The global trend of ageing populations is present in Brazil. Brazilian society is going through an intensive process of transformation, where young adults are increasingly fewer, thus raising the question of who will take care of the elderly in the 21st Century? There is an urgent need to establish care networks for the elderly, covering primary care, including care with housing (Healthy Housing), as well as to ensure access to secondary and tertiary levels of care, providing early diagnoses and rapid access to treatment. This article reports two Brazilian experiences: the restructuring of a shelter for elderly indigents, with 500 elderly residents in Rio de Janeiro, which involved a broad process of humanization, with strong physical restructuring of the buildings that dated from 1930; and the process of organizing the care referral line for the elderly within the Conceição Hospital Group, in Rio Grande do Sul State, which included a health care complex with diverse health units, encompassing all stages of care: from primary care to the most complex hospital treatment.

RESUMO Este artigo analisa o perfil dos recursos fisicos de saude disponiveis no Brasil, utilizando o exemplo do trauma de alta complexidade para subsidiar o debate da reorganizacao da rede de servicos de saude. Utilizou-se o Cadastro Nacional de Estabelecimentos de Saude de 2013. Identificou-se que ha insuficiencia quantitativa e baixa oferta na distribuicao geografica de leitos de Unidades de Terapia Intensiva, de hospitais de grande porte e das oito estruturas exigidas pelo Ministerio da Saude para habilitacao dos estabelecimentos ao atendimento do trauma de alta complexidade. A aquisicao de uma estrutura nos estabelecimentos que ja possuem as outras sete aumentaria esta oferta de 30 para 80 do numero de municipios com atendimento ao trauma de alta complexidade.

To investigate the debate about South-South cooperation, we conducted an analysis of a course on the Management of Physical and Technological Resources in Health given in Haiti as part of a partnership between Fiocruz and the Brazilian and Haitian ministries of health with the aim of enabling the functioning of the units built by Brazil, contributing to the sustainability of the tripartite cooperation project. The course format - designing interventions to overcome real problems identified by the students - resulted in the development of eight intervention pre-projects geared towards overcoming management problems at the reference community hospitals built by Brazil in Haiti.

In Brazil, the neoplasies took the second place in the period of 1996 to 2001. In relation to morbidity indicators, they express the emergence of chronic and degenerative diseases, caused by the fast aging of the population, an acute phenomenon explained more strongly by the reducing rates of infant mortality and the reduction in birth rates, presenting a situation for which the health systems are not prepared. In Brazil, it is confirmed a global tendency: the longer survival of women than men, and the depression as a major cause of morbidity. Brazilian society is going through intensive process of transformation, where young adults are increasingly fewer, imposing the question: who will take care of our elderly in the XXI century? The structuring of networks of attention to the elderly person is urgent, covering primary care, including the care with housing: Healthy Housing, and ensuring access to secondary and tertiary levels of care, providing early diagnosis and fast access to treatment. This article reports a case study experience: the restructuring of a shelter for elderly indigents, with 500 elderly in Rio de Janeiro, where it was held a broad process of humanization, with strong physical restructuring of the buildings from 1930.

Este ensaio e o relato de uma experiencia envolvendo a elaboracao de projetos arquitetonicos para a construcao de oito novas Escolas Tecnicas de Saude do Servico Unico de Saude (ETSUS) e a reestruturacao de sete escolas existentes. Para essa finalidade, o Projeto de Profissionalizacao de Trabalhadores da Area de Enfermagem (PROFAE) estruturou o Escritorio de Projetos e Obras, vinculado ao Componente II do Projeto, composto por quatro arquitetos com experiencia na area da arquitetura hospitalar e equipe tecnica responsavel pela modernizacao e fortalecimento das ETSUS e as escolas envolvidas no processo. Os trabalhos foram desenvolvidos de setembro de 2001 e outubro de 2002, quando foram concebidos os estudos preliminares das escolas e, posteriormente, os projetos executivos com vistas a licitacao das obras. A concepcao dos projetos arquitetonicos pautou-se em algumas diretrizes basicas: concepcao de espacos flexiveis que dessem conta de absorver a dinâmica das transformacoes da area da saude, alem de propiciar a capacitacao de profissionais tanto em processos de trabalhos envolvendo tecnologias de alto custo como em atividades voltadas para a atencao basica em saude; adocao de partidos arquitetonicos que visassem a racionalizacao de energia; a construcao de uma identidade para as ETSUS; e concepcao de projetos que facilitassem a sustentabilidade das escolas tecnicas.
