laced catheter. Insertion of the cannula under direct visual ontrol with a fibreoptic bronchoscope and portable on-table ltrasound to identify variations in the vascular anatomy may inimise complications.3 It is also important that all personnel involved in the ostoperative care of the patient are aware that only a igh-pressure oxygen source will be effective in achieving dequate oxygenation through the cannula. A recent audit in ur department showed that most nursing and junior medical taff on the head and neck wards were unaware of how to set p the equipment needed for emergency jet ventilation.

Postoperative monitoring of microsurgical free tissue transfers for head and neck reconstruction: a systematic review of current techniques--part I. Non-invasive techniques.

Khalid Abdel-Galil; David A. Mitchell

A systematic review of the literature relating to methods of monitoring viability of microvascular free tissue transfers in the head and neck region was conducted. The aim of this review is to identify the best method of monitoring that would allow timely salvage of potentially failing free flaps. An analysis and description of the various studied techniques is also given. In this first part, non-invasive modalities are covered.

Fractures of the mandibular condyle: evidence base and current concepts of management

Khalid Abdel-Galil; Richard Loukota

Management of mandibular condylar fractures remains a source of ongoing controversy. While some condylar fractures can be managed non-surgically, recognition of fracture patterns that require surgical intervention and selection of an appropriate operative procedure are paramount to success in treating these injuries. The objective of this review is to appraise the current evidence regarding the effectiveness of interventions that are used in the management of fractures of the mandibular condyle.

Fixation of comminuted diacapitular fractures of the mandibular condyle with ultrasound-activated resorbable pins.

Khalid Abdel-Galil; Richard Loukota

We describe the fixation of bilateral comminuted diacapitular fractures of the mandibular condyle with ultrasound activated resorbable pins (KLS Martin SonicWeld Rx) by an open approach with primary fixation of the fractures using the resorbable osteosynthesis pins.

Buccal Mucosal Grafts for Urethroplasty: Comparison of Postoperative Oral and Perineal Morbidity

Khalid Abdel-Galil; Ian Eardley; Richard Loukota

INTRODUCTION A prospective study of postoperative oral and perineal pain experienced by a group of patients undergoing buccal mucosal graft harvest for urethral reconstruction. PATIENTS AND METHODS A consecutive group of 24 male patients undergoing buccal mucosa graft harvest for urethral reconstruction of stricture disease was prospectively studied between June 2006 and December 2008. All patients were examined pre-operatively and entered into the study prospectively. After surgery, all patients were reviewed at 24 h and 48 h. On both occasions, they were asked to complete a proforma containing visual analogue pain scales for both the oral donor site as well as the perineum. RESULTS A statistically significant higher level of pain was experienced from the perineum than the oral donor site on both the first and second postoperative days. CONCLUSIONS Comparative analysis of visual analogue pain scale scores between oral donor site and perineum showed that patients experience significantly more pain from the latter postoperatively.

Bilateral procedures in maxillofacial surgery in the United Kingdom: survey of higher trainees' performance and experience.

Khalid Abdel-Galil; Kandasamy Ganesan; John Russell

We conducted a survey targeting all higher surgical trainees in oral and maxillofacial surgery in the United Kingdom to find out their dominant hand, ability and confidence using powered instruments, and any trends in the likelihood of operating on one or other side of an index bilateral operation: the bilateral sagittal split mandibular osteotomy. We also explored the attitudes of trainees and trainers towards non-dominant manual dexterity, and whether senior consultants were more likely to adopt certain rehearsed routines and approaches when supervising trainees.
