Determination of the potential benefit of time-frequency gain manipulation

Objective: The purpose of this study was to determine the maximum benefit provided by a time-frequency gain-manipulation algorithm for noise-reduction (NR) based on an ideal detector of speech energy. The amount of detected energy necessary to show benefit using this type of NR algorithm was examined, as well as the necessary speed and frequency resolution of the gain manipulation. Design: NR was performed using time-frequency gain manipulation, wherein the gains of individual frequency bands depended on the absence or presence of speech energy within each band. Three different experiments were performed: (1) NR using ideal detectors, (2) NR with nonideal detectors, and (3) NR with ideal detectors and different processing speeds and frequency resolutions. All experiments were performed using the Hearing-in-Noise test (HINT). A total of 6 listeners with normal hearing and 14 listeners with hearing loss were tested. Results: HINT thresholds improved for all listeners with NR based on the ideal detectors used in Experiment I. The nonideal detectors of Experiment II required detection of at least 90% of the speech energy before an improvement was seen in HINT thresholds. The results of Experiment III demonstrated that relatively high temporal resolution (<100 msec) was required by the NR algorithm to improve HINT thresholds. Conclusions: The results indicated that a single-microphone NR system based on time-frequency gain manipulation improved the HINT thresholds of listeners. However, to obtain benefit in speech intelligibility, the detectors used in such a strategy were required to detect an unrealistically high percentage of the speech energy and to perform the gain manipulations on a fast temporal basis.

Linguistic contributions to speech-on-speech masking for native and non-native listeners: Language familiarity and semantic content

This study examined whether speech-on-speech masking is sensitive to variation in the degree of similarity between the target and the masker speech. Three experiments investigated whether speech-in-speech recognition varies across different background speech languages (English vs Dutch) for both English and Dutch targets, as well as across variation in the semantic content of the background speech (meaningful vs semantically anomalous sentences), and across variation in listener status vis-à-vis the target and masker languages (native, non-native, or unfamiliar). The results showed that the more similar the target speech is to the masker speech (e.g., same vs different language, same vs different levels of semantic content), the greater the interference on speech recognition accuracy. Moreover, the listeners knowledge of the target and the background language modulate the size of the release from masking. These factors had an especially strong effect on masking effectiveness in highly unfavorable listening conditions. Overall this research provided evidence that that the degree of target-masker similarity plays a significant role in speech-in-speech recognition. The results also give insight into how listeners assign their resources differently depending on whether they are listening to their first or second language.

Contralateral acoustic stimulation alters the magnitude and phase of distortion product otoacoustic emissions

Activation of medial olivocochlear efferents through contralateral acoustic stimulation (CAS) has been shown to modulate distortion product otoacoustic emission (DPOAE) level in various ways (enhancement, reduction, or no change). The goal of this study was to investigate the effect of a range of CAS levels on DPOAE fine structure. The 2f(1)-f(2) DPOAE was recorded (f(2)/f(1)=1.22, L(1)=55 dB, and L(2)=40 dB) from eight normal-hearing subjects, using both a frequency-sweep paradigm and a fixed frequency paradigm. Contamination due to the middle ear muscle reflex was avoided by monitoring the magnitude and phase of a probe in the test ear and by monitoring DPOAE stimulus levels throughout testing. Results show modulations in both level and frequency of DPOAE fine structure patterns. Frequency shifts observed at DPOAE level minima could explain reports of enhancement in DPOAE level due to efferent activation. CAS affected the magnitude and phase of the DPOAE component from the characteristic frequency region to a greater extent than the component from the overlap region between the stimulus tones. This differential effect explains the occasional enhancement observed in DPOAE level as well as the frequency shift in fine structure patterns.

Increase in Speech Recognition due to Linguistic Mismatch Between Target and Masker Speech: Monolingual and Simultaneous Bilingual Performance

PURPOSE To examine whether improved speech recognition during linguistically mismatched target-masker experiments is due to linguistic unfamiliarity of the masker speech or linguistic dissimilarity between the target and masker speech. METHOD Monolingual English speakers (n = 20) and English-Greek simultaneous bilinguals (n = 20) listened to English sentences in the presence of competing English and Greek speech. Data were analyzed using mixed-effects regression models to determine differences in English recogition performance between the 2 groups and 2 masker conditions. RESULTS Results indicated that English sentence recognition for monolinguals and simultaneous English-Greek bilinguals improved when the masker speech changed from competing English to competing Greek speech. CONCLUSION The improvement in speech recognition that has been observed for linguistically mismatched target-masker experiments cannot be simply explained by the masker language being linguistically unknown or unfamiliar to the listeners. Listeners can improve their speech recognition in linguistically mismatched target-masker experiments even when the listener is able to obtain meaningful linguistic information from the masker speech.

Changes in transient-evoked otoacoustic emission levels with negative tympanometric peak pressure in infants and toddlers.

Objectives: Otoacoustic emission (OAE) testing is now a standard component of the diagnostic audiology protocol for infants and toddlers and is an excellent tool for detecting moderate-to-profound cochlear hearing loss. Detection of hearing loss is especially important in infants and toddlers. Unfortunately, middle-ear dysfunction has a high incidence in this age range and can confound interpretation of OAEs. The goal of the study was to determine how transient-evoked otoacoustic emission (TEOAE) and noise levels were different when tympanometric peak pressures (TPP) measured from tympanograms were normal versus negative in the same individual. Another goal was to determine how TEOAE screening pass rates using a priori pass criteria were affected on days when TPP was negative. Design: TEOAE and noise levels were collected in 18 cases under 2 conditions: on a day when the tympanogram TPP was normal and on a day when the tympanogram TPP was negative. Data were collected from 11 children aged 3 to 39 mo, some of whom were tested more than once. Paired t tests were performed to determine whether there were changes in overall TEOAE and noise levels and TEOAE and noise levels analyzed into half-octave bands. A one-way ANOVA was performed on differences across half-octave bands to determine whether TPP affected TEOAE levels for some frequency bands more than others. Equality-of-proportion Z tests were run to determine whether there were significant differences in the percentage of “passes” on days when TPP was negative and TPP was normal. Results: Mean TEOAE level was lower when TPP was negative, but noise levels did not change between the 2 conditions. Mean TEOAE levels were lower for all frequency bands from 1000 to 4000 Hz and no significant differences were found among the mean reduction across frequency bands. There were no significant differences in the percentage of passes between TEOAEs collected on days when TPP was normal and when TPP was negative. Conclusions: Mean data indicated that when tympanograms had negative TPP, TEOAE level was lower by approximately 4 dB across all frequency bands. However, this affected the pass rate in only 5% to 6% of cases. Although the number of participants in the current study was small, the data suggest that it is possible to measure TEOAEs in children with negative TPP. If emission-to-noise ratio is used to identify hearing loss in mid-to-high frequency bands, the majority of children will still have TEOAEs that meet clinical criteria, this providing the clinician with important information about cochlear status.

Development and Preliminary Evaluation of a Pediatric Spanish–English Speech Perception Task

PURPOSE The purpose of this study was to develop a task to evaluate childrens English and Spanish speech perception abilities in either noise or competing speech maskers. METHOD Eight bilingual Spanish-English and 8 age-matched monolingual English children (ages 4.9-16.4 years) were tested. A forced-choice, picture-pointing paradigm was selected for adaptively estimating masked speech reception thresholds. Speech stimuli were spoken by simultaneous bilingual Spanish-English talkers. The target stimuli were 30 disyllabic English and Spanish words, familiar to 5-year-olds and easily illustrated. Competing stimuli included either 2-talker English or 2-talker Spanish speech (corresponding to target language) and spectrally matched noise. RESULTS For both groups of children, regardless of test language, performance was significantly worse for the 2-talker than for the noise masker condition. No difference in performance was found between bilingual and monolingual children. Bilingual children performed significantly better in English than in Spanish in competing speech. For all listening conditions, performance improved with increasing age. CONCLUSIONS Results indicated that the stimuli and task were appropriate for speech recognition testing in both languages, providing a more conventional measure of speech-in-noise perception as well as a measure of complex listening. Further research is needed to determine performance for Spanish-dominant listeners and to evaluate the feasibility of implementation into routine clinical use.

Predictability of Speech-In-Noise Performance from Real Ear Measures of Directional Hearing Aids

Objective Inability to understand speech in noise has been cited repeatedly as the principal complaint of hearing aid users. While data exist documenting the benefit provided by hearing aids with directional microphones when listening to speech in noise, little work has been done to develop a standard clinical protocol for fitting these hearing aids. Our goal was to evaluate a clinical measure of the acoustic directivity of a directional hearing aid, including its association with a test of speech perception in noise. Design The performance of two commercially available directional behind-the-ear (BTE) hearing aids was evaluated using the Hearing in Noise Test (HINT) and the Real Ear Aided Response (REAR) on 24 adult participants with symmetric, mild to moderately severe, sensorineural hearing loss. The HINT was conducted with the speech signal presented from 0° and the noise from 180° and either 135° or 225° degrees, depending on the ear tested. REAR was measured at the above three angles using swept pure tones, and these measures were used to compute in situ directivity for each subject and hearing aid. Conclusions Directional benefit for the HINT was greatest when noise was presented from the azimuth of the published polar diagram null of a given hearing aid in its directional mode (180 or 135/225°). The only significant correlation between HINT and REAR results, however, was found when the noise source was at 180°. These results confirm the validity of using real ear measures as a way to assess directionality in situ, but also indicate the complexity of predicting perceptual benefit from them. These data suggest that factors beyond acoustic directionality may contribute to improvement in speech perception in noise when such improvements are found.

Linguistic Masking Release in School-Age Children and Adults

PURPOSE This study assessed if 6- to 8-year-old children benefit from a language mismatch between target and masker speech for sentence recognition in a 2-talker masker. METHOD English sentence recognition was evaluated for English monolingual children (ages 6-8 years, n = 15) and adults (n = 15) in an English 2-talker and a Spanish 2-talker masker. A regression analysis with subject as a random variable was used to test the fixed effect of listener group and masker language and the interaction of these two effects. RESULTS Thresholds were approximately 5 dB higher for children than for adults in both maskers. However, children and adults benefited to the same degree from a mismatch between the target and masker language with approximately 3 dB lower thresholds in the Spanish than the English masker. CONCLUSIONS Results suggest that children are able to take advantage of linguistic differences between English and Spanish speech maskers to the same degree as adults. Yet, overall worse performance for children may indicate general cognitive immaturity compared with adults, perhaps causing children to be less efficient when combining glimpses of degraded speech information into a meaningful sentence.

Speech-on-speech masking with variable access to the linguistic content of the masker speech for native and nonnative English speakers

BACKGROUND Masking release for an English sentence-recognition task in the presence of foreign-accented English speech compared with native-accented English speech was reported in Calandruccio et al (2010a). The masking release appeared to increase as the masker intelligibility decreased. However, it could not be ruled out that spectral differences between the speech maskers were influencing the significant differences observed. PURPOSE The purpose of the current experiment was to minimize spectral differences between speech maskers to determine how various amounts of linguistic information within competing speech Affiliationect masking release. RESEARCH DESIGN A mixed-model design with within-subject (four two-talker speech maskers) and between-subject (listener group) factors was conducted. Speech maskers included native-accented English speech and high-intelligibility, moderate-intelligibility, and low-intelligibility Mandarin-accented English. Normalizing the long-term average speech spectra of the maskers to each other minimized spectral differences between the masker conditions. STUDY SAMPLE Three listener groups were tested, including monolingual English speakers with normal hearing, nonnative English speakers with normal hearing, and monolingual English speakers with hearing loss. The nonnative English speakers were from various native language backgrounds, not including Mandarin (or any other Chinese dialect). Listeners with hearing loss had symmetric mild sloping to moderate sensorineural hearing loss. DATA COLLECTION AND ANALYSIS Listeners were asked to repeat back sentences that were presented in the presence of four different two-talker speech maskers. Responses were scored based on the key words within the sentences (100 key words per masker condition). A mixed-model regression analysis was used to analyze the difference in performance scores between the masker conditions and listener groups. RESULTS Monolingual English speakers with normal hearing benefited when the competing speech signal was foreign accented compared with native accented, allowing for improved speech recognition. Various levels of intelligibility across the foreign-accented speech maskers did not influence results. Neither the nonnative English-speaking listeners with normal hearing nor the monolingual English speakers with hearing loss benefited from masking release when the masker was changed from native-accented to foreign-accented English. CONCLUSIONS Slight modifications between the target and the masker speech allowed monolingual English speakers with normal hearing to improve their recognition of native-accented English, even when the competing speech was highly intelligible. Further research is needed to determine which modifications within the competing speech signal caused the Mandarin-accented English to be less effective with respect to masking. Determining the influences within the competing speech that make it less effective as a masker or determining why monolingual normal-hearing listeners can take advantage of these differences could help improve speech recognition for those with hearing loss in the future.

Effects of linguistic experience on the ability to benefit from temporal and spectral masker modulation

Masked speech perception can often be improved by modulating the masker temporally and/or spectrally. These effects tend to be larger in normal-hearing listeners than hearing-impaired listeners, and effects of temporal modulation are larger in adults than young children [Hall et al. (2012). Ear Hear. 33, 340-348]. Initial reports indicate non-native adult speakers of the target language also have a reduced ability to benefit from temporal masker modulation [Stuart et al. (2010). J. Am. Acad. Aud. 21, 239-248]. The present study further investigated the effect of masker modulation on English speech recognition in normal-hearing adults who are non-native speakers of English. Sentence recognition was assessed in a steady-state baseline masker condition and in three modulated masker conditions, characterized by spectral, temporal, or spectro-temporal modulation. Thresholds for non-natives were poorer than those of native English speakers in all conditions, particularly in the presence of a modulated masker. The group differences were consistent across maskers when assessed in percent correct, suggesting that a single factor may limit the performance of non-native listeners similarly in all conditions.
