Potential Infectious Etiology of Behçet's Disease

Behçets disease is a multisystem inflammatory disorder characterized by recurrent oral aphthous ulcers, genital ulcers, uveitis, and skin lesions. The cause of Behçets disease remains unknown, but epidemiologic findings suggest that an autoimmune process is triggered by an environmental agent in a genetically predisposed individual. An infectious agent could operate through molecular mimicry, and subsequently the disease could be perpetuated by an abnormal immune response to an autoantigen in the absence of ongoing infection. Potentia bacterial are Saccharomyces cerevisiae, mycobacteria, Borrelia burgdorferi, Helicobacter pylori, Escherichia coli, Staphylococcus aureus, and Mycoplasma fermentans, but the most commonly investigated microorganism is Streptococcus sanguinis. The relationship between streptococcal infections and Behçets disease is suggested by clinical observations that an unhygienic oral condition is frequently noted in the oral cavity of Behçets disease patients. Several viral agents, including herpes simplex virus-1, hepatitis C virus, parvovirus B19, cytomegalovirus, Epstein-Barr virus and varicella zoster virus, may also have some role.

Erythema elevatum diutinum

A 58-year-old man was referred to our clinic with an 8-month history of red-brown nodules on the dorsal surface of his left hand. He reported that he collected fish in his home aquarium as a hobby. On this basis, a prior diagnosis of nontuberculous mycobacterial infection was made, but systemic clarithromycin was ineffective. The patient also reported a constant painful sensation, which worsened in the evening and after exposure to low temperatures. A 2-Month-Old Boy with Desquamative Skin Fold Dermatitis

A 2-month-old boy was found to have a red, desquamative dermatitis of the diaper area that had appeared 15 days after birth. He had been born at term with no perinatal problems. Our initial diagnosis was candidal intertrigo, which was confirmed by mycological exam. Unilateral eyelid involvement in discoid lupus erythematosus

Discoid lupus erythematosus (DLE) is a chronic autoimmune skin disorder, typically characterized by well-defined erythematous lesions with thick adherent scales and follicular plugging. It usually affects photo-exposed skin, particularly on the scalp, forehead and cheeks. Isolated eyelid involvement is uncommon and frequently lacks the typical clinical morphologic findings, imposing a diagnostic challenge and a potential delay in the provision of therapy.

We report a case of a 30-year-old woman with vegetative lesions in the perianal area to highlight the importance of considering perianal Crohn disease in the differential diagnosis of a resistant condylomata acuminata that does not improve after treatment.
