Sarcoidosis and its otolaryngological implications.

Sarcoidosis and its aetiopathogenesis, epidemiology and diagnostic procedures (including the Kveim reaction) are presented in this paper. The clinical manifestations of this disease, especially in otolaryngological organs, including the larynx, salivary glands, nose and paranasal sinuses, are described. Treatment procedures, including surgical interventions and prognosis, are also discussed.

Kikuchi–Fujimoto and Kimura diseases: the selected, rare causes of neck lymphadenopathy

Kikuchi–Fujimoto disease (KFD), also known as histiocytic necrotizing lymphadenitis, is a benign and self-limiting disease typically characterized by the enlargement of regional lymph nodes and accompanied by fever. KFD affects predominantly young adult females of Asian origin and is rarely seen in European countries, where it may cause diagnostic difficulties. Kimura disease is a rare and benign chronic inflammatory soft tissue disorder of unknown origin, characterized by a triad of painless subcutaneous masses in the head or neck region accompanied by regional lymphadenopathy, blood and tissue eosinophilia, and markedly elevated serum immunoglobulin E levels. Although most cases of Kimura disease have originated in China, Japan or Southeast Asia, there have been sporadic case reports from Europe and America. Herein, we review in detail the clinical presentations, complications and current concepts in the pathogenesis, diagnosis and treatment of these diseases.

Current view on nasal polyps management in Samter's triad patients

UNLABELLED The nasal polyps associated with Samters triad are often very extensive, difficult to treat, with great tendency to recurrence. In this paper the current opinion on nasal polyps management in aspirin triad patients was presented. PATHOGENESIS Opinions on pathogenesis of these disease was remembered as well as its epidemiology. DIAGNOSTIC METHODS The available diagnostic methods were presented. Treatment options: The available preservative treatment options was analyzed including aspirin desensitization. The role of surgical treatment, functional endoscopic sinus surgery was analyzed.

Kikuchi-Fujimoto disease as a rare cause of lymphadenopathy--two cases report and review of current literature.

Kikuchi-Fujimoto disease (KFD), also known as histiocytic necrotizing lymphadenitis, is a benign and self-limited disease, characterized typically by enlargement of regional lymph nodes accompanied by fever. KFD affects predominantly young adult females of Asian origin and is rarely seen in European countries, where it may cause diagnostic difficulties. Two cases of KFD in a 33 and 27-year-old woman with mild fever, malaise, lymphadenopathy initially misdiagnosed for indolent non-Hodgkins lymphoma was presented. The definitive diagnosis was established on the basis of histopathological examination of totally excised cervical lymph nodes. The propriety diagnosis allowed us to avoid inappropriate chemotherapy. The disease course in our patient was uneventful during the 1.5 and 12-year follow-up period. The clinical presentations, complications as well as current concepts on pathogenesis, diagnosis and treatment of the Kikuchi-Fujimoto disease was briefly reviewed in this paper. The need of a long-term follow-up of patients with Kikuchi-Fujimoto disease was emphasized.

An updated view on tuberculous lymphadenitis in the context of HIV epidemic as well as multidrug and extensively drug-resistant tuberculosis.

BACKGROUND An updated view on tuberculous lymphadenitis was presented in this paper. OBJECTIVES This article describes tuberculous lymphadenitis as the most common presentation of extrapulmonary tuberculosis and reviews its epidemiology, clinical aspects, diagnosis and treatment methods. The role of surgical procedures was also underlined. MATERIAL AND METHODS Since the 1990s, the incidence and severity of tuberculosis - particularly the prevalence of extrapulmonary tuberculosis - are increasing internationally, mainly due to the HIV epidemic and the emergence of multidrug-resistant and extensively drug-resistant strains. Hence, tuberculosis is once again a serious global problem not only in the developing countries of Africa or South East Asia, but also in highly-developed communities. CONCLUSIONS The latest data suggest that tuberculosis should be included in the list of differential diagnoses for all neck lymph node involvements, even in the absence of pulmonary involvement, in HIV-positive patients. If the otolaryngologist maintains a high index of suspicion, an early diagnosis can be made to contribute to more successful outcome of the disease.

Leczenie inwazyjnej grzybicy zatok przynosowych – opis przypadku ☆

The frequency of fungal etiology of sinusitis contains between 9 and 49 % according to different authors. Fungal rhinosinusitis has been divided into noninvasive and invasive forms. Noninvasive infections include fungus ball (mycetoma) and allergic fungal rhinosinusitis. Invasive infections include acute (or fulminates) fungal rhinosinusitis and invasive chronic rhinosinusitis. The case of chronic invasive rhinosinusitis was presented. Patient performed surgery--Functional Endoscopic Sinus Surgery and paranasal sinuses operation from subcranial approach. Surgery procedures were followed by antifungal pharmacotherapy (Orungal and Worykonazol). The presented case of fungal invasive chronic rhinosinusitis was treated according to recommended procedures with surgery followed by adequate antifungal farmacotherapy. Leczenie chirurgiczne przewlekłego zapalenia zatok przynosowych u dzieci

Summary Introduction Chronic sinusitis presents important problem in children. General approach in chronic sinusitis in children consists of: findings the proper diagnosis, optimal pharmacotherapy (oral or intravenous antibiotics), treatment coexisting allergic or systemic diseases and considering employing surgical treatment. The main surgical procedures in the treatment of chronic sinusitis in children are adenoidectomy and functional endoscopic sinus surgery (FESS). Material and methods 32 children with chronic sinusitis treated in ENT Department Silesian Medical University in Zabrze in the years 2000–2005 were presented in this study. 26 patients were treated surgically and 6 children by pharmacotherapy. In the surgically treated patients we performed 10 Functional Endoscopic Sinus Surgery, 4 unilateral and 8 bilateral sinusoscopy of maxillary sinus, and in 4 cases we removed choanal polyps. Results In the group of 26 surgically treated children we did not observed the recurrence of symptoms in the 24 months follow-up period.

Immunohistochemiczna ocena molekuły Ki67 w tkance polipów nosa

Summary Immunohistochemical assessment of proliferation marker – Ki67 in nasal polyps tissue and in healthy nasal mucosa was performed. There was no statistically significant difference between percentage of Ki67 positive cells in nasal polyps and nasal mucosa. These cells were observed mainly in epithelial layer and few in subepithelial layer. In examined group Ki67 expression was higher in planoepithelial metaplasia. Assessment of cells proliferation processes could be sensitive and early indicator of neoplasmatic transformation. Examined subgroup in which Ki67 expression was also higher, was subgroup with eosinophilic infiltrations. Cells proliferation processes in nasal polyps tissue are very important occurrence and require further examinations.

The presence of fungal floras in sinuses in chronic sinusitis patients with polyps

Several fungal species are known to cause severe respiratory and cutaneous diseases. The aim of this work was to analyze the fungal flora in maxillary sinuses in patients suffering from chronic sinusitis with polyps. Twenty five patients (11 woman, 14 man) who underwent endoscopic surgical procedures were included into the study. During the operation, the smear from the maxillary sinus were obtained for the incubation. In 6 persons fungal floras was present.

Ziarniniakowatość Wegenera o różnym początku jako problem kliniczny

Streszczenie Ziarniniakowatośc Wegenera jest martwiczym ziarniniakowym zapaleniem naczyn o nieznanej etiologii. W proces zapalny wciągniete są naczynia roznych narządow, szczegolnie naczynia ukladu oddechowego i nerek. Jest to choroba źle rokująca, przebiegająca z okresami remisji oraz nawrotami o wysokiej aktywności.
