Elevated Postoperative Blood Glucose and Preoperative Hemoglobin A1C Are Associated with Increased Wound Complications Following Total Joint Arthroplasty

BACKGROUND Diabetes is an established risk factor for complications following total joint arthroplasty. However, the correlation between postoperative blood glucose and preoperative hemoglobin A1C levels with complications following total joint arthroplasty is not well described. METHODS All patients undergoing elective primary total joint arthroplasty at our institution from 2004 through 2011 with both postoperative blood glucose and preoperative hemoglobin A1C levels were identified in a retrospective review. From among 1702 patients, those with wound complications within thirty days after the index arthroplasty were identified. A control group matched for exact age, sex, procedure, tourniquet use, surgical approach, and use of antibiotic cement was also created. Thirty patients met the study group inclusion criteria. The mean patient age was seventy-two years (range, fifty-three to eighty-nine years); the majority (53%) of patients were female. RESULTS The odds ratio for developing a wound complication was 3.75 (95% confidence interval, 1.25 to 11.22; p = 0.02) in patients with a mean postoperative glucose of >200 mg/dL, 3.0 (95% confidence interval, 0.97 to 9.30; p = 0.08) in patients with a maximum postoperative blood glucose of >260 mg/dL, and 9.0 (95% confidence interval, 1.14 to 71.20; p = 0.03) in patients with a preoperative hemoglobin A1C value of >6.7%. CONCLUSIONS Patients with a mean postoperative blood glucose of >200 mg/dL or a preoperative hemoglobin A1C level of >6.7% are at increased risk for wound complications following elective primary total joint arthroplasty. These results show that poor preoperative and postoperative glucose control is independently associated with wound complications.

Increased long-term survival of posterior cruciate-retaining versus posterior cruciate-stabilizing total knee replacements.

BACKGROUND Considerable debate remains regarding the use of posterior cruciate-retaining or posterior cruciate-stabilizing designs for total knee arthroplasty. Multiple studies have investigated kinematic, radiographic, and clinical outcomes of both. Nevertheless, long-term survivorship analyses directly comparing the two designs have not been performed, to our knowledge. Our goal was to analyze the fifteen-year survival of posterior cruciate-retaining and posterior cruciate-stabilizing total knee replacements at our institution. METHODS A retrospective review identified 8117 total knee arthroplasties (5389 posterior cruciate-retaining and 2728 posterior cruciate-stabilizing) that had been performed from 1988 to 1998. This range was chosen because both designs were used in high volumes at our institution during this period. Patients were followed via our total joint registry at one, two, and five years after the arthroplasty and every five years thereafter. Aseptic revision surgery was the primary end point of our analysis. Implant survival was estimated with Kaplan-Meier curves. RESULTS Survival at fifteen years was 90% for posterior cruciate-retaining total knee replacements, compared with 77% for posterior cruciate-stabilizing total knee replacements (p < 0.001). In knees with preoperative deformity, the fifteen-year survival was 90% for posterior cruciate-retaining total knee replacements, compared with 75% for posterior cruciate-stabilizing total knee replacements (p < 0.04). Likewise, in knees without preoperative deformity, the fifteen-year survival was 88% for posterior cruciate-retaining total knee replacements, compared with 78% for posterior cruciate-stabilizing total knee replacements (p < 0.001). After adjustment for age, sex, preoperative diagnosis, and preoperative deformity, the risk of revision was significantly lower in knees with a posterior cruciate-retaining total knee replacement (p < 0.001; hazard ratio = 0.5; 95% confidence interval, 0.4 to 0.6). CONCLUSIONS In evaluating the implants used at our institution for total knee arthroplasty during the study period, posterior cruciate-retaining prostheses had significantly improved survival in comparison with posterior cruciate-stabilizing prostheses at fifteen years. Furthermore, this significant difference remained when accounting for age, sex, diagnosis, and deformity.

What Safe Zone? The Vast Majority of Dislocated THAs Are Within the Lewinnek Safe Zone for Acetabular Component Position

BackgroundNumerous factors influence total hip arthroplasty (THA) stability including surgical approach and soft tissue tension, patient compliance, and component position. One long-held tenet regarding component position is that cup inclination and anteversion of 40° ± 10° and 15° ± 10°, respectively, represent a “safe zone” as defined by Lewinnek that minimizes dislocation after primary THA; however, it is clear that components positioned in this zone can and do dislocate.Questions/purposesWe sought to determine if these classic radiographic targets for cup inclination and anteversion accurately predicted a safe zone limiting dislocation in a contemporary THA practice.MethodsFrom a cohort of 9784 primary THAs performed between 2003 and 2012 at one institution, we retrospectively identified 206 THAs (2%) that subsequently dislocated. Radiographic parameters including inclination, anteversion, center of rotation, and limb length discrepancy were analyzed. Mean followup was 27 months (range, 0–133 months).ResultsThe majority (58% [120 of 206]) of dislocated THAs had a socket within the Lewinnek safe zone. Mean cup inclination was 44° ± 8° with 84% within the safe zone for inclination. Mean anteversion was 15° ± 9° with 69% within the safe zone for anteversion. Sixty-five percent of dislocated THAs that were performed through a posterior approach had an acetabular component within the combined acetabular safe zones, whereas this was true for only 33% performed through an anterolateral approach. An acetabular component performed through a posterior approach was three times as likely to be within the combined acetabular safe zones (odds ratio [OR], 1.3; 95% confidence interval [CI], 1.1–1.6) than after an anterolateral approach (OR, 0.4; 95% CI, 0.2–0.7; p < 0.0001). In contrast, acetabular components performed through a posterior approach (OR, 1.6; 95% CI, 1.2–1.9) had an increased risk of dislocation compared with those performed through an anterolateral approach (OR, 0.8; 95% CI, 0.7–0.9; p < 0.0001).ConclusionsThe historical target values for cup inclination and anteversion may be useful but should not be considered a safe zone given that the majority of these contemporary THAs that dislocated were within those target values. Stability is likely multifactorial; the ideal cup position for some patients may lie outside the Lewinnek safe zone and more advanced analysis is required to identify the right target in that subgroup.Level of EvidenceLevel III, therapeutic study.

Coronal alignment in total knee replacement: historical review, contemporary analysis, and future direction

Substantial healthcare resources have been devoted to computer navigation and patient-specific instrumentation systems that improve the reproducibility with which neutral mechanical alignment can be achieved following total knee replacement (TKR). This choice of alignment is based on the long-held tenet that the alignment of the limb post-operatively should be within 3° of a neutral mechanical axis. Several recent studies have demonstrated no significant difference in survivorship when comparing well aligned versus malaligned TKRs. Our aim was to review the anatomical alignment of the knee, the historical and contemporary data on a neutral mechanical axis in TKR, and the feasibility of kinematically-aligned TKRs. Review of the literature suggests that a neutral mechanical axis remains the optimal guide to alignment.

No Benefit of Patient-specific Instrumentation in TKA on Functional and Gait Outcomes: A Randomized Clinical Trial

BackgroundAlthough some clinical reports suggest patient-specific instrumentation in TKA may improve alignment, reduce surgical time, and lower hospital costs, it is unknown whether it improves pain- and function-related outcomes and gait.Questions/purposesWe hypothesized that TKA performed with patient-specific instrumentation would improve patient-reported outcomes measured by validated scoring tools and level gait as ascertained with three-dimensional (3-D) analysis compared with conventional instrumentation 3 months after surgery.MethodsWe randomized 40 patients into two groups using either patient-specific instrumentation or conventional instrumentation. Patients were evaluated preoperatively and 3 months after surgery. Assessment tools included subjective functional outcome and quality-of-life (QOL) scores using validated questionnaires (New Knee Society Score© [KSS], Knee Injury and Osteoarthritis Outcome Score [KOOS], and SF-12). In addition, gait analysis was evaluated with a 3-D system during level walking. The study was powered a priori at 90% to detect a difference in walking speed of 0.1 m/second, which was considered a clinically important difference, and in a post hoc analysis at 80% to detect a difference of 10 points in KSS.ResultsThere were improvements from preoperatively to 3 months postoperatively in functional scores, QOL, and knee kinematic and kinetic gait parameters during level walking. However, there was no difference between the patient-specific instrumentation and conventional instrumentation groups in KSS, KOOS, SF-12, or 3-D gait parameters.ConclusionsOur observations suggest that patient-specific instrumentation does not confer a substantial advantage in early functional or gait outcomes after TKA. It is possible that differences may emerge, and this study does not allow one to predict any additional variances in the intermediate followup period from 6 months to 1 year postoperatively. However, the goals of the study were to investigate the recovery period as early pain and functional outcomes are becoming increasingly important to patients and surgeons.Level of EvidenceLevel I, therapeutic study. Epidemiology of periprosthetic fracture of the femur in 32 644 primary total hip arthroplasties: a 40-year experience

AIM AND METHODS The goals of this study were to define the risk factors, nature, chronology, and treatment strategies adopted for periprosthetic femoral fractures in 32 644 primary total hip arthroplasties (THAs). RESULTS There were 564 intra-operative fractures (1.7%); 529 during uncemented stem placement (3.0%) and 35 during cemented stem placement (0.23%). Intra-operative fractures were more common in females and patients over 65 years (p < 0.001). The majority occurred during placement of the femoral component (60%), and involved the calcar (69%). There were 557 post-operative fractures (20-year probability: 3.5%; 95% confidence interval (CI) 3.2 to 3.9); 335 fractures after placement of an uncemented stem (20-year probability: 7.7%; 95% CI 6.2 to 9.1) and 222 after placement of a cemented stem (20-year probability: 2.1%; 95% CI 1.8 to 2.5). The probability of a post-operative fracture within 30 days after an uncemented stem was ten times higher than a cemented stem. The most common post-operative fracture type was a Vancouver AG (32%; n = 135), with 67% occurring after a fall. In all, 36% (n = 152) were treated with revision arthroplasty. CONCLUSION In summary, intra-operative fractures occur 14 times more often with uncemented stems. Female patients over 65 years of age are at highest risk. Post-operative fractures are also most common with uncemented stems, but are independent of age or gender. Cumulative risk of post-operative periprosthetic femur fracture was 3.5% at 20 years. TAKE HOME MESSAGE Intra-operative fractures occur 14 times more often with uncemented stems, particularly with female patients over 65 years of age, while post-operative fracture risk is independent of age or gender, but still increased with uncemented stems.

New rabbit knee model of posttraumatic joint contracture: Indirect capsular damage induces a severe contracture

Reported models of joint contracture fail to result in severe motion loss. Our purpose was to develop a rabbit model of knee contracture and compare it to another well‐accepted model to determine if more severe stiffness can be achieved. Sixteen skeletally mature New Zealand White female rabbits had their right knee operated to create 3‐mm defects on the noncartilaginous portions of the femoral condyles, hyperextend the joint to disrupt the posterior capsule, and immobilize the joint in maximum flexion with a Kirschner‐wire for 8 weeks (group I). Sixteen additional rabbits were operated on using an identical protocol except for the absence of capsular injury (group II). In each group, mechanical testing was performed at the time of Kirschner‐wire release in eight animals, and 16 weeks after remobilization in eight animals. At immobilization release, the average contracture was 76 ± 24° in group I versus 20 ± 10° in group II (p < 0.001). Sixteen weeks after remobilization, the mean contracture was 49 ± 15° group I versus 11 ± 10° in group II (p < 0.001). When associated with bone perforations and immobilization in forced flexion, injury to the posterior capsule results in a severe contracture in the rabbit knee.

Increased Aseptic Tibial Failures in Patients With a BMI ≥35 and Well-Aligned Total Knee Arthroplasties

The purpose of this study was to calculate the risk of revision secondary to aseptic tibial loosening correlated with increased BMI in 5088 primary TKAs. The mean age was 69 years, with a mean follow-up of 7 years. Fifty-two (1.0%) revision TKAs were performed due to aseptic tibial loosening, with the 15-year risk being 2.7%. Patients with a BMI ≥35 kg/m(2) were significantly more likely to undergo revision due to aseptic tibial failure (HR=1.9; P<0.05). Coronal alignment was equivalent between those who did and did not experience tibial loosening. Given that the risk of revision TKA due to aseptic tibial component failure is 2-fold greater in those with a BMI of ≥35 kg/m(2), consideration should be given to additional fixation. Seronegative infections in hip and knee arthroplasty: periprosthetic infections with normal erythrocyte sedimentation rate and C-reactive protein level

The aim of our study was to describe the characteristics, treatment, and outcomes of patients with periprosthetic joint infection (PJI) and normal inflammatory markers after total knee arthroplasty (TKA) and total hip arthroplasty (THA). In total 538 TKAs and 414 THAs underwent surgical treatment for PJI and met the inclusion criteria. Pre-operative erythrocyte sedimentation rate (ESR) and C-reactive protein level (CRP) were reviewed to identify the seronegative cohort. An age- and gender-matched cohort was identified from the remaining patients for comparison. Overall, 4% of confirmed infections were seronegative (21 TKA and 17 THA). Of those who underwent pre-operative aspiration, cultures were positive in 76% of TKAs (n = 13) and 64% of THAs (n = 7). Cell count and differential were suggestive of infection in 85% of TKA (n = 11) and all THA aspirates (n = 5). The most common organism was coagulase-negative Staphylococcus. Seronegative infections were associated with a lower aspirate cell count and a lower incidence of Staphylococcus aureus infection. Two-stage revision was performed in 35 cases (95%). At a mean of five years (14 to 162 months) following revision, re-operation for infection occurred in two TKAs, and one THA. From our study we estimate around 4% of patients with PJI may present with normal ESR and CRP. When performed, pre-operative aspirate is useful in delivering a definitive diagnosis. When treated, similar outcomes can be obtained compared with patients with positive serology.
