Endoscopic ultrasonography of the esophagus

Endoscopic ultrasonography (EUS) is a generally accepted technique for the preoperative staging of malignant tumors in the upper and lower gastrointestinal tracts. In particular, EUS has been considered the method of choice in diagnosing esophageal carcinoma due to the relative ease in performing the examination and the accuracy of staging based on high-resolution ultrasonic imaging from within the lumen of the esophagus. This comprehensive review covers currently available EUS instruments, image characteristics of esophageal carcinoma, and images by the recently introduced miniprobe scanner. The role of the method in diagnosing superficial esophageal carcinoma and the possible treatment by endoscopic mucosal resection of this particular disease entity are discussed.

Diagnostic précoce du carcinome pancréatique grâce à l’E.R.C.P. et à la tomographie ultrasonique

RésuméL’E.R.C.P. a été une méthode efficace de diagnostic des maladies pancréatiques. Mais l’E.R.C.P. seule, n’est pas suffisante pour le diagnostic des carcinomes pancréatiques au début.La tomographie ultrasonique s’est développée pour le diagnostic des tumeurs abdominales grâce à l’utilisation de méthodes telles que l’échographie à échelle de gris.Dans ce rapport, nous décrivons l’utilité des deux méthodes (E.R.C.P. et tomographie ultrasonique) pour le diagnostic. Dans les cas où il y a eu résection, nous avons examiné et comparé l’E.R.C.P. pré-opératoire avec la pancréatographie post-opératoire et les schémas ultrasoniques avec l’examen des tumeurs.L’E.R.C.P. est utile pour diagnostiquer la localisation de la tumeur mais elle présente des inconvénients pour déterminer la taille, les régions infiltrées, les métastases lymphoïdes et la résécabilité. Elle n’est pas suffisante pour réaliser un diagnostic différentiel de carcinome pancréatique et de pancréatite.Grâce aux examens ultrasonographiques, nous n’avons relevé aucune plainte des patients ni complication. L’examen avait pour but de déterminer la localisation et la taille des tumeurs, la dilatation de la voie biliaire principale, des canaux intra-pancréatiques et la résécabilité. Cette dernière est en relation avec l’influence des tumeurs sur les vaisseaux.SummaryE.R.C.P. has been an effective method for the diagnosis of pancreatic diseases. But E.R.C.P. alone is not sufficient for the diagnosis of early stages of pancreatic carcinomas. Ultrasonic tomography has been developped to diagnose abdominal tumors by the use of advanced methods such as greyscale echography. In this report, the usefulness of diagnosis with both E.R.C.P. and ultrasonic tomography is described.In cases where resection took place, we examined and compared preoperative E.R.C.P. with postoperative pancreaticography and ultrasonic figures with examination of the tumors. E.R.C.P. was useful for the diagnosis of location of the tumor, but it had disadvantages for the determination of size, infiltrated area, lymphnode metastases and resectability. It was not sufficient to perform differential diagnosis of pancreatic carcinoma and pancreatitis. A CASE OF PARATHYROID HORMONE PRODUCING CARCINOMA OF LIVER

本来hormoneを産生しない臓器から発生した腫瘍組織がhormoneを産生するものを異所性hormone産生腫瘍と呼んでいる.この異所性hormone産生腫瘍は,癌細胞の偏倚および発癌機構の解明に示唆を与える材料として注目されている.異所hormone産生腫瘍の直接的表付けは,腫瘍組織中に含まれるhormoneが有意に高いことの証明である.今回われわれは,63才,男性で,肝腫張,腹水を来たして来院し,α-fetoproteinが強陽性で,超音波断層像,血管造影などから原発性肝癌と診断し,経過中に著明な高Ca血症を認めた症例を経験した.本例の血清,腹水および腫瘍組織中に異常高値のPTHをradioimmunoassayにより証明し,剖検後の病理組織学的検索では,脇上皮化性型腺癌の像が主体であるが,索状配列の部位もあり多彩な縁を呈した.本例はα-fetoproteinと異所性hOrmoneの産生機序を考察するうえで示唆に富む症例と考えられたので報告する.
