Although the hip musculature is found to be very important in connecting the core to the lower extremities and in transferring forces from and to the core, it is proposed to leave the hip musculature out of consideration when talking about the concept of core stability. A low level of co-contraction of the trunk muscles is important for core stability. It provides a level of stiffness, which gives sufficient stability against minor perturbations. Next to this stiffness, direction-specific muscle reflex responses are also important in providing core stability, particularly when encountering sudden perturbations.It appears that most trunk muscles, both the local and global stabilization system, must work coherently to achieve core stability. The contributions of the various trunk muscles depend on the task being performed. In the search for a precise balance between the amount of stability and mobility, the role of sensory-motor control is much more important than the role of strength or endurance of the trunk muscles. The CNS creates a stable foundation for movement of the extremities through co-contraction of particular muscles. Appropriate muscle recruitment and timing is extremely important in providing core stability.No clear evidence has been found for a positive relationship between core stability and physical performance and more research in this area is needed. On the other hand, with respect to the relationship between core stability and injury, several studies have found an association between a decreased stability and a higher risk of sustaining a low back or knee injury. Subjects with such injuries have been shown to demonstrate impaired postural control, delayed muscle reflex responses following sudden trunk unloading and abnormal trunk muscle recruitment patterns. In addition, various relationships have been demonstrated between core stability, balance performance and activation characteristics of the trunk muscles. Most importantly, a significant correlation was found between poor balance performance in a sitting balance task and delayed firing of the trunk muscles during sudden perturbation. It was suggested that both phenomena are caused by proprioceptive deficits.The importance of sensory-motor control has implications for the development of measurement and training protocols. It has been shown that challenging propriocepsis during training activities, for example, by making use of unstable surfaces, leads to increased demands on trunk muscles, thereby improving core stability and balance. Various tests to directly or indirectly measure neuromuscular control and coordination have been developed and are discussed in the present article. Sitting balance performance and trunk muscle response times may be good indicators of core stability. In light of this, it would be interesting to quantify core stability using a sitting balance task, for example by making use of accelerometry. Further research is required to develop training programmes and evaluation methods that are suitable for various target groups.

Objective In this study, the incidence and the sex-specific predictors of running-related injury (RRI) among a group of recreational runners training for a 4-mile running event were determined and identified, respectively. Design Prospective cohort study. Methods Several potential risk factors were prospectively measured in 629 novice and recreational runners. They were observed during an 8-week training period for any running-related musculoskeletal injuries of the lower limbs and back. A running-related injury was defined as any musculoskeletal pain of the lower limb or back causing a restriction of running for at least 1 day. Results At least one RRI was reported by 25.9% of the runners during the 8-week observation period. The incidence of RRI was 30.1 (95% CI 25.4 to 34.7) per 1000 h of running exposure. Multivariate Cox regression showed that male participants were more prone to sustain a RRI than female participants (HR 1.4; 95% CI 1.0 to 2.0). No previous running experience was the most important risk factor in male (HR 2.6; 95% CI 1.2 to 5.5) and female (HR 2.1; 95% CI 1.2 to 3.7) participants. Conclusions The incidence of running-related injuries in recreational runners preparing for a 4-mile running event is substantially high. Male and female participants have different risk profiles. Furthermore, the findings suggest that novice runners may benefit the most out of preventive interventions for RRI.

To determine the relationship between multidimensional performance characteristics and level of performance in talented youth field hockey players, elite youth players (n = 38, mean age 13.2 years, s = 1.26) were compared with sub-elite youth players (n = 88, mean age 14.2 years, s = 1.26) on anthropometric, physiological, technical, tactical and psychological characteristics. Multivariate analyses with performance level and gender as factors, and age as the covariate, showed that the elite youth players scored better than the sub-elite youth players on technical (dribble performance in a peak and repeated shuttle run), tactical (general tactics; tactics for possession and non-possession of the ball) and psychological variables (motivation) (P < 0.05). The most discriminating variables were tactics for possession of the ball, motivation and performance in a slalom dribble. Age discriminated between the two groups, indicating that the elite youth players were younger than the sub-elite players. In the guidance of young talented players to the top as well as in the detection of talented players, more attention has to be paid to tactical qualities, motivation and specific technical skills.

Abstract There is a need for a collective variable that captures the dynamics of team sports like soccer at match level. The centroid positions and surface areas of two soccer teams potentially describe the coordinated flow of attacking and defending in small-sided soccer games at team level. The aim of the present study was to identify an overall game pattern by establishing whether the proposed variables were linearly related between teams over the course of the game. In addition, we tried to identify patterns in the build-up of goals. A positive linear relation and a negative linear relation were hypothesized for the centroid positions and surface areas respectively. Finally, we hypothesized that deviations from these patterns are present in the build-up of goals. Ten young male elite soccer players (mean age 17.3, s=0.7) played three small-sided soccer games (4-a-side) of 8 minutes as part of their regular training routine. An innovative player tracking system, local position measurement (LPM), was used for obtaining player positions at 45 Hz per player. Pearson correlation coefficients were calculated to investigate the proposed linear relation of the key variables. Correlation coefficients indicate a strong positive linear relation during a whole game for the centroid position in all three games, with the strongest relation for the forward-backward direction (r>0.94). For 10 out of 19 goals a crossing of the centroids in this direction can be seen. No negative linear relation was found for surface area (−0.01 < r<0.07). From this study, we concluded that over the course of a whole small-sided game, the forward-backward motion of the centroids is most strongly linearly related. Furthermore, goals show a specific pattern in the forward-backward motion of the centroid. Therefore, surface area and particularly centroid position may provide a sound basis for a collective variable that captures the dynamics of attacking and defending in soccer at team level. Future research should develop these ideas further.

Background The popularity of running is still growing. As participation increases, running-related injuries also increase. Until now, little is known about the predictors for injuries in novice runners. Hypothesis Predictors for running-related injuries (RRIs) will differ between male and female novice runners. Study Design Cohort study; Level of evidence, 2. Methods Participants were 532 novice runners (226 men, 306 women) preparing for a recreational 4-mile (6.7-km) running event. After completing a baseline questionnaire and undergoing an orthopaedic examination, they were followed during the training period of 13 weeks. An RRI was defined as any self-reported running-related musculoskeletal pain of the lower extremity or back causing a restriction of running for at least 1 week. Results Twenty-one percent of the novice runners had at least one RRI during follow-up. The multivariate adjusted Cox regression model for male participants showed that body mass index (BMI) (hazard ratio [HR], 1.15; 95% confidence interval [CI], 1.05-1.26), previous injury in the past year (HR, 2.7; 95% CI, 1.36-5.55), and previous participation in sports without axial load (HR, 2.05; 95% CI, 1.03-4.11) were associated with RRI. In female participants, only navicular drop (HR, 0.85; 95% CI, 0.75-0.97) remained a significant predictor for RRI in the multivariate Cox regression modeling. Type A behavior and range of motion (ROM) of the hip and ankle did not affect risk. Conclusion Male and female novice runners have different risk profiles. Higher BMI, previous injury, and previous sports participation without axial loading are important predictors for RRI in male participants. Further research is needed to detect more predictors for female novice runners.

Objective Elite youth soccer players have a relatively high risk for injuries and illnesses due to increased physical and psychosocial stress. The aim of this study is to investigate how measures to monitor stress and recovery, and its analysis, provide useful information for the prevention of injuries and illnesses in elite youth soccer players. Methods 53 elite soccer players between 15 and 18 years of age participated in this study. To determine physical stress, soccer players registered training and match duration and session rating of perceived exertion for two competitive seasons by means of daily training logs. The Dutch version of the Recovery Stress Questionnaire for athletes (RESTQ-Sport) was administered monthly to assess the psychosocial stress–recovery state of players. The medical staff collected injury and illness data using the standardised Fédération Internationale de Football Association registration system. ORs and 95% CIs were calculated for injuries and illnesses using multinomial regression analyses. The independent measures were stress and recovery. Results During the study period, 320 injuries and 82 illnesses occurred. Multinomial regression demonstrated that physical stress was related to both injury and illness (range OR 1.01 to 2.59). Psychosocial stress and recovery were related the occurrence of illness (range OR 0.56 to 2.27). Conclusions Injuries are related to physical stress. Physical stress and psychosocial stress and recovery are important in relation to illness. Individual monitoring of stress and recovery may provide useful information to prevent soccer players from injuries and illnesses.

Abstract To identify performance characteristics that could help predict future elite field hockey players, we measured the anthropometric, physiological, technical, tactical, and psychological characteristics of 30 elite and 35 sub-elite youth players at the end of three consecutive seasons. The mean age of the players at the end of the first season was 14.2 years (s = 1.1). Repeated-measures analyses of covariance, with standard of performance and measurement occasion as factors and age as a covariate, showed that the elite players fared better than the sub-elite players on technical and tactical variables. Female elite youth players also scored better on interval endurance capacity, motivation, and confidence. Future elite players appear to have excellent tactical skills by the age of 14. They also have good specific technical skills and develop these together with interval endurance capacity better than sub-elite youth players in the subsequent 2 years. To verify our conclusions, we will be tracking these players into adulthood.

Abstract The soccer “penalty shootout” in the knock-out phase of major international tournaments is one of the most dramatic events in international soccer. The outcome of these kicks is typically attributed to factors such as psychology (e.g. coping with stress), skill (e.g. kicking technique), physiology (e.g. overcoming the fatigue of 120 min play), and chance (e.g. in what direction the goalkeeper moves). The purpose of this study was to use internet game record data to estimate the relative importance of these performance components for the outcome of penalty kicks in international matches. Data were collected from soccer statistics internet sites on all 41 penalty shootouts and 409 kicks taken in the World Cup, European Championships, and Copa America between 1976 and 2004. The results showed that the importance of the kicks (indicative of stress) was negatively related to the outcomes of the kicks, whereas skill and fatigue were less, or not, related to outcome. It was concluded that psychological components are most influential for the outcome of penalty kicks. We recommend that practitioners work with players to reduce the perceived importance of each kick.

Background Although running has positive effects on health and fitness, the incidence of a running-related injury (RRI) is high. Research on prevention of RRI is scarce; to date, no studies have involved novice runners. Hypothesis A graded training program for novice runners will lead to a decrease in the absolute number of RRIs compared with a standard training program. Study Design Randomized controlled trial; Level of evidence, 1. Methods GRONORUN (Groningen Novice Running) is a 2-armed randomized controlled trial comparing a standard 8-week training program (control group) and an adapted, graded, 13-week training program (intervention group), on the risk of sustaining an RRI. Participants were novice runners (N = 532) preparing for a recreational 4-mile (6.7-km) running event. The graded 13-week training program was based on the 10% training rule. Both groups registered information on running characteristics and RRI using an Internet-based running log. The primary outcome measure was RRIs per 100 participants. An RRI was defined as any musculoskeletal complaint of the lower extremity or back causing a restriction of running for at least 1 week. Results The graded training program was not preventive for sustaining an RRI (χ 2 = 0.016, df = 1, P = .90). The incidence of RRI was 20.8% in the graded training program group and 20.3% in the standard training program group. Conclusions This randomized controlled trial showed no effect of a graded training program (13 weeks) in novice runners, applying the 10% rule, on the incidence of RRI compared with a standard 8-week training program.

BACKGROUND Injuries to lower extremities are common in team sports such as soccer, basketball, volleyball, football and field hockey. Considering personal grief, disabling consequences and high costs caused by injuries to lower extremities, the importance for the prevention of these injuries is evident. From this point of view it is important to know which screening tools can identify athletes who are at risk of injury to their lower extremities. OBJECTIVE The aim of this article is to determine the predictive values of anthropometric and/or physical screening tests for injuries to the leg, anterior cruciate ligament (ACL), knee, hamstring, groin and ankle in team sports. METHODS A systematic review was conducted in MEDLINE (1966 to September 2011), EMBASE (1989 to September 2011) and CINAHL (1982 to September 2011). Based on inclusion criteria defined a priori, titles, abstracts and full texts were analysed to find relevant studies. RESULTS The analysis showed that different screening tools can be predictive for injuries to the knee, ACL, hamstring, groin and ankle. For injuries in general there is some support in the literature to suggest that general joint laxity is a predictive measure for leg injuries. The anterior right/left reach distance >4 cm and the composite reach distance <4.0% of limb length in girls measured with the star excursion balance test (SEBT) may predict leg injuries. Furthermore, an increasing age, a lower hamstring/quadriceps (H : Q) ratio and a decreased range of motion (ROM) of hip abduction may predict the occurrence of leg injuries. Hyperextension of the knee, side-to-side differences in anterior-posterior knee laxity and differences in knee abduction moment between both legs are suggested to be predictive tests for sustaining an ACL injury and height was a predictive screening tool for knee ligament injuries. There is some evidence that when age increases, the probability of sustaining a hamstring injury increases. Debate exists in the analysed literature regarding measurement of the flexibility of the hamstring as a predictive screening tool, as well as using the H : Q ratio. Hip-adduction-to-abduction strength is a predictive test for hip adductor muscle strain. Studies do not agree on whether ROM of the hamstring is a predictive screening tool for groin injury. Body mass index and the age of an athlete could contribute to an ankle sprain. There is support in the literature to suggest that greater strength of the plantar flexors may be a predictive measure for sustaining an ankle injury. Furthermore, there is some agreement that the measurement of postural sway is a predictive test for an ankle injury. CONCLUSIONS The screening tools mentioned above can be recommended to medical staff and coaches for screening their athletes. Future research should focus on prospective studies in larger groups and should follow athletes over several seasons.
