Complete blood count alterations due to the opioid use: what about the lymphocyte-related ratios, especially in monocyte to lymphocyte ratio and platelet to lymphocyte ratio?

ABSTRACT This study was designed to compare the complete blood count values of opioid users (N = 61) and healthy subjects (N = 61), particularly monocyte-to-lymphocyte ratio (MLR) and platelet-to-lymphocyte ratio (PLR). PLR, MLR, and percentage of monocyte (MONO%) were significantly lower in opioid use disorder (OUD) group (P = 0.012, P = 0.005, P = 0.000). The area under the ROC curve of MLR and PLR levels for OUD was 0.349 and 0.368. MONO% correlated with substance use duration. Measurements like lymphocyte-related ratios and MONO% in opioid use can be important in substance monitoring, detection, and differentiation of acute and chronic conditions.

Intercourse type of situational anejaculation or inability to ejaculate intra-vaginally: three case reports from a conservative islamic community

ABSTRACT Anejaculation is defined as the inability to ejaculate semen despite sexual stimulation by intercourse or masturbation. It may result from organic or psychological causes and can be classified as orgasmic, anorgasmic, and situational. Situational anejaculation may be present in intercourse type. This means existence of ejaculation by masturbation, but not during sexual intercourse. Performance anxiety, hostility toward the partner, dysfunctional psychosexual development, and unconscious desire to avoid pregnancy are the possible underlying conditions. We herein report situational anejaculation in 24-, 26-, 34-year-old males who presented with inability to ejaculate intra-vaginally since they got married. All of them were the sole males in their family, had a childhood in a conservative community, and had arranged marriages. Due to the negative aspects of psychosexual counselling in rural societies, it was not possible to continue the follow-up and treatment of patients. Despite this, the authors conclude that this clinical course of situational intercourse anejaculation suggests a psychological problem in these patients.

Improvement of an atypical Kikuchi-Fujimoto disease (KFD) with antidepressant treatment: the first psychiatric approach to a KFD case

ABSTRACT Kikuchi-Fujimoto disease (KFD) is a sporadic and benign disorder of the lymph nodes of young individuals. A preceding fever, occasional skin rashes, and lymphopenia, suggest a viral aetiology and there have been reports of viral association. However, so far, no infectious agent has been proved to be aetiologically related. We herein report KFD in a 27-year-old female who presented with fevers, weight loss and tender cervical lymph nodes. The patient had depressive symptoms before the onset of KFD. During the disease process, depressive symptoms worsened with the KFD course. She had not relieved with the supportive treatment and major depressive disorder (MDD) became more severe gradually. After the treatment of MDD and with the improvement of mental health, clinical symptoms and the lymph node growth became reversed. The authors conclude that this clinical course of KFD with the treatment of MDD suggests an immune system response or immunological problem in these patients.

Cotard and Capgras delusions in a patient with bipolar disorder: “I’ll prove, I’m dead!”

ABSTRACT Cotard is a syndrome that is characterized by ideas of damnation or rejection, anxious melancholia, insensitivity to pain, and nihilistic delusions concerning one’s own body or existence. It is most often encountered in middle age or older women who are severely depressed. Capgras syndrome is a rare psychiatric disorder with colourful symptoms. The patient believes that the identities of close relatives or friends are not real but are replaced by others. Co-existences of psychiatric and organic diseases with Cotard’s syndrome and Capgras syndrome are reported in different studies. There is still requirement of more research to establish a position in diagnostic classification systems for these syndromes which are thought to have a multifactorial etiology. In this report, we described a patient with bipolar disorder type-2 who displayed comorbid Cotard and Capgras delusions which were most evident at the onset of menstrual periods.

Mean platelet volume and neutrophil to lymphocyte ratio as parameters to indicate the severity of suicide attempt

ABSTRACT This study was designed to compare the complete blood count (CBC) values of 38 violent suicidal attempt (VSA) and 38 nonviolent suicidal attempt (NVSA) patients and 38 healthy controls, particularly mean platelet volume (MPV) and neutrophil to lymphocyte ratio (NLR). MPV was found to be significantly higher (P = 0.000), and PLT was found to be significantly lower in the VSA group (P = 0.001). NLR was found to be significantly higher (P = 0.005) in the VSA group. The area under the receiver operating characteristic curve of MPV levels for VSA was 0.78 and 0.68 for NLR in the VSA group when compared with the NVSA group.
