Effects of Round Window Membrane Rupture on Cochlear Blood Flow and Inner Ear Pressures

Tsutomu Nakashima; Yusuke Watanabe; Masahide Kaida; Noriyuki Yanagita

Effects of round window membrane rupture on cochlear blood flow and inner ear pressures were investigated using non-radioactive microspheres and a servo-micropipet system in guinea pigs. When perilymphatic pressure was raised through a glass capillary tube inserted into the perilymphatic space, both endolymph pressure and perilymph pressure rose. When these inner ear pressures were raised to relatively high levels, cochlear blood vessels could be compressed and endocochlear potential decreased to a negative value due to the interruption of cochlear blood flow. However, in the case when the round window membrane was ruptured by further increase in perilymph pressure, the inner ear pressures decreased and restoration of cochlear blood flow and endocochlear potential was observed. When the round window membrane was ruptured with a fine needle under ordinary inner ear pressures, cochlear blood flow did not change significantly.

Intratympanic Injection of Gentamicin for the Treatment of Delayed Endolymphatic Hydrops

Tsutomu Nakashima; Yumiko Kaseki; Masahide Kaida; Kyoji Miyazawa

In three patients with ipsilateral delayed endolymphatic hydrops (DEH), ototoxic gentamicin sulfate was injected through the tympanic membrane for the ablative treatment of frequent attacks of vertigo. Two patients received six injections and one patient received two. Treatment produced complete control of vertigo attacks in one patient who received six injections, and substantial control in the other two patients. The present results showed that there was a delayed onset of the ototoxic effect of gentamicin in the treatment of ipsilateral DEH.

Cochlear Blood Flow After Gentamicin Induced Damage to Organ of Corti in Guinea Pigs

Masahide Kaida; Tsutomu Nakashima; Toru Suzuki; Noriyuki Yanagita

The effect of toxic damage to the organ of Corti on cochlear blood flow (CoBF) by chronic gentamicin administration was studied in guinea pigs using the nonradioactive microsphere method. Despite damage to the sensory cells, there were no changes in total CoBF or in distribution of blood flow.

Maxillary Ameloblastoma. Maxillary Prosthesis with Osseointegrated Implants; A Case Report.

We treated a 40-year-old female with recurrent right maxillary ameloblastoma. She had had a right Caldwell-Luc operation for the initial ameloblastoma. The second surgery was a right partial maxillectomy with simultaneous osseointegrated implant surgery. She was satisfied with dentures which had good retention and stability in the bone-anchored prosthesis. We consider that this technique facilitates the choice of radical surgery for maxillary ameloblastoma and reduces the risk of recurrence.

In 30 patients with peritonsillar abscess, we performed bacteriological examinations and evaluated the results of our treatment. Aerobic bacteria were isolated in 24 and anaerobic bacteria in 22 of 29 specimens aspirated from peritonsillar abscesses. Anaerobes only were recovered in 5 cases and in 17 both aerobic and anaerobic bacteria were isolated. Sulperazon® was administered by intravenous drip infusion in all 30 patients, and the results of our treatment could be evaluated in 27 patients. In all but one of the 27, both subjective symptoms and objective signs improved within 6 days after the start of treatment.Anaerobic bacteria are usually difficult to isolate. We describe methods of bacterial detection and therapy for the treatment of peritonsillar abscess.
