Valve failure with reflux and post-thrombotic syndrome are the factors most commonly correlated with varicose disease. Other rare etiologies can be put forward when these two main causes are ruled out. We report a case in which a young man presented chronic pain in the left tibia, varicose veins in the lower limbs and frequent occurrences of erysipelas. During investigation of the etiology of the varicose veins, radiographs and magnetic resonance imaging of the left leg were requested. These showed images suggestive of an osteolytic lesion in the tibia, but led us to the diagnosis of an intraosseous vein with anomalous drainage. This was confirmed through vascular examinations comprising Doppler venous flow measurement and phlebography. Recognition of this rare intraosseous anomaly is fundamental for proper patient management, but an intraosseous surgical approach is unnecessary.

Peripheral arterial embolism (PAE) caused by malignant tumors is a rare manifestation of cancer. PAE may originate from several sites, including heart, aorta, and pulmonary veins. Such veins are a major source of thrombotic embolism or tumors with vascular erosion. Although uncommon, lung cancer should be regarded as a source of emboli in the extremities, especially when there is neoplastic invasion of the pulmonary veins. We report on a case of a male patient who underwent pneumonectomy for lung cancer who developed acute arterial occlusion of the lower extremities caused by saddle tumor embolus.

With regards to the creation of an arteriovenous fistula (AV fistula) for hemodialysis, autogenous venous grafts clearly show high performance when compared with prosthetic material in terms of primary or secondary patency. Polytetrafluoroethylene (PTFE) grafts for the reconstruction of AV fistulae must be restricted to cases of failure of the autogenous material, which is generally used in upper limb fistulae. We describe a case of a 52-year-old patient, who, after access failure for hemodialysis and the impossibility of performing peritoneal dialysis due to bacterial peritonitis, underwent the reconstruction of an AV fistula between the right axillary artery and the cava vein using a 6-mm PTFE prosthesis. One month after surgery, this AV fistula started to be used for hemodialysis. The AV fistula remains patent 24 months after its creation. No infectious complications, cardiac insufficiency symptoms, or steal syndromes of right upper limb were detected.

Behcets disease is form of systemic vasculitis of unknown etiology. One surprising feature is that arterial involvement is less common in this disease than venous forms, accounting for 1 to 7% of patients. In 7 to 30% of cases the vascular symptoms precede the clinical diagnosis of Behcet. We describe the case of a patient with Thromboangiitis Obliterans who was treated with lumbar sympathectomy and an aortobiiliac bypass. Around 1 year later he was diagnosed with Behcets disease after presenting with oral ulcers, genital ulcers and scaling lesions on the hands.

A amiloidose consiste em um deposito de proteinas fibrilares insoluveis em tecidos e orgaos, causando disfuncao nos mesmos, e pode contribuir, associada a outros fatores, para formacao de complicacoes, como pseudoaneurisma em locais de puncao arterial. O pseudoaneurisma consiste de uma complicacao cada vez mais frequente e, por isso, a importância de se identificarem seus fatores de risco, para que, entao, sua incidencia possa ser minimizada.
