Balloon occlusion of the hypogastric arteries in the management of placenta accreta: A case report and review of the literature

M. Knuttinen; Aarti Jani; Ron C. Gaba; James T. Bui; Tami C. Carrillo

Obstetric hemorrhage from placenta accreta is associated with a high rate of maternal morbidity and mortality. Recently, balloon occlusion catheters have been used to control intraoperative bleeding during the surgical management of placenta accreta. In this article, we present a review of the literature reporting the use of balloon occlusion catheters in the management of placenta accrete, and a case presentation outlining the use of a Fogarty balloon occlusion catheter to achieve hemostasis in the preoperative management of placenta percreta.

Abstract Placenta accreta is abnormal placental adherence or invasion of the myometrium or extrauterine structures. It is increasing in incidence because of increasing number of cesarean sections and is one of the main causes of excessive postpartum hemorrhage. Recognition of this entity is crucial because improved outcomes have been shown when the antenatal diagnosis of placenta accreta is made. Ultrasound is the first-line tool; magnetic resonance imaging (MRI) is complementary. Ultrasound and MRI features and MRI protocols will be reviewed.

Learning Objectives To detail the increasing number of transcatheter renal interventions that can be performed by Interventional Radiologists. -To critically review the scientific literature supporting the most commonly performed transcatheter renal interventions. Background Transcatheter renal interventions play an important role in the treatment of patients with a wide variety of renal pathologies. With the recent development of catheter-based renal denervation for select patients with arterial hypertension, there is an increasing number of transcatheter therapies that Interventional Radiologists can perform. The goal of this exhibit is to critically review the most commonly performed transcatheter renal interventions, with a detailed review of the literature and case examples of the included procedures. Clinical Findings/Procedure Details For each of the following procedures, we will include brief epidemiology data, brief pathophysiology, and detailed review of the scientific literature. Each procedure will be illustrated with case examples from our institution: 1. Renal artery stenosis: angioplasty +/- stenting 2. Renal artery (pseudo)aneurysm embolization 3. Renal arteriovenous fistula embolization 4. Renal vein thrombosis thrombectomy 5. (Pre-surgical) embolization of renal tumors 6. Catheter-based renal denervation 7. Miscellaneous Conclusion and/or Teaching Points • Transcatheter renal interventions are used to treat a wide variety of disease that affect the kidneys, including renal vascular pathologies, renovascular and arterial hypertension, and renal neoplasms. • There is an increasing number of transcatheter renal interventions that can be performed by interventional radiologists. • This exhibit critically reviews the most commonly performed transcatheter renal interventions.
