BackgroundDue to the absence of a current and validated food frequency questionnaire (FFQ) for use in New Zealand adolescents, there is a need to develop one as a cost-effective way to assess adolescents’ food patterns. This study aims to examine the test-retest reliability and relative validity of the New Zealand Adolescent FFQ (NZAFFQ) to assess food group intake in adolescents aged 14 to 18 years.MethodsA non-quantitative (without portion size), 72-item FFQ was developed and pretested. Fifty-two participants (aged 14.9 ± 0.8 years) completed the NZAFFQ twice within a two-week period for test-retest reliability. Forty-one participants (aged 15.1 ± 0.9 years) completed a four-day estimated food record (4DFR) in addition to the FFQs to enable assessment of validity. Spearman’s correlations and cross-classification analyses were used to examine relative validity while intra-class correlations were additionally used for test-retest reliability.ResultsWeekly intakes were estimated for each food item and aggregated into 34 food groups. The median Spearman’s correlation coefficient (SCC) between FFQ administrations was 0.71. SCCs ranged from 0.46 for fruit juice or cordial to 0.87 for non-standard milk. The median intra-class correlation coefficient (ICC) between FFQ administrations was 0.69. The median SCC between food groups from the FFQ and the 4DFR was 0.40 with the highest SCC seen for standard milk (0.70). The exact agreement between the methods in ranking participants into thirds was highest for meat alternatives (78%), but lowest for red or yellow vegetables and potatoes (27%). The mean percent of participants misclassified into extreme thirds for food group intake was 12%.ConclusionsDespite a small sample size, the NZAFFQ exhibited good to excellent short-term test-retest reliability and reasonable validity in ranking the majority of the food group intakes among adolescents aged 14 to 18 years. The comparability of the validity to that in the current literature suggests that the NZAFFQ may be used among adolescent New Zealanders to identify dietary patterns and rank them according to food group intake.

OBJECTIVES The attenuation of the number and severity of infections is of importance to athletes. Probiotics use has increased over recent years with beneficial effects believed to include improvements in immune function. Research has focused on their effectiveness for reducing the number, duration and severity of infections amongst endurance athletes. At present no research has been undertaken with team sport athletes. This randomised controlled trial aimed to determine the effectiveness of probiotics on the number, duration and severity of infections amongst elite union rugby players. DESIGN Randomised control trial with two arms; placebo and probiotic. METHODS Thirty elite rugby union players were allocated in random order to receive a probiotics supplement or a placebo for four weeks each. Supplements were consumed on a daily basis. There was a four week washout period between treatments. Participants completed a daily diary to identify and rate the severity of any infectious symptoms. RESULTS During the probiotic treatment 14/30 participants never experienced a single upper respiratory tract illness (URTI) or gastrointestinal (GI) episode, compared to 6/30 on the placebo supplementation (p=0.033). The mean±standard deviation for the number of days of illness tended to be higher for the placebo, (5.8±6.6 days) than probiotic (3.4±4.6 days), (p=0.054). There was no significant difference in the severity of the symptoms between the two treatment groups (p=0.110). CONCLUSIONS These positive effects of probiotic supplements provide evidence for the beneficial effects of daily supplementation with these probiotic strains in highly trained rugby union players.

BackgroundAssociations between food choice and body composition in previous studies of adolescents have been inconsistent. This may be due to the body composition measures used, or these associations may be affected by the dieting status of adolescents. The objective of this study was to investigate the association between dietary patterns and body composition in adolescents, and determine if these associations are moderated by dieting status.MethodsInformation on food consumption and current dieting status was collected, using a web-based survey, in 681 adolescents (mean age 15.8 (SD 0.9) years) from schools in Otago, New Zealand. Non-dieters were defined as those reporting not being on a diet as they were “happy with their weight”. Principal components analysis (PCA) was used to determine dietary patterns. Body mass index (BMI), waist circumference (WC), waist-to-height ratio (WHtR), fat mass index (FMI), and fat-free mass index (FFMI) were examined as outcomes. Generalized estimating equations were used to examine associations between dietary patterns and body composition.ResultsPCA produced three dietary patterns: ‘Treat Foods’, ‘Fruits and Vegetables’, and ‘Basic Foods’. A standard deviation increase in ‘Basic Foods’ was associated with a 3.58% decrease in FMI (95%CI −6.14, -0.94) in the total sample. When separate sex analysis was undertaken significant negative associations were found in boys only, between the ‘Basic Food’ score and WC, WHtR, FMI, and FFMI, while the ‘Fruits and Vegetables’ pattern was negatively associated with FMI. Associations between ‘Treat Foods’ and BMI, WC, and WHtR in non-dieters were positive, while these associations were negative for all other participants.ConclusionsSignificant associations were found between dietary patterns and indices of both central and total adiposity, but not BMI. Therefore using only BMI measures may not be useful in this age group. Since our results were significant for boys and not girls, nutrition messages designed to prevent obesity may be particularly important for adolescent boys. As an interaction between dieting status and ‘Treat Foods’ existed, future studies should also explore the role of dieting when investigating food choice and body composition.

Cardiovascular complications have emerged as a major concern for cancer patients. Many chemotherapy agents are cardiotoxic and some appear to also alter lipid profiles, although the mechanism for this is unknown. We studied plasma lipid levels in 12 breast cancer patients throughout their chemotherapy. Patients received either four cycles of doxorubicin and cyclophosphamide followed by weekly paclitaxel or three cycles of epirubicin, cyclophosphamide and 5’-fluorouracil followed by three cycles of docetaxel. Patients demonstrated a significant reduction (0.32 mmol/L) in high density lipoprotein cholesterol (HDL-C) and apolipoprotein A1 (apoA1) levels (0.18 g/L) and an elevation in apolipoprotein B (apoB) levels (0.15 g/L) after treatment. Investigation of the individual chemotherapy agents for their effect on genes involved in lipoprotein metabolism in liver cells showed that doxorubicin decreased ATP binding cassette transporter A1 (ABCA1) via a downregulation of the peroxisomal proliferator activated receptor γ (PPARγ) and liver X receptor α (LXRα) transcription factors. In contrast, ABCA1 levels were not affected by cyclophosphamide or paclitaxel. Likewise, apoA1 levels were reduced by doxorubicin and remained unaffected by cyclophosphamide and paclitaxel. Doxorubicin and paclitaxel both increased apoB protein levels and paclitaxel also decreased low density lipoprotein receptor (LDLR) protein levels. These findings correlate with the observed reduction in HDL-C and apoA1 and increase in apoB levels seen in these patients. The unfavourable lipid profiles produced by some chemotherapy agents may be detrimental in the longer term to cancer patients, especially those already at risk of cardiovascular disease (CVD). This knowledge may be useful in tailoring effective follow-up care plans for cancer survivors.

Abstract Cosgrove, SD, Love, TD, Brown, RC, Baker, DF, Howe, AS, and Black, KE. Fluid and electrolyte balance during two different preseason training sessions in elite rugby union players. J Strength Cond Res 28(2): 520–527, 2014—The purpose of this study was to compare fluid balance between a resistance and an aerobic training sessions, in elite rugby players. It is hypothesized that resistance exercise will result in a higher prevalence of overdrinking, whereas during the aerobic session, underdrinking will be more prevalent. As with previous fluid balance studies, this was an observational study. Twenty-six players completed the resistance training session, and 20 players completed the aerobic training session. All players were members of an elite rugby union squad competing in the southern hemispheres premier competition. For both sessions, players provided a preexercise urine sample to determine hydration status, pre- and postexercise measures of body mass, and blood sodium concentration were taken, and the weight of drink bottles were recorded to calculate sweat rates and fluid intake rates. Sweat patches were positioned on the shoulder of the players, and these remained in place throughout each training session and were later analyzed for sodium concentration. The percentage of sweat loss replaced was higher in the resistance (196 ± 130%) than the aerobic training session (56 ± 17%; p = 0.002). Despite this, no cases of hyponatremia were detected. The results also indicated that more than 80% of players started training in a hypohydrated state. Fluid intake seems to differ depending on the nature of the exercise session. In this group of athletes, players did not match their fluid intakes with their sweat loss, resulting in overdrinking during resistance training and underdrinking in aerobic training. Therefore, hydration strategies and education need to be tailored to the exercise session. Furthermore, given the large number of players arriving at training hypohydrated, improved hydration strategies away from the training venue are required.

BackgroundSun safety and vitamin D status are important for prolonged health. They are of particular interest to those working with athletes for whom for whom safe sun practices maybe limited.The aim of this cross-sectional study was to describe the attitudes of elite New Zealand athletes to both vitamin D and sun exposure.Methods110 elite New Zealand outdoor athletes volunteered to participate in an interview with a trained interviewer. The interviewer asked the athletes questions on their Vitamin D knowledge, attitudes and practices regarding sun exposure as well as their concerns about skin cancer.ResultsAthletes were more concerned about their risk of skin cancer (66%) than their vitamin D status (6%). Although the majority (97%) were aware of Vitamin D and could identify the sun as a source (76%) only 17% could name another source of Vitamin D.Only 10 (9%) reported always applying sunscreen before going out in the sun. No athlete reported reapplying sunscreen every hour and 25 suggesting that they never reapply sunscreen.ConclusionsAthletes are concerned about skin cancer however, their use of sunscreen is not optimal suggesting reapplication of sunscreen could be targeted in order to reduce the risk of sun cancer. Awareness of sources of Vitamin D other than the sun may also need to be improved potentially through educational interventions and possibly in conjunction with sun smart messages.

Previous research investigating the relationship between parents’ and children’s diets has focused on single foods or nutrients, and not on global diet, which may be more important for good health. The aim of the study was to investigate the relationship between parental diet quality and child dietary patterns. A cross-sectional survey was conducted in 17 primary schools in Dunedin, New Zealand. Information on food consumption and related factors in children and their primary caregiver/parent were collected. Principal component analysis (PCA) was used to investigate dietary patterns in children and diet quality index (DQI) scores were calculated in parents. Relationships between parental DQI and child dietary patterns were examined in 401 child-parent pairs using mixed regression models. PCA generated two patterns; ‘Fruit and Vegetables’ and ‘Snacks’. A one unit higher parental DQI score was associated with a 0.03SD (CI: 0.02, 0.04) lower child ‘Snacks’ score. There was no significant relationship between ‘Fruit and Vegetables’ score and parental diet quality. Higher parental diet quality was associated with a lower dietary pattern score in children that was characterised by a lower consumption frequency of confectionery, chocolate, cakes, biscuits and savoury snacks. These results highlight the importance of parental modelling, in terms of their dietary choices, on the diet of children.

OBJECTIVE To examine the potential associations between diet quality and multiple measures of body composition in a sample of New Zealand adolescents aged 14-18 years. DESIGN Cross-sectional survey of eleven high schools in Otago, New Zealand. Each participant completed an online FFQ and a New Zealand Diet Quality Index for Adolescents (NZDQI-A) score was calculated based on variety and adequacy of intake for five major food groups. Besides height and waist circumference measurements, body composition was assessed using segmental bio-impedance analysis. Generalized estimating equations were used to examine associations between diet quality and body composition in models adjusted for sex, age, ethnicity and socio-economic status. SETTING High schools in Otago, New Zealand. SUBJECTS High-school students (n 681, 56 % male, mean age 16·1 (sd 1·5) years) participating in the Otago School Students Lifestyle Survey Two. RESULTS Higher NZDQI-A scores were significantly associated with lower body fat percentage (β=-0·19; 95 % CI -0·35, -0·04; P=0·014), fat-to-lean mass ratio (β=-0·26; 95 % CI -0·46, -0·05; P=0·016) and lower fat mass index (β=-0·23; 95 % CI -0·45, -0·004; P=0·046) after multivariate adjustment. No association was found between NZDQI-A and BMI, waist circumference or waist-to-height ratio. CONCLUSIONS Diet quality, as measured by NZDQI-A, was associated only with measures of body fat, not measures of overall body size. Measures specific to body fat should be used for more accurate ascertainment of body composition in examining the diet-body composition associations in this age group.

Using a sample of adolescents from schools in Otago, New Zealand, associations between food outlets around schools and dietary quality were investigated. Food outlet environment data were derived using GIS data. Multivariate regression analysis results showed that outlet density, in an 800m buffer around schools, of cafes and restaurants, supermarkets and takeaways was associated with higher Diet Quality Index scores in boys, and distance to nearest outlet for convenience stores, cafes and restaurants and supermarkets with lower scores for girls. Effect sizes were small, suggesting that the food environment around schools plays a minor role in adolescent diet quality.

Abstract Current hydration guidelines are designed to address the fine balance between minimising dehydration while reducing the risk of hyponatremia. During prolonged cycling events small discrepancies between drinking behaviour and fluid requirements may be detrimental to health and performance. The present study aimed to investigate the hydration practices of competitors in a 387 km cycle race in order to evaluate the effect on fluid balance and monitor the prevalence of dysnatremia. Eighteen participants provided blood and urine samples pre- and post-race to measure sodium and fluid balance. Sweat samples were collected via patches for analysis of sodium concentration. Body weight was measured at the start and end of the race. On average participants consumed 0.58 L h−1 of fluid. Upon completion of the race 7 of the 18 (39%) cyclists had blood sodium concentrations of 135 mmol L−1 or lower with one cyclist recording a value of 132 mmol L−1. Only two cyclists appeared to be moderately dehydrated. A post-race questionnaire indicated cyclists were most concerned with preventing dehydration. It appears that cyclists taking part in prolonged endurance events are at more risk of hyponatremia than dehydration and do not readily change their drinking behaviour to match their sweat losses.
