Dental injuries among children and adolescents aged 1-15 years attending to public hospital in Temuco, Chile

A cross-sectional study was carried out in children and adolescents of both sexes, aged 1-15 years that sought dental emergency attention to the Regional Hospital between 2004 and 2007 in Temuco, Chile. The purpose of this study was to identify the aetiology, types of traumatic dental injuries in primary and permanent dentitions, sex and age distributions, accident location; and time elapsed before emergency treatment in children and adolescents. The sample consisted of 359 patients with 145 primary teeth and 525 permanent teeth affected by dental trauma. The results showed a 2:1 male:female ratio distribution (242/117) with a mean age of 8.4 years. The 7- to 12-year-old group had the highest frequency of dental injuries (66.6%). Unspecific accidental falls were the main cause of injury to primary and permanent dentition (51.8%), followed by striking teeth against objects (15.6%) and bike accidents (13.9%). In primary dentition the most common diagnosis were subluxation (38.6%) and avulsion (16.6%), whereas in permanent dentition was uncomplicated crown fracture (32.9%). A high proportion of the patients received their first emergency attention 24 h after the accident (32.6%). This study revealed a high frequency (37.9%) in 1-15 aged population that sought emergency attention by dental trauma in the period of time study. A large proportion of children with dental trauma received delayed first emergency care, even 24 h after the accident. Considering the high frequency of traumatic dental injuries in 1-15 aged population and the high percentage of delayed emergency attention is necessary to develop effective educational campaigns in regard to causes, prevention and emergency management of traumatic dental injuries, especially in deprived areas. In conclusion, traumatic dental injury may be considered as a serious dental public health problem especially in children of deprived areas.

Initial validation of a questionnaire for detecting gastroesophageal reflux disease in epidemiological settings

There is a high prevalence of gastroesophageal reflux disease (GERD) in the general population. Prevalence studies are scarce, and there is a lack of valid instruments for measuring them. The aim of this paper is to validate a questionnaire for detecting GERD. A validity study design with pathologic GERD patients and controls was used. A sample of 240 subjects age and sex paired was selected in the ratio of 3:1 (patients to controls). The initial structured questionnaire contained a variety of GERD symptoms. Internal consistency, interobserver reliability, criteria validity using 24-h esophageal pH monitoring, construct validity, and extreme group validation were assessed. Sensitivity, specificity, and predictive values were also obtained in different cutoff points of the definitive scale. A total of 180 confirmed GERD patients and 60 controls were included in the study. Mean age in years was 45 +/- 13, with no statistical difference by gender (67% were female). Internal consistency of 0.75 and interobserver reliability of 0.87 was achieved in building the scale. Extreme group validation was highly significant by assessing the scale score with 24-h esophageal pH monitoring (P <.0001). At cutoff point 3 of the scale and with a correct classification of subjects of 92.4%, sensitivity, specificity, positive, and negative predictive values were 92, 95, 98, and 79%, respectively. The conclusion of this article is that a reliable and valid instrument was built to detect GERD.

Liver abscess of hydatid origin: Clinical features and results of aggressive treatment

Knowledge of the management of paediatric dental traumas by non-dental professionals in emergency rooms in South Araucanía, Temuco, Chile

AIM The objective of this study was to investigate the level of knowledge and attitudes regarding first aid for dental trauma in children (TDI) by non-dental professionals and paramedical technicians of hospital emergency rooms in the South Araucanía Health Service, Chile, which was attained through application of a survey. MATERIALS AND METHODS Samples were collected from people with occupations in the respective emergency rooms. The participants were 82 people that were interviewed using a questionnaire regarding management of dental trauma. Paramedic technicians, general and specialist doctors, and nurses were included in this survey. The appraisal covered diverse aspects: birth date, age, sex, years of experience in the emergency room, and questions regarding specific dental trauma topics, which focused on crown fractures, luxation injuries in permanent dentition, avulsion in primary and permanent teeth, and the respective emergency treatments. RESULTS Of the participants, 78.1% reported to have been presented with a TDI patient. The majority (90.2%) had not received formal training on TDI. These results revealed a wide distribution of responses. The overall dental trauma knowledge among the participants was relatively poor. For crown fractures management 54.9% indicated that they would ask the affected child about the crown remnants. In regard to transport and storage medium of avulsed permanent teeth, only 9.8% of the participants answered correctly and 43.9% of respondents stated that they would not replant an avulsed permanent tooth, since that procedure is considered the responsibility of a dentist. CONCLUSIONS The majority of the respondents were not knowledgeable regarding TDI or the management and benefits of timely care, particularly in cases of avulsed permanent teeth. Therefore, formal education and training on the topic is suggested during undergraduate studies.

Abdominal echinococcosis during pregnancy: clinical aspects and management of a series of cases in Chile

Twelve pregnant women with hydatid disease are presented with median age of 29;11 (91.7%) had a liver cyst and one (8.3%) had a kidney cyst as the primary disease location. Four (33.3%) had additional cysts located in the pelvis, peritoneal cavity and/or spleen; eight (66.7%) had two or more abdominal cysts. Three patients (25.0%) had surgery at the 3rd month after delivery and nine (75.0%) during their pregnancy. There was no histological evidence of hydatid disease in placentas, and no serological evidence of echinococcosis in the newborns was confirmed. One patient died after surgery. After a mean follow-up time of 39.5 months, we found one recurrent case of pelvic hydatid disease. Management of abdominal echinococcosis during pregnancy is an uncommon and difficult problem owing to the serious potential risks for mother and child.

Colangiohidatidosis: Una complicación evolutiva de la hidatidosis hepática

Hydatidosis is and endemic disease in the south of Chile, especially in Region IX where remain rates of high prevalence. Cholangiohydatidosis is an infrequent complication of liver hydatidosis. The objective of this paper is to describe clinical features of a series of patients with cholangiohydatidosis. Series of cases of cholangiohydatidosis treated consecutively and the corresponding follow-up is reported. Clinical, laboratory and images characteristics are described. Descriptive statistic was used its incidence was calculated. In the studied period, 13 patients with cholangiohydatidosis were recruited, 9 men (69.2%) and 4 women (30.7%). Observed laboratory abnormalities were an increment of leukocytes in 8 patients (61.5%) and hepatic cholestasis with hepatocyte signs of cytolysis in 11 patients (84.6%). Choledocus diameter average measured by ultrasonography was 24.7 mm. All the patients had at least one liver cyst whose diameter average was of 12.1 cm. At surgery, evidence of biliary communications was detected in all patients and biliary decompression was carried out through a choledocostomy in 12 patients (92.3%) and by choledocoduodenal anastomoses in one case (7.7%). With a mean follow-up of 38.7 months morbility was 23% and mortality 7.7%. Accumulated incidence of this entity was of 0.07 cases in 5 years. Cholangiohydatidosis is an uncommon complication of liver hydatidosis that presents considerable morbidity and mortality rates.

Prenatal attachment and associated factors during the third trimester of pregnancy in Temuco, Chile

OBJECTIVE to estimate the prevalence of poorer prenatal attachment and its association with psycho-affective factors in pregnant women during the third trimester. DESIGN AND SETTING cross-sectional study in Temuco, La Araucanía Region, Chile. MEASUREMENTS data were collected by structured interview with closed questions for the sociodemographic characterisation of the sample and measurement of six aspects: prenatal attachment, perceived stress, depression, perception of relationship with partner, subjective family support, and obstetric information regarding current and previous pregnancies. PARTICIPANTS 244 pregnant women selected by stratified random sampling in all centres (n=5) of the public health system in Temuco, Chile, with proportional allocation. FINDINGS the prevalence of poorer prenatal attachment was 24.3% (95% confidence interval 19-30%), and this was found to be associated with discontent with the pregnancy, unwanted pregnancy, higher levels of perceived stress, depression and low family support. Religious activity and work were found to modulate the association between poorer prenatal attachment and psycho-affective aspects. The percentage of unplanned pregnancies was high in this study (61.35), and although this does not have a direct influence on poorer prenatal attachment, it is associated with discontent with the pregnancy and unwanted pregnancy. KEY CONCLUSIONS AND IMPLICATIONS FOR PRACTICE the high proportion of poorer prenatal attachment during the third trimester of pregnancy associated with potentially detectable psychosocial factors means that early diagnosis and timely intervention during prenatal care are an essential challenge for midwives in their work. Any progress that can be made during pregnancy will favour the development of the bonding experience after birth, and thus the balanced development of the child.

Uncommon locations of abdominal hydatid disease.

South Chile is an endemic zoonosis area of hydatid disease, where prevalence rates can be 30 to 40/100 000 habitants. Not only is surgery performed on a great number of liver and lung hydatid cysts every year, but also upon cysts found in uncommon abdominal locations such as spleen, peritoneum and kidney. Information on this subject is scarce. We reviewed PubMed index from 1975 to 2001 and found only five related articles, reporting more than 30 cases.

Diagnostic properties of a symptoms scale for diagnosing reflux esophagitis

OBJECTIVE To the objective of the study was to determine accuracy and predictive values of a symptoms scale for diagnosing reflux esophagitis (RE). STUDY DESIGN AND SETTING Standard criterion study. All recruited patients from two centers in Chile underwent both digestive endoscopy (reference standard) and a symptoms scale known to be valid and reliable for diagnosing gastroesophageal reflux disease. The RE variable was dealt with dichotomously. A receiver operating characteristic curve was constructed. Sensitivity, specificity, positive and negative predictive values, and positive and negative likelihood ratios of the scale were calculated. RESULTS Two hundred and thirty eight (238) subjects (57.6% female), with an average age of 44.2+/-13.0 years were included. Of these, 57.1% presented with RE. With a cut-off score of six, association was confirmed between the symptoms scale and RE with an odds ratio of 7.26 and a correct classification i.e. diagnostic accuracy of 73.1%. Sensitivity, specificity, positive and negative predictive values, positive and negative likelihood ratios, of 74.3%, 71.6%, 77.7%, 67.6%, 2.61, and 0.36 respectively, were obtained. CONCLUSION A seven-item symptoms scale when compared to endoscopy as gold standard was useful for diagnosing RE. Using a cutoff of six points, the diagnostic accuracy of the scale was 73.1%.

Reliability of Two Techniques for Measuring Condylar Asymmetry with X-Rays

Dentro de las alteraciones estructurales que pueden ser un factor de riesgo de desarrollo de un trastorno temporomandibular (TTM) se menciona a la asimetria condilar. Para realizar la medicion cuantitativa del indice de asimetria condilar en radiografias panoramicas se han propuesto dos metodos, Habets y Kjellberg. El objetivo de este estudio fue determinar si el metodo radiografico de medicion de asimetrias condilares en ortopantomografias que presenta menor tendencia al error por leves desplazamientos de la cabeza en el plano horizontal. Se evaluaron 30 pacientes entre 18 y 25 anos de edad. Cada uno de ellos se sometio a tres radiografias panoramicas en tres posiciones distintas: posicion ortoradia, 5 y 10 de angulacion horizontal. Posteriormente, se realizaron las mediciones de Habets y Kjellberg. La tecnica de Habets no mostro diferencias estadisticamente significativas en las radiografias con 5° y 10° de angulacion horizontal con respecto al angulo de 0. Sin embargo, la tecnica de Kjellberg mostro diferencias estadisticamente significativas solo al angulo de 10° con respecto al angulo de 0. Las alteraciones de 10° produjeron variaciones lineales y de razones, sin embargo no variaron los indices. Se concluye que ambos metodos entregan informacion clinica aceptable con las limitaciones que estas tecnicas tienen para obtener informacion sobre simetrias o asimetrias condilares de cuerpo o de rama.
