-Palliative Association database to provide most recent national data on hospice-palliative care. In addition, legal, financial and educational issues are also discussed. At present there are 90 active hospice providers in Hungary, which provide service for more than 8000 terminally ill cancer patients. According to WHO recommendations there would be a need for much more service providers, institutional supply and more beds. There are also problems concerning the attitude and, therefore, patients are admitted into hospice care too late. Hospice care is often confused with chronic or nursing care due to lack of information. The situation may be improved with proper forms of education such as palliative licence and compulsory, 40-hour palliative training for residents. The authors conclude that a broad dissemination of data may help to overcome misbeliefs concerning hospice and raise awareness concerning death and dying.A hospice-palliativ ellatas tobb mint 20 eve letezik Magyarorszagon, de az orvosok meg keveset tudnak rola.
