A rare cause of chronic sciatic pain: Schwannoma of the sciatic nerve

Schwannomas are common, benign tumors of the shelth of peripheral nerves. Sciatic schwannomas are rare. Their symptomatology usually mimics sciatic pain due to a herniated disc, which can delay the diagnosis. If there is no lumbar pain and lumbar MRI is normal, the sciatic nerve must be clinically and radiologically examined all along its course. We report a case of sciatic nerve schwannoma presenting with chronic sciatica which was diagnosed and monitored radiologically for several years before successful surgical resection.

Open medial ankle dislocation without associated fracture: a case report.

Tibiotalar dislocation without associated fracture is a rare injury. We report a case of an unusual open medial ankle dislocation without any associated bony injury. After reduction and debridement under general anaesthesia, capsule suture and ligaments repair were performed. An external fixator was applied for ankle immobilization. After 3 years follow-up, functional results were excellent without signs of instability or degenerative arthritis.

Upward tibiotalar dislocation without fracture: A case report.

Pure tibiotalar dislocations without associated fracture are rare. The authors report a case of an unusual closed tibiotalar dislocation without fracture involving proximal ascension of the talus. Immediate closed reduction was performed. Repair of the disrupted medial ligament was performed and one syndesmosis screw inserted, followed by 6 weeks of immobilization. After 1 year of follow-up, functional results were excellent.

Bilateral anterior shoulder dislocation with bilateral fractures of the greater tuberosity: A case report.

Bilateral anterior dislocation of the shoulders with fractures of both greater tuberosities is very rare. A 76-year-old woman sustained a bilateral anterior dislocation of her shoulders with fractures of the greater tuberosity on both sides after a fall on stairs. Her arms were abducted and externally rotated. Radiological examination revealed the bilateral anterior dislocation and also the bilateral fractures of the greater tuberosity. Prompt closed reduction followed by a 3 weeks immobilization and subsequent rehabilitation allows a good outcome. Results at one-year follow-up were satisfactory with normal range of motion and no redislocations occurring. To our knowledge, this is the first reported case of bilateral anterior shoulder dislocation associated with fractures of both greater tuberosities in elderly woman.

The Monteggia fracture: series of 20 cases

The Monteggia fracture is one of the pitfalls of conventional diagnosis of upper limb trauma. Through a retrospective study of 20 cases diagnosed at the Department of Orthopaedic Surgery and Traumatology, University Hospital Center, Ibn Sina, Mohamed V University, Rabat, Morocco, between 2010 and 2014, we have tried to do an update on the management of Monteggia fractures either at of paraclinical exams or the surgical management. We support the idea that the dislocation of the radial head should be sought systematically to any isolated fracture of the ulna, for not to miss fracture Monteggia authentic. Rehabilitation of the upper limb must be done as soon as possible.

Multifocal bone tuberculosis: a case report.

Multifocal long bones tuberculosis without articular involvement is very rare. Pain and swelling are the common presenting symptoms. Difficulties in diagnosis often lead to delayed treatment, sometimes with devastating consequences for patients. Radiographs may mimic other diseases. Histopathological study is necessary to establish the diagnosis. The antibacillary chemotherapy produces excellent results. We report a case of a 60-year-old woman afflicted with multifocal tibial tuberculosis.

The role of scapulectomy in the care of the scapular tumors: a report of 6 cases

Abstract This retrospective study aims to demonstrate the role of scapulectomy in the treatment of scapular tumours by assessing the results of 6 patients treated for various scapular tumours in the Orthopaedic Department of IBN SINA hospital in Rabat between 1996 and 2009. The evaluation of these patients by the Enneking system showed, on a functional scale, an excellent score for 4 patients and good one for 2 patients; on the oncology scale, a single case required a total scapulectomy after recurrence with a good evolution. Since an enlarged scapulectomy leads to functional deficit, the majority of malignant tumours of the shoulder are currently treated conservatively, by performing a simple scapulectomy. Patient selection must be made after appropriate staging, a complete imaging study and a good knowledge of anatomy for a correct surgical indication

Multipartite Patella: Diagnosis Trap

Our study reports a case of a multipartite patella that was initially diagnosed as a patellar fracture in a traumatic context but in view of the smooth, regular and dense appearance of the fracture traits, a comparative radiological evaluation of the other knee was necessary, which precluded the diagnosis of fracture in favor of a multipartite patella with knee sprain treated by immobilization and treatment anti-inflammatory with a good evolution, hence the interests of performing a comparative radiological assessment of the contralateral knee in front of any patellar fracture. The multipartite patella is recognized as a developmental anomaly of ossification. Most of them are asymptomatic and are discovered incidentally. The multipartite patella is sometimes misdiagnosed as a patellar fracture, because the x-ray images of both these conditions may appear very similar.

Cubitus varus or malunion is most often secondary to displaced supracondylar fractures of the lower end of the humerus in children. Various surgical techniques have been proposed with different success rates but also with different reported complication rates. Our retrospective study of 25 cases diagnosed at the University Hospital of Rabat evaluated the results of the technique of external subtraction which was used in our training to handle this deformation.

Luxation postérieure divergente de la cheville sans fracture associée. À propos d’un cas

RésuméLes luxations tibiotaliennes sans fracture ni effraction cutanée sont très rares. Nous rapportons le cas chez un jeune sportif d’une luxation tibiotalienne postérieure divergente sans fracture associée et qui a nécessité une réduction en urgence. Le contrôle radiologique a objectivé la persistance d’un diastasis. L’exploration chirurgicale a mis en évidence une rupture complète du ligament collatéral médial qui a été réparé. Cette réparation a été complétée par la mise en place d’une vis de syndesmodèse et par une immobilisation plâtrée pendant six semaines. L’évolution a été favorable permettant la reprise de l’activité sportive légère à partir du troisième mois et un contrôle radioclinique satisfaisant à 12 mois.AbstractPure tibiotalar dislocations without associated fracture are rare.
