Predicting Sasang Constitution Using Body-Shape Information

Objectives. Body measurement plays a pivotal role not only in the diagnosis of disease but also in the classification of typology. Sasang constitutional medicine, which is one of the forms of Traditional Korean Medicine, is considered to be strongly associated with body shape. We attempted to determine whether a Sasang constitutional analytic tool based on body shape information (SCAT-B) could predict Sasang constitution (SC). Methods. After surveying 23 Oriental medical clinics, 2,677 subjects were recruited and body shape information was collected. The SCAT-Bs for males and females were developed using multinomial logistic regression. Stepwise forward-variable selection was applied using the score statistic and Walds test. Results. The predictive rates of the SCAT-B for Tae-eumin (TE), Soeumin (SE), and Soyangin (SY) types in males and females were 80.2%, 56.9%, and 37.7% (males) and 69.3%, 38.9%, and 50.0% (females) in the training set and were 74%, 70.1%, and 35% (males), and 67.4%, 66.3%, and 53.7% (females) in the test set, respectively. Higher constitutional probability scores showed a trend for association with higher predictability. Conclusions. This study shows that the Sasang constitutional analytic tool, which is based on body shape information, may be relatively highly predictive of TE type but may be less predictive when used for SY type.

The Sasang Constitution as an Independent Risk Factor for Metabolic Syndrome: Propensity Matching Analysis

The Sasang Constitutional Medicine is a traditional Korean customized medicine that classifies people into four types: Tae-eumin (TE), Soyangin (SY), Soeumin (SE), and Taeyangin. The aim of this study was to show whether the Sasang Constitution (SC) could be an independent risk factor for the metabolic syndrome (MS). Totally, 3,334 subjects from 24 Korean medicine clinics participated in this study. A one-way ANOVA for the continuous variables and a chi-square test for the prevalence of MS were conducted. A logistic regression was conducted to calculate the propensity score and the odds ratios (ORs). The prevalence for MS in TE, SY, and SE was 50.6%, 30.9%, and 17.7% (P < 0.001) before matching, and 36.7%, 28.6% and 28.2% (P = 0.042) after matching, respectively. The TE was associated with an increased OR for MS compared with the SE and SY in both crude (OR 4.773, 95% CI 3.889–5.859, and OR 2.292, 95% CI 1.942–2.704, resp.) and matched groups (OR 1.476, 95% CI 1.043–2.089, and OR 1.452, 95% CI 1.026–2.053, resp.). This study reveals that the SC, especially the TE type, could be considered as a risk element for MS even in people with otherwise similar physical characteristics.

The Difference of Digestion, Sweat, Stool, Urination, Drinking, Coldness and Hotness Characteristics according to Sasang Constitutional Exterior and Interior Diagnosis

Abstract The Difference of Digestion, Sweat, Stool, Urination, Drinking, Coldness and Hotness Characteristics according to Sasang Constitutional Exterior and Interior Diagnosis Eun-Su Jang, Young-Hwa Baek, Ki-Hyun Park, Si-Woo Lee Korea Institute of Oriental Medicine 1. Objective It is well-known that even if some people are born with same constitution, their symptoms can be different according to Exterior and Interior diagnosis. This study aimed to suggest different clinical symptoms according to Exterior and Interior group in individual Sasang constitution. 2. Methods We collected 706 physiological and pathological data of subjects from August 2009 to July 2011 using case report form of Questionnaire. The Sasang constitutional diagnosis and Exterior and Interior diagnosis were conducted by Sasang constitutional experts. All data were analyzed with Chi-square test and significant p value was 0.05. 3. Results There are different symptoms between Exterior and Interior group in Taeeumin as followed, the frequence of famine and burp, the part of sweat, brown or not in color of stool, painful or not in evacuating, the frequence of loose feces, the frequence of abdominal inflating, the costive feeling frequence after evacuating, foam in urine or not, urination times, amount of Coldness and Hotness in belly, the frequence of turning fale in face. Soeumin as followed, digestion well or not, amount of sweat in exercise, the brown or not in color of stool, the frequence of turning fale in face, athe frequence of clearness in urine and feeling hot. Soyangin as followed, amount of appetite, amount of sweat in hot weather, sweat in neck or not, the red color in urine or not, Coldness and Hotness in foot or not, amount of drinking.

A Study on the Reliability Assessment of Sasang Constitution Questionnaire Developed by KIOM for Japanese

Abstract A Study on the Reliability Assessment of Sasang Constitution Qu estionnaire Developed by KIOM for Japanese Jong-Hyang Yoo, Eun-Su Jang, Yun-Yung Kim, Ki-Hyun Park, Si-Woo Lee Korea Institute of Oriental Medicine 1. Objectives This study aimed to assessing the reliability of questionnaire for Japanese as one of method to diagnose Sasang Constitution and evaluating the possibility of applying domestic questionnaire to foreign site. 2. Methods A cross-sectional survey approved by IRB (Independent Review Board) at Tohoku University was performed for 119 participants with informed consent at a particular city in Japan using self-reported questionnaires developed by KIOM (Korea Institute of Oriental Medicine). Among the entire samples, 112 were retested. The test-retest reliability was assessed by Kappa for normal variables and internal consistency was evaluated by Cronbach’s alpha. 3. Results Four of 77 questions are impossible to calculate consistency owing to unsymmetric matrix between primary reply and secondary one. Seventeen of them (22%) showed 0.4 or below in Kappa, 56 were ranged from 0.4 to 0.8 and no question marked 0.8 or over. Internal consistency was possibly checked for several questions about character, digestion, sweat, excrement, urine, cold-heat pattern. Cronbach’s alpha for questions except urine and cold-heat pattern was over 0.6

Informed consent to take part in the study was obtained from each of the participants. The performance of SCAT was assessed through Kappa coefficient and the concordance rate between SCAT and expert diagnosis.ResultsThe concordance rate between SCAT and expert was 61.1% in total and the Kappa was 0.408. When the constitutionalprobability increased, the concordance rate and Kappa showed a increasing tendency. Perception of cold and heat pattern identification in diseases: a survey of Korean medicine doctors

Background This study aimed to investigate the extent to which Korean Medicine doctors consider cold and heat pattern identification when prescribing herbal treatment for a disease. Methods A survey was sent by e-mail to 15,841 members of the Association of Korean Medicine for whom member information was registered. Of these, 699 (4.4%) members participated in the survey. The survey included questions regarding the frequency of use of cold and heat pattern identification in deciding a herbal treatment prescription, the diseases for which cold and heat pattern identification-related herbal treatment was most efficacious, the type of herbal treatment prescribed, and the duration of the treatment. Results Of the 699 respondents, 591 (84.5%) reported that they considered cold and heat when prescribing herbal treatment. The diseases for which consideration of cold and heat patterns was effective were, in order, menopausal disorder (124, 18.3%), chronic rhinitis (98, 14.5%), dyspepsia (94, 13.9%), hwa-byung (92, 13.6%), diarrhea (83, 12.3%), dysmenorrhea (61, 9.0%), headache (59, 8.7%), inflammation in the digestive tract (58, 8.6%), coldness in hands and feet (58, 8.6%), and atopic dermatitis (55, 8.1%). The typical treatment duration differed widely for different diseases: atopic dermatitis was most frequently treated for >2 months (38, 34.5%), whereas diarrhea was most frequently treated for ≤ 10 days (73, 43.6%). Conclusion These findings indicate that cold and heat pattern identification is a useful tool employed by Korean Medicine doctors. This study may provide a basis for clinical research investigating the effect of pattern identification-based treatment of diseases.

Correlation among Heat and Cold, Life Style, Stress and Quality of Life for Adult Female with Constipation

In this research, we diagnosed constipation of adult female by taking into characteristic consideration of Korean, and identified the correlation of a constipation with life style, heat and cold symptoms, stress and life quality of them. We also identified the relationships of the constipation with stress and life quality. A Survey was conducted from April 23, 2010 to October 25, 2010 for 464 women and the collected data was analyzed with computer software of SPSS 19.0. In the result, the constipation group appealed for cold stomach(p=.008), dyspepsia(p=.006), and tiredness(p=.001). more often than the normal group. The constipation group experienced Dreaming(p=.003), light sleeping(p=.007) and a trouble in sleeping(p=.001) more often, which impaired the quality of sleeping. Compared to the normal group, the constipation group also showed a tendency of high stress and low life quality. Based on the results, all the factors of heat and cold symptom, life style, stress and life quality should be considered when developing medications for the prevention and improvement of constipation.

Development and Validation of Brief KS-15 (Korea Sasang Constitutional Diagnostic Questionnaire) Based on Body Shape, Temperament and Symptoms

Methods The data of body shape, temperament and symptoms were collected from 11 nationwide multi-centers in Korean Medical Clinics between Nov. 2009 to Aug. 2012. KS-15 algorithm was developed through Multinomial Logistic Regression and was applied for test group to verify it’s classificatory function. Results The weight value of variables of KS-15 were different between male and female. The concordance of KS-15 with the expert in training group was 58.34% (72.8% in Tae-eumin, 54.8% in Soeumin, and 44.1% in Soyangin). The concordance of KS-15 with the expert in test group was 63.1% (63% in Tae-eumin, 83.3% in Soeumin, and 51.9% in Soyangin). Conclusions KS-15 seems to have suitable implement with practicality and validity. Further study is necessary to verify it accuracy and cut-off.

The effects of constitutional food preference on health status of the twenties

1. Objectives : This research aimed to understand the effects of food consumption that fits to the constitution of subjects on health status by recognizing the constitutional food preference according to Sasang constitution. 2. Methods : The participants who are college students in D city were invited from Mar. 17, 2009 to Sep. 5 2010 for the research. More than two medical doctors who are specialized in Sasang constitution diagnosed the constitution of participants, and the participants were selected as subjects only if the diagnosis of the two doctors were agreed. Total 422 subjects (Taeumin: 175, Soeumin: 101, Soyangin: 144) were selected from the diagnosis, and the collected data was analyzed with computer software of SPSS 17.0. The frequency analysis was executed for the general characteristics of subjects, and the relationship of constitutional food preference and health status (SF-36) were analyzed with Pearson Correlation Coefficient. 3. Results : From the results about the distribution of constitutional food preference according to the constitutions, Soeumin showed the highest frequency for both of lower than 10 points (35.0 %), which stands for high preference of constitutional food, and higher than 40 points (1.9 %), which stands for low preference of constitutional food. And, they showed no correlation for the results about the relationship between health status and constitutional food preference based on 9 sub-categories of SF-36 including Physical Component Score (PCS), Mental Component Score (MCS). 4. Conclusions : In conclusion, the effects of food consumption that fits to the constitution of the twenties on their health status were understood by recognizing the constitutional food preference according to Sasang constitution. Comparing with the previous research results that constitutional food consumption can enhance their health status, there were no correlation between health status and constitutional food preference. The discrepancy of results can be addressed to difficulties of group selection, constitutional food categorization and lack of constitutional food-related questionnaires. Thus, it is required to conduct a follow-up studies that were made up for the mentioned shortcomings.
