Cervical cancer screening in Hungary – epidemiological, historical and methodological aspects

The issue of cervical cancer has been raised again recently, as opposed to other states of the European Union our country shows a high incidence and mortality rate of cervical carcinoma. Although in the 21st century not a single woman should die of cervical cancer, cervical cancer claims the lives of approximately 500 women in Hungary annually until this day. The most typical point of development is where the columnar epithelium of the cervical canal and the squamous epithelium of the uterine cervix meet, the so called transformation zone (squamocolumnar junction). The disease is a several year long process of squamous epithelium metaplasia. This is what provides the opportunity for screening, as by recognizing the lesion in a precancerous state, treatment is possible prior to the development of a tumor. Authors review some epidemiological, historical and methodological issues related to cervical cancer screening.

Uterine papillary serous carcinoma

A kórképet elsőként Lauchlan és mtsai említették szövettani szempontból teljesen különálló elváltozásként [3]. Hendrikson és mtsai megállapították, hogy a daganat viselkedése – a méhtestráknál általában tapasztaltakkal ellentétben – igen agresszív, gyakoribb a myometrium mély infi ltrációja, gyakrabban lépnek fel nyirokcsomóáttétek, és magasabb arányban fordul elő daganatkiújulás a felső hasüregben [4]. Bokhman 1983-ban két fő csoportba sorolta a méhtestrákokat. Az I. csoportba az endometrioid szövettani típusú daganatok (80%), míg a II. csoportba a seropapillaris és világos sejtes rákok kerültek [5]. Megállapították, hogy a két fő típus között igen jelentős különbségek vannak mind az etiológiát, mind a kórjóslatot tekintve. A II. típusú daganatok az atrófi ás nyálkahártyában alakulnak ki, nem ösztrogéntúlsúly talaján. A metabolikus szindróma jelenléte az I. típusú daganatokra jellemző. Az SPC biológiai viselkedése inkább a petefészekrákéhoz hasonló, de a kórjóslat még rosszabb, kevésbé érzékenyek kemoterápiára [2, 5].

Hungarian women's knowledge about cervical cancer

Abstract Introduction The human papillomavirus infection is one of the most frequent sexually transmitted diseases. The presence of human papillomavirus is demonstrable in nearly all cases of cervical cancer. Cervical cancer is the second most frequent tumourous disease among women, not just in the world but in Hungary. Aim Our aim was to estimate womens knowledge of the human papillomavirus and cervical cancer. We searched connection between the frequent mortality, the low participation rate of screenings and the knowledge of cervical cancer. Methods The related questionnaire contained 18 questions and was filled in by 422 women in cities and villages. The lower age limit of the randomly canvassed people was 14 years, but we reached older women as well. The incoming data were processed with a computer program.
