Cardiac cephalgia: A headache of the heart

Atypical presentations of common diseases are often difficult to identify in time, nonetheless necessary, especially in cases of diseases like ischemic heart disease (IHD) which otherwise may progress into irreversible stage and ultimately, fatality, in the absence of timely administered medical treatment. We report a case of a 51-year-old male, presenting initially with only headache as the sole symptom, who later was diagnosed with severe coronary artery disease, and the symptoms resolved completely after coronary angioplasty with stenting. <Learning objective: Atypical presentations, such as headache, can be more prevalent in ischemic heart disease (IHD) than was thought of, so awareness of the possibility of IHD while assessing headache and further study to see actually how commonly headache is associated with IHD are necessary.>.

Situs inversus totalis is the mirror-image of normal position of the thoracic and abdominal viscera. It may be detected incidentally when the patient seek medical attention for other medical illness. From medico-legal points of view, this rare disorder is important in many ways for a junior physician to prevent a big mishap, especially surgical. Here, we report a case, who was 55- year-old, seeking medical attention for infective exacerbation of chronic obstructive pulmonary disease incidentally diagnosed as a case of situs inversus totalis. REDUCING VOLTAGE DROP IN DISTRIBUTION FEEDERS

Two number 11KV feeders namely Mandra and Kalyam of Rawalpindi Subdivision feeding different categories of loads have been selected as case study. The single line data has been collected through actual field visit and survey, from which single line diagrams have been generated. Using computer software programme, computer Aided Distribution Planning and Design (CADPAD) the input data has been analyzed for the existing conditions of the feeders under consideration. After detailed study, the different voltage drop reduction techniques such as Bifurcation, Reconductoring, Load Balancing and Capacitor Application have been applied to bring the voltage drop into as international standard i.e. ± 5%. During the study the main emphasis has been given on exploiting ways and means to upgrade the existing outdated distribution system not only to provide a high level of service to public at present but also to make it flexible to such an extent that it will accommodate/future anticipated load quite adequately.
