A case of peripheral cementifying fibroma of the mandible

Peripheral cementifying fibroma is a rare tumor. It is classified as an osteogenic neoplasm by the WHO classification (1992). We encountered a 67 years old man who developed it on the buccal gingiva in the left lower posterior molar region of the mandible. The lesion was diagnosed clinically as epulis osteoplastica. The lesion was surgically excised under local anesthesia with first and second molar extractions. Histopathological examination indicated the lesion to be a peripheral cementifying fibroma. The post operative course was good.

Terminal care for oral cancer patients in hospital dentistry and oral and maxillofacial surgery at a local area

A sequential study concerning suicide of patients with oral cancer, notification of oral cancer, terminal care of patients with oral cancer at home, etc. has been performed in our department. As part of the study, a possible role of hospital dentistry and oral and maxillofacial surgery at a local area was examined recently with reference to the terminal alleviating care provided to three patients with oral cancer at Joetsu General Hospital.The result was as follows: It is natural that most patients want to die at home in the presence of their family. In order to fulfill such a desire, it is considered necessary that a hospital dentistry and oral and maxillofacial surgery must handle terminal alleviating care. However, many problems still remain in managing patients with terminal cancer in a hospital dentistry. But also a cooperation with a university hospital and cooperation with the department of dentistry in other hospitals, education activity concerning oral cancer at local medical treatment including within hospital care, effective utilization of local nursing station, and examination of the home care system, are considered essential.

This chemically induced carcinogenesis experiment was performed for purposes of preliminary experimenting for further oral carcinoma studies. Painting 0.5% DMBA in liquid paraffin into six-week-old Golden Syrian Hamsters buccal pouches, threetimes a week for 14 weeks, the following results were elucidated.Within the first 4 weeks, inflammatory reactions of the mucosa, i. e., redness, formation of plaques, and bleeding were found. Thereafter, they naturaly disappeared from around the 5th to the 6 th experimental weeks. On the 8 th week, papillomatous tumors suddenly appeared which were growing gradually in size, number, and even in atypism. The surrounding epithelia showed changes comparable to epithelial dysplasia or carcinoma in situ. It was from the 12-week-painted group that well-differentiated squamous cell carcinomas were-first recognized.These tumors were rather exophytic growths and showed a minimal invasive trend. However, invasive tumors were formed as well in the 14-week-painted group. Therefore, two possible ways for carcinoma formation might be suggested. One is from papilloma through increased atypism, such as verrucous carcinoma, and the other from epithelial dysplasia or carcinoma in situ with downward growth of the rete-processes to the carcinoma.
