Abstract Of 48 patients given selective mesenteric arterial infusion of vasopressin for the control of gastrointestinal hemorrhage, 28 were actively bleeding from varices secondary to portal hypertension, 14 were infused electively at the time of portosystemic shunt surgery, and 6 were infused because of hemorrhage from an arterial or capillary source demonstrated on prior selective arteriography. The indications, technique, and results of the vasopressin infusions are described.

A case of congenital anomalies of rotation and fixation of the midgut with volvulus in an adult is described. The diagnosis of volvulus was based on the characteristic angiographic appearance or “barber pole sign.” Demonstration of vascular obstruction secondary to volvulus led to successful surgical correction, thus avoiding fatal complications.

Abstract Eight patients with massive arterial gastrointestinal hemorrhage whose source of bleeding was demonstrated by selective mesenteric arteriography underwent subselective intra-arterial infusion of vasopressin with complete control of their bleeding. Seven of these eight patients left the hospital without requiring surgery. The eighth patient underwent right colectomy while still in the hospital in order to correct vascular ectasia of the cecum. The technic of this procedure and some of its pitfalls have been described.

A case of nonocclusive mesenteric vascular insufficiency in a patient with congestive heart failure on digitalis and with arrhythmia is presented. The preoperative diagnosis was made through mesenteric arteriography. A course of treatment was followed which we believe prevented intestinal gangrene. This regimen includes measures to increase cardiac output, local pharmacologic agents to induce mesenteric vasodilatation, antibiotics, and oxygen. The viability of the intra-abdominal organs was demonstrated by laparotomy.

Abstract We have found direct serial magnification arteriography to be an exceedingly valuable addition to our diagnostic armamentarium. It is anticipated that with further refinement in x-ray tube design we will perhaps be able to see even further advances in the in vivo demonstration of vascular changes in various disease processes.

Abstract Preoperative emergency arteriography demonstrated bleeding from the junction of the second and third portions of the duodenum in 5 of 8 patients with stress bleeding. Bleeding possibly occurred at this point because of (1) a vascular anatomy leading to hypoperfusion and (2) the effects of digitalis. The incidence of bleeding from the mid-duodenum may be underestimated in the literature because of difficulty of its detection by barium examination, surgery, or autopsy, and because lack of hematemesis may lead to nonrecognition of internal bleeding during life.
