Long-term Results of Cryotherapy on Malignant Epithelial Tumors of the Conjunctiva

Gonul Peksayar; Mete K. Soytürk; Misten Demiryont

We studied the results of combined treatment with excision and cryotherapy with the nitrous oxide probe in 22 eyes of 20 patients with intraepithelial tumors or squamous cell carcinomas of the conjunctiva. Patients were followed up for five to 12 years. Only two recurrences (9%) were seen, both of which occurred within the first two years. Over the long term, excision combined with cryotherapy decreased the recurrence rate of intraepithelial tumors and squamous cell carcinomas of the conjunctiva.

We report 4 cases of malignant peripheral nerve sheath tumors (MPNST) with neurofibromatosis type 1 (NF1). Mean age was 29.5. Two of them had a family history. Three of them were male. All of them had enlarging mass and pain in the background of neurofibromas. Locations were popliteal, thigh and forearm. The masses were greater than 5 cm in diameter in each case. In two cases the mass was showing continuity with a nerve. One patient had a nonossifying fibroma as well as a MPNST. Wide excision and radiotherapy were applied to three of the patients. One of them did not take any therapy after surgical resection. Two of the patients died of lung metastases after a mean period of 12.5 months. In a majority of NF1 patients MPNST emerges from a preexisting neurofibroma. The patients with NF1 are at greatest risk for developing sarcomas, so they should be followed closely.

a a m i r i t c a t T m p a t t l n D n t pithelial odontogenic tumors arise from odontoenic epithelial structures. Malignant epithelial dontogenic tumors are extremely rare. Malignant meloblastoma, ameloblastic carcinoma, primary nterosseous squamous cell carcinoma, clear cell dontogenic carcinoma, and malignant epithelial host cell tumor are the components of this roup. The clinical course and histopathology of hese tumors are similar to each other, so the ifferential diagnosis is not easy to make. Ameloblastic carcinoma is an aggressive tumor ith a poor prognosis. It can be seen in different ges, but especially in the third decade, and 80% f the patients have ameloblastic carcinoma of the andible. The patients usually present with the ymptom of a painful, rapidly growing mass. Reional and remote metastases, especially lung meastasis, is a characteristic of the tumor. Recurence after resection is frequent. In the literature, ameloblastic carcinoma arising rom preexisting ameloblastoma is very rare. e present the case of a patient with an amelolastic carcinoma arising from recurrence of an meloblastoma.

Synovial sarcoma is a soft tissue sarcoma of unknown histogenesis and occurs predominantly in the lower extremities of young adults. The head and neck is a relative rare location. There are about 10 cases with laryngeal localization in the literature. We present a 24 year-old male with an endolaryngeal mass. Incisional biopsy and the hemilaryngectomy material revealed a biphasic synovial sarcoma. One year later a local recurrence occurred. Tumor excision and neck dissection were performed. Radiotherapy was added. Six months later lung metastases was discovered on thoracic CT. The patient received chemotherapy for 6 courses. The metastases responded well to chemotherapy and the patient is now alive without tumor on radiological and clinical examination after 3.5 years of follow-up.

PURPOSE Alteration in the p53 tumor suppressor gene is the most common tumor specific genetic change identified in most major cancer types including rhabdomyosarcomas. To investigate the overexpression of p53 and its relation to clinical features and outcome in patients with rhabdomyosarcoma (RMS), an immunocytochemical study was performed. METHODS Formalin-fixed paraffin embedded tissue sections obtained from 42 cases of RMS were immunostained with a mouse monoclonal antibody p53-D07. Staining was assessed by evaluating the percentage of p53 immunopositive cancer cell nuclei. RESULTS Nuclear accumulation of p53 protein was detected in 8 of 42 (19%) samples. Clinical analyses of patients demonstrated no correlation between positive staining and age, sex, histological subtype, stage and overall survival. This analysis, however, was limited by the small number of patients who demonstrated p53 immunostaining. Nonetheless, a statistically significant association was observed between p53 expression and adverse outcome. Nuclear p53 expression was associated with disease progression or recurrence (p <0.001) and with a worse event free survival (p = 0.0015). CONCLUSION The nuclear p53 immunoreaction rate is low in RMS, but p53 expression appears to correlate with poor prognosis.

An unusual metastatic site of laryngeal carcinoma: scapular muscles

Emre A. Yucel; Tayfun Demirel; Misten Demiryont; Unal Egeli; Kemal Deger

It is well known that in advanced cervical metastatic disease of head and neck squamous cell carcinoma, the incidence of distant metastasis is high. A case of distant metastasis to the scapular muscles from an uncontrolled cervical metastasis of laryngeal carcinoma is presented. Other unusual metastatic sites are reviewed and the possible mechanism of the spread is discussed.

Hydatid Cystic Disease of the Soft Tissues with Pulmonary and Hepatic Involvement: Report of a Case

Sukru Dilege; Murat Aksoy; Ismail Okan; Alper Toker; Goksel Kalayci; Misten Demiryont

Abstract.Hydatid disease, which is caused by the Echinococcus granulosis parasite, is endemic in certain parts of the world where close contact between humans and sheep exists. The disease most commonly involves the liver and other solid organs. This report describes the case of a teenage girl with hydatid disease involving the right axilla and various other sites concomitantly. The patient was admitted to hospital with a mass in the right axillary region, and further examinations revealed that the liver and right lung were also involved. We report this case to emphasize that hydatid disease should be considered in the differential diagnosis of patients from endemic areas who present with soft tissue masses.

Prognostic value of proliferating cell nuclear antigen immunostaining in pediatric rhabdomyosarcomas

Gülnur Tokuç; Oner Dogan; Inci Ayan; Rejin Kebudi; Sema DogAn; Misten Demiryont; Hakan Camlica

Background : The levels of proliferating cell nuclear antigen (PCNA) are almost negligible in long‐term quiescent cells and increase dramatically during the cell cycle. Recently, the monoclonal antibodies to PCNA have been used to demonstrate the proliferative component of paraffin‐embedded tumor tissues. It has been shown to be available as a simple histological marker of proliferative activity and the PCNA labeling index has been correlated with the prognosis of several malignant neoplasms.

Rupture of a Silicone Gel Mammary Prosthesis and Amyloidosis: A Case Report

Ufuk Emekli; Burçak Tümerdem; Misten Demiryont

There has been a great deal of controversy regarding the safety of silicone breast implants. Silicone gel prostheses older than 10 years have the risk of either leakage or outright rupture. The role of silicone gel in relation to connective tissue disease or amyloidosis has not been proved by current serologic, immunologic, or epidemiologic tests. In this paper, we present a case with renal amyloidosis and a history of untreated silicone implant rupture for five years.
