Yuksek konsantrasyonlarda CO’e maruz kalmak oldurucu olabilir ve dunya genelinde zehirlenmelere bagli olumlerin en sik nedenidir. CO’e maruziyet sonrasi, miyokard iskemisi, kalp yetmezligi ve aritmi gibi kardiyak belirtiler bildirilmistir. 28 yasinda bir hasta, soba kaynakli akut CO’e maruz kalmanin bir sonucu olarak bilinc kaybi ile acil servise kabul edildi. Karboksihemoglobin seviyesi % 39 idi. Hemen oksijen tedavisine baslandi ve terapotik eritrosit degisimi yapildi. Elektrokardiyogramda supraventrikuler tasikardi (SVT) saptandi ve ekokardiyografik incelemede normal kardiyak fonksiyonlar goruldu. Bizim bilgimize gore bu calisma, akut CO zehirlenmesine bagli ikinci SVT olgusu raporudur. Bu yazida CO ile zehirlenmis hastalardaki bu komplikasyona yaklasim tartisilmaktadir.

The aim of this study is to evaluate patients with pulmonary edema due to periodical consumption of licorice syrup. The patients who consumed licoric syrup in recently and admitted to emergency department with dyspnea were examined comprehensive during Ramadan in 2011 and 2012. They underwent routine biochemical investigations, chest X-rays, and electrocardiogram assessment. In addition, cardiac function was examined using echocardiography. Information relating to patients detected with interstitial pulmonary edema was recorded, and all recorded data were evaluated. The number of patients diagnosed with pulmonary edema in the 2-month period was 82. Thirty two of these patients (39%) had a history of consumption of licorice syrup. At first assessment, the mean systolic blood pressure of the study participants was 160.5±18.3 mmHg (130-220), and diastolic blood pressure was 92.6 ±9.8 mmHg (75- 120). The mean sodium level was 143.2±3.47 mmol/L (134-149), potassium was 3.4±0.3 mmol/L (3,1-4,3), urea was 56.1±18.9 mg/dl (26-114), and creatinine was 1.1±0.27 mg/dL (0,6-1,8). The injection fraction of 12 patients was under 40%. Of the study participants, 19 were hospitalized in a coronary intensive care unit, while 13 were treated with an emergency department. All study participants recovered and were subsequently discharged from hospital. Excessive consumption of licorice syrup is a hidden cause of pulmonary edema and/or hypertension and therefore should be investigated for in individuals diagnosed with these conditions.

Scorpion stings are common in many regions of the world, particularly in rural areas. While most of the stings are harmless and tend to be milder, some stings rarely have severe clinical course, including neurological, cardiovascular and respiratory system complications. Although there are many studies in the literature related to the scorpion sting, data on effects of scorpion stings in pregnant woman are very little. The authors have not come across any case report of eclampsia as a complication of scorpion sting. With this study, the authors aimed to discuss a scorpion sting lead to an unexpected complication, eclampsia.

Recognizing arrest victim, starting early chest compressions, activating the emergency medical services and knowing the steps of Basic Life Support especially the chest compression for bystanders who witness the sudden collapse of an adult will have positive effects on the mortality of out-of-hospital cardio-pulmonary arrest cases. In present case we discussed a cardiopulmonary arrested patient to whom cardiopulmonary resuscitation was applied by an untrained savior.

Ozet Bitkisel ilaclar, geleneksel ilaclar ile hayal kirikliginin bir sonucu olarak dunya ge- nelinde giderek daha populer hale gelmektedir. Toplum onlari genellikle zararsiz olarak kabul etmektedir. Ancak bu urunler veya bunlarin metabolitleri karaciger hasari gibi bazi yan etkilere neden olabilir. Bu calismada bitkisel cay icimine bag- li akut hepatit gelisen iki hasta sunulmustur. Yirmi alti ve otuz yasinda iki kadin; mide bulantisi, kusma, istahsizlik ve halsizlik sikâyeti ile klinigimize basvurdu. Se- rum alanin aminotransferaz ve aspartat aminotransferaz duzeyleri artmisti. Viral hepatit ve otoimmun hastaliklar icin tum serolojik testleri negatif bulundu. Onlar sut yapimini artirmak icin uc dort hafta boyunca her gun rezene ve kimyon iceren bir bitkisel cay icmislerdi. Bitkisel cayin kesilmesinden dort-bes hafta sonra ka - raciger enzimleri normale dondu. Bizim bilgimize gore bu muhtemelen, rezene ve kimyon iceren bir bitki cayi kullanimi ile ilgili ilk hepatit rapordur. Bu calisma, re- zene ve kimyon iceren bitkisel cayin akut hepatitin diger nedenleri ile klinik olarak karisabilen hepatotoksisiteye sebep olabilecegini gostermektedir. Klinisyenler ko- layca tanisi konulamayan akut hepatit olgusu ile karsi karsiya kalabilirler ve bitki- sel ilac kullanimi konusunda hastayi sorgulamalari gerekmektedir. Abstract

Renal angiomyolipoma (AML) is relatively a rare benign tumour including vascular smooth muscle, and fatty elements; and the majority of renal AML run an asymptomatic, benign course. Potentially life-threatening complication of renal AML is tumour rupturing that can be seen after a low-velocity trauma. Flank pain and haematuria may be considered not important in emergency department if underlying cause is not kept in mind. In present study, the authors aimed to discuss a patient who developed ruptured AML during physical examination.
