Pulmonary alveolar proteinosis: Two cases studied by electron microscopy

Abstract Clinical, physiologic and light and electron microscopic observations were made in two cases of pulmonary alveolar proteinosis. Apart from the occurrence of hemoptysis in one instance, the clinical features were similar to those frequently encountered in this condition. No certain etiologic agent was identified by light or electron microscopy. Endobronchial lavage was associated with improvement in one patient but had no effect in the other. The typical physiologic defect of alveolar-capillary block was present in both. Under the electron microscope the blood-air barrier was normal in width and in structure, and the laminated bodies previously observed in the alveoli with the light microscope appeared to be disintegrating alveolar cells. Fragmentation of basement membranes could be seen in some areas, and the material in the alveoli was made up of osmiophilic densities of varions sizes, some of them vacuolated, and myelin figures as well as mononuclear cells. The origin and fate of the lipid material remains uncertain.
