Early Workplace Intervention to Improve the Work Ability of Employees with Musculoskeletal Disorders in a German University Hospital-Results of a Pilot Study.

Health promotion is becoming increasingly important in work life. Healthcare workers seem to be at special risk, experiencing musculoskeletal disorders (MSD); their situation is strongly influenced by demographic changes. The aim of this study is to evaluate the feasibility and outcome of a worksite intervention. In a one-group pretest-posttest design, 118 employees of a hospital were recruited from 2010 to 2011. The raised parameters were satisfaction with the program, work ability (Work Ability Index), and sickness absence (provided by human resource management). Patient-reported questionnaire data was raised at baseline (t1) and after three months (t2). Sickness leave was evaluated in the period six months prior to and six months after the intervention. Means, frequencies, standardized effect sizes (SES), analysis of variance, and regression analysis were carried out. Participants were found to be highly satisfied. Work ability increased with moderate effects (SES = 0.34; p < 0.001) and prognosis of gainful employment (SES = −0.19; p ≤ 0.047) with small effects. Days of MSD-related sickness absence were reduced by 38.5% after six months. The worksite intervention program is transferable to a hospital setting and integration in occupational health management is recommended. The use of a control group is necessary to demonstrate the effectiveness.

Many studies have shown that return-to-work after acute disease or injury as well as in patients with chronic conditions is not only infl uenced by health parameters but infl uenced by many other factors. Examples are lack of motivation, social interactions at the work place, comorbidities such as depression or anxiety, prolonged sick-leave and others. In terms of the ICF-model these factors may be described as environmental and personal factors. As standard medical rehabilitation often focuses on the health condition, body functions and activities, the main goal to achieve full participation is not reached. Therefore, additionally to standard medical rehabilitation two strategies have to be established:

Ein Blick Down Under. Selbstmanagementinitiativen und Patientenschulungen in Australien

ZusammenfassungChronische Krankheiten nehmen in der westlichen Welt stark zu und stellen die Gesundheitssysteme vor neue Herausforderungen. Im Zuge verstärkter Patientenorientierung sollen Strukturen zur Unterstützung von Selbstmanagement und Selbstverantwortung geschaffen werden, wobei effizienten Schulungsprogrammen eine wichtige Rolle zukommt. Im australischen Gesundheitssystem wurden bereits Strategien im Umgang mit chronischen Erkrankungen umgesetzt. Eine davon ist die „Sharing Health Care Initiative“, die seit 1999 die verschiedenen Selbstmanagementprogramme untersucht. Eine nationale Evaluation dieser Initiative hat Empfehlungen zur Optimierung und Implementierung von Selbstmanagementprogrammen ausgesprochen, die in die Entwicklung der „National Chronic Disease Strategy“ eingeflossen sind, in deren Rahmen umfangreiche finanzielle Mittel ausgeschüttet werden. Vor dem Hintergrund der als notwendig erachteten Einführung eines Qualitätssicherungssystems entwickelte die Universität Melbourne den „Health Education Impact Questionnaire“ (heiQ) zur Evaluation und Qualitätssicherung von Selbstmanagementprogrammen. Im Rahmen eines vom Bundesministerium für Bildung und Forschung (BMBF) geförderten Forschungsprojekts soll dieser nun auch in Deutschland eingesetzt werden.AbstractThe growing burden of chronic disease and the current nature of healthcare systems which are structurally ill-equipped to cater to the complex needs of patients with chronic conditions has led to governments and healthcare providers seeking alternative ways to improve patients own capacity to actively self-manage their chronic condition. In Australia, there has been a focus on patient education and self-management programs within the healthcare system to achieve this. These programs aim to empower patients through providing information and teaching skills and techniques to improve self-care and doctor-patient interaction with the ultimate goal of improving quality of life. Patient education and self-management programs have been supported through several national government initiatives and implemented within the healthcare setting. This paper describes the current position of patient education and self-management within the Australian healthcare system. It further describes a new collaboration project between an Australian and a German research team which aims at translating an assessment questionnaire used in Australia for the evaluation of self-management programs, the “Health Education Impact Questionnaire” (“heiQ”); this instrument is expected to be of significant use in the German rehabilitative system.
