Prevalence, severity and correlates of alcohol use in adult renal transplant recipients

Abstract: Background: Severe alcohol use is recognized as a major public health concern, even though light to moderate alcohol use might have beneficial effects on health. Alcohol use has been studied to some extent in solid organ transplant populations, yet evidence is lacking on alcohol use and its correlates in the renal transplant population. The aim of this study was therefore to determine the prevalence, severity and correlates of alcohol use in renal transplant recipients.

Symptom Management in HIV/AIDS: A Mixed Methods Approach to Describe Collaboration and Concordance Between Persons Living With HIV and Their Close Support Persons

For persons living with HIV, effective symptom management is crucial for good health outcomes and usually involves close support persons. The collaboration of persons living with HIV and their close support persons in the process of symptom management, including symptom experience reports, was investigated using an exploratory mixed methods design. Integration of methods included hypothesis generation from participants’ narratives. Results revealed that collaboration is constituted by distinct but integrative positions of manager and partner that are reflected in diverse themes of symptom management and confirmed in quantitatively assessed symptom reports. Divergent qualitative and quantitative findings highlighted problems in neurocognitive symptom communication. Symptom management processes should be supported by better integrating the close support persons into clinical service, and further research on neurocognitive symptom experience is needed.

[An evidence-based guideline to support HIV/AIDS patients with alcohol problems].

Aufgrund der Praxiserfahrung wissen wir, dass auch Menschen mit einer HIV/AIDS-Infektion davon betroffen sind, genaue Zahlen gibt es bis jetzt aller...It is estimated that 300,000 people with alcohol problems and alcohol-related illnesses are presently living in Switzerland. Many of these are infected with HIV. Yet, the number is unknown. At the HIV outpatient department of the University Hospital in Basel, nurses are assessing patients and it is evident that alcohol intake is a serious health problem for some of them. In order to provide evidenced-based care to these patients, a clinical practice guideline was developed based on a comprehensive literature review and synthesis. The guideline has been implemented and helps nurses and other healthcare providers to effectively assess and provide a short-term intervention to those patients with HIV who have alcohol-related problems. For the assessment, two instruments are utilized: the AUDIT and the CAGE. These instruments were selected after evaluation as being the most sensitive and specific for patients in an ambulatory setting. Patients are also assessed for their readiness to change. In this article, the literature will be reviewed and the guideline introduced, as a basis for nurses and other health care providers to provide evidence-based care for HIV-infected patients who have alcohol-related problems.

Zusammenfassung. Hintergrund: Malnutrition ist ein Phanomen, das bei Patient(inn)en in Gesundheitsinstitutionen haufig vorkommt und fur die Betroffenen mit negativen gesundheitlichen Konsequenzen verbunden ist. In vielen Spitalern wurden deshalb Malnutritionsprogramme entwickelt und etabliert, jedoch in der Regel ohne die Perspektive der Betroffenen miteinzubeziehen. Ob ein Programm den tatsachlichen Bedurfnissen der Betroffenen entspricht, kann bis heute lediglich aus Alltagsgesprachen abgeleitet werden. Ziel: Deshalb war das Ziel der Studie, die Bedurfnisse von Betroffenen in Bezug auf ihr Malnutritionsmanagement und ihr Erleben der angebotenen Ernahrungsinterventionen zu explorieren. Methode: Es wurde ein qualitativer, induktiver Ansatz gewahlt. Qualitäts- und Praxisentwicklung in Schweizer Akutspitälern durch Studienergebnisse - Eine Follow-up-Befragung von Pflegedirektor(inn)en der RN4CAST Spitäler.

Im Rahmen der RN4CAST (Nurse forecasting in Europe) Studie erhielten die 35 teilnehmenden Schweizer Spitaler einen Studienbericht, der ihnen einen anonymisierten Vergleich mit den teilnehmenden Spitalern in Bezug auf Struktur-, Prozess- und Outcome-Faktoren ermoglichte. Ruckmeldung zu Studienergebnissen im Rahmen dieses Benchmarking ermoglichte den Spitalern Verbesserungspotenziale aufzudecken und entsprechende Masnahmen zur Qualitats- und Praxisentwicklung zu ergreifen. Wir befragten die Pflegedirektionen der RN4CAST Spitaler, ob die Ergebnisse des Studienberichts in ihrem Spital diskutiert und analysiert wurden, und ob die Ergebnisse zu Praxis- und Qualitatsentwicklungsprojekten – rund 18 Monate nach Ubermittlung des Studienberichts – gefuhrt haben. Von den 35 versandten Fragebogen wurden 30 retourniert (Rucklaufquote = 85 %). Der Studienbericht wurde in 27 Spitalern (90 %) spitalintern diskutiert und analysiert, wobei die Qualitat (n = 27) und der Nutzen (n = 22) als gut oder sehr gut beurteilt wurden....

Einführung der geburtshilflichen Akupunktur im Universitätsspital Basel

Mit unserem Praxisentwicklungsprojekt entsprachen wir dem Wunsch vieler Frauen, Akupunktur als unterstutzende Masnahme wahrend der Geburt nutzen zu konnen. Die Aufteilung in vier Projektphasen war die Grundlage fur den reibungslosen Ablauf und die anhaltend sehr gute interdisziplinare Zusammenarbeit.
