Relación entre el síndrome metabólico y la mortalidad por cardiopatía isquémica en España

La rotura subaguda de la pared libre del ventriculo izquierdo es una complicacion de la fase aguda del infarto de miocardio. La presentacion subaguda permite una resolucion quirurgica. No se conoce si la manipulacion del pericardio y el empleo de parches pueden tener consecuencias a medio plazo en la funcion ventricular izquierda. Nuestro objetivo es evaluar la evolucion de la funcion ventricular izquierda y el desarrollo de datos de constriccion a medio plazo en pacientes con rotura subaguda del ventriculo izquierdo intervenida quirurgicamente. Se intervino de rotura subaguda a 11 pacientes, de los que se siguio a medio plazo a 6. Los resultados muestran que a medio plazo hay una mejoria discreta de la funcion ventricular y no aparecen datos de constriccion. Como conclusion, se puede decir que es una tecnica segura a medio plazo y que no se acompana de secuelas en la funcion ventricular izquierda ni de desarrollo de constriccion.

Cardiología preventiva y rehabilitación cardiaca

The Preventive Cardiology and Rehabilitation Section of the Spanish Society of Cardiology provides the principal national reference point for scientific knowledge about cardiovascular risk factors in the Spanish population, about the incidence and prevalence of cardiovascular disease in Spain, and about disease prevention and the use of cardiac rehabilitation to improve the quality of life and prolong the survival of individuals already affected by the disease. By necessity, research into cardiovascular prevention involves sponsoring the implementation of studies into cardiovascular risk factors and disease occurrence. The MESYAS study is a good example. It is the fruit of the combined efforts of members of the metabolic syndrome working group belonging to our Section of the Spanish Society of Cardiology and its aim was to answer a number of unresolved questions that had arisen about the incidence, prevalence and consequences of cardiovascular disease in the Spanish population. Today, after this cohort has been followed up for more than 5 years, the first analysis of the cardiovascular events that occurred in the cohort and their relationship with individual risk factors observed many years previously has become available. Here, the metabolic syndrome working group reports some interesting findings. With the aim of communicating up-to-date information that has undergone expert review, the coordinator of the cardiac rehabilitation working group has the responsibility of summarizing, evaluating and updating the scientific data available on the important subject of the secondary prevention of cardiovascular disease.

Relationship Between Metabolic Syndrome and Ischemic Heart Disease Mortality in Spain

Morbidity and mortality due to ischemic heart disease (IHD) is subject to wide geographic variation both between and within countries. The aim of this study was to determine whether geographic variations exist in the prevalence of metabolic syndrome in the Spanish working population or in its relationship with IHD mortality. We analyzed clinical and laboratory data obtained during health check-ups carried out in Spanish workers (n=17,837) during 2003. The prevalence of metabolic syndrome was 17% in men and 6.5% in women. However, there was a heterogeneous distribution across the different regions studied. The prevalence in southern and western regions (e.g., in men: 22.15% in Extremadura and 20.6% in Galicia) was double that observed in central and northern zones (e.g., in the Basque Country and Castile and León). This research indicates that there is a significant association between IHD mortality and the prevalence of metabolic syndrome in workers from different Spanish regions.

Perfil psicológico diferencial entre dolor torácico de causa no cardiológica y enfermedad coronaria: un estudio controlado

El objetivo del trabajo es identificar variables que permitan diferenciar el dolor toracico de causa no cardiologica (DTCNC) de la enfermedad coronaria (EC). Se realizo un estudio observacional, de casos (pacientes con DTCNC, n = 40) y controles (pacientes con EC, n = 40). Se analizaron variables sociodemograficas, medicas y psicologicas. No existian diferencias en personalidad, excepto en el rasgo «control emocional», menor en DTCNC. Estos enfermos presentaron mayor alexitimia y empleaban mas frecuentemente dos estrategias de afrontamiento: la religion y la busqueda de ayuda medica. Cuando se realizo un analisis multivariable, el modelo compuesto por las siguientes cuatro variables: alexitimia, calidad de vida y afrontamientos basados en la religion y en la busqueda de ayuda medica muestra una sensibilidad del 85,4% y una especificidad del 80%. Este modelo predictivo podria emplearse como test de screening para diferenciar ambos trastornos.
