Cardiac functions of patients on chronic hemodialysis using echocardiography

慢性透析患者の心機能および心機能に影響を及ぼす心臓の器質的変化について, 心エコー図所見を中心に検討した. 対象は慢性血液透析患者197名で, 男性131名, 女性46名である. 平均年齢は45.7歳であった.結果は, 1) 心エコー図所見では, 左室壁肥厚が88例 (44.6%) と高率に認められた. 駆出率低下は13.2%, 心係数の低下は9.1%であった. 2) 壁肥厚の原因では高血圧の関与が強く示唆された. また糖尿病性腎症と悪性高血圧で高率に壁肥厚例が認められた点も注目された. 3) 壁肥厚症例数と透析期間とは必ずしも相関せず, 10年以上透析を受けた症例でも心エコー図上変化に乏しい症例も多かった. 逆に透析導入早期から著明な壁厚変化を示す例も認められ, 10年以前にこれらの多くが脱落すると推定された. 以上より, 透析導入早期から心エコー図所見を中心に異常所見を有する群と有さない群とに区別し, シャントの影響や, 透析自体の影響につき前者では厳密な管理と頻回の心血管系の検査を施行して行く必要があると思われた. 4) 8例にTl 201心筋シンチグラフィーを施行したが心筋障害の有無および程度の判定に極めて有用であった. 5) 少数例ながら施行した剖検例と心エコー図所見の対比では器質的変化の有無に関して, 心エコー図所見は十分信頼できる所見であった.
