Dubin-Johnson syndrome with glycogen nuclei of the liver

Hisao Hayashi; Kazuro Masuko; Kozo Yamada; Takeshi Oguri

SummaryTwo cases of Dubin-Johnson syndrome with abundant glycogen nuclei in the liver were presented. They showed persistent jaundice, dark liver and rebound of Bromsulphalein clearance. The specimen of the liver revealed brown pigment deposited in the central zone and vacuolated nuclei in the periphery. On electron-microscopy, the electron dense granules observed in the central zone were found to be identical with those of Dubin-Johnson syndrome. In the periphery, however, they were rather similar to those of lipofuscin.A relation between glycogen nuclei and alteration of these granules was discussed.

Since the level of activity of serum glucokinase is high in normal and low in liver diseases. Serum glucokinase may be considered liberated from the liver. The level of serum hexokinase is high in cancerous patient. In cancerous tissues, are increased generally not only the activity of the supernatant fraction but also that of the mitochondrial fraction in many cases. In the liver of cancerous patient the activity of hexokinase was found either extremely increased or not increased. In the cases of cancerous patients, there was observed the affinity for hexokinase to liver mitochondria.

noma dur ing the recent 13 years. The sex and age incidence were as follows; 51 cases were males and 21 were females . The youngest pat ient of our series was a 26 year-old male, the eldest was a 79 year-old male and the major i ty of patients was ranged in the 50 to 59 years of age. According to si te of carc inoma in the pancreas , 35 of 72 cases were located in the head, 13 in the body, 3 in the tail, 7 in the whole pancreas and 14 in the per iampul la ry area. The diagnosis of the ear ly carc inoma of the pancreas is difficult, par t icu lary in the body and tail of pancreas . We could hardly discovered the carc inoma of the body and tail of pancreas before the operation. Theremore , it is impor tan t for the diagnosis of carc inoma of the pancreas tha t 3 cases of 13 carc inoma of the body of pancreas had been sufferred from acute pancreat i t is a few years ago. The seventy-two cases has had following operat ions; the radical operation was only 7 cases (9.7%), the pall iat ive operation 37 (51.4%) and the exploratory laparotomy 28 (38.9%). The survia l rate was very poor. The major i ty of the cases of carc inoma of the body and tail of pancreas died within a year. According to carcinoma of the head of pancreas and per iampul lary lesion, we could not an apparent difference between the radical operation and palliative operation such as biliary reconstruct ion. Therefore, even in the advanced cancer or the older cases in whom the radical operation could not be performed, the reconstruct ion of biliary t rac t should be tried for the purpose of the decrease of jaundice as fa r as possible.

On the carbohyddrate metabolism of the isolated perfused rat liver—carbohydrate metabolism on the development of fatty cirrhosis

Carbohydrate metabol i sms of the isolated perfused l iver f rom normal rat , and ra ts with fatty, fibrotic and c i r rhot ic l iver derived from choline deficiency, were studied. The isolated ra t l iver perfusions were carr ied out dur ing 2 hours with 200 ml of saline diluted, heparinized ra t blood, using Sellers-Okuyamas perfusion apparatus. Glucose, lactic acid, pyruvic acid and urea concentrat ion in perfusate were determined respectively. In the normal l iver perfusion, glucose level in perfusate increased as a resul t of glycogenolysis and gluconeogenesis. Then, lactic acid level decreased according as the s t imula t ion of gluconeogenesis. In the diseased l iver perfusion, in accordance with the development of histologic findings f rom fat ty l iver via fibrosis to cirrhosis , glucose level tended to decrease and lactic acid level tended to increase. Urea format ion in the diseased liver perfusion was lower than in the normal l iver perfusion. These data suggest tha t according as the development of l iver injuries, carbohydrate metabolism, especially gluconeogenesis is suppressed m.arkedly.

Studies on Some Actions of Sulphated Polysaccharides on Arteriosclerosis (III) : CLINICAL USE OF DEXTRAN SULPHATE

Kozo Yamada; Humio Kuzuya; Masatoshi Noda; Takeshi Oguri

The repeated intravenous administration of dextran sulphate which we synthesized decreased serum total cholesterol, especially free cholesterol in several patients, without the prolongation of clotting time of blood.

Studies on some actions of sulphated polysaccharides on arteriosclerosis. IV. Oral administration of dextran sulphate.

Kozo Yamada; Humio Kuzuya; Masatoshi Noda; Takeshi Oguri

As the dextran sulphate which we synthesized has a low molecular weight, the intestinal absorption of this dextran sulphate was considered. The clearing activity and clotting time were measured after oral administration of this preactivity and clotting time were measured after oral administration of this preparation. And it was confirmed that this preparation is absorbed from the intestine in both man and dog. The improvement of lipid metabolism in aged human subjects was observed by the oral administration of this preparation.
