Effect of experimentally altered occlusal vertical dimension on temporomandibular articulation.

Abstract The occlusal relations of the teeth of monkeys were experimentally altered to increase the vertical dimension of occlusion for a period of 90 days. This procedure resulted in pathologic changes in the temporomandibular articulation which included extensive resorption of bone in the subchondral regions as well as of the bone along the anterior and posterior borders of the neck of the condyle and along the floor of the glenoid fossa. The articular cartilage of the condyle became thickened, and the condyle appeared to have been flattened.

Improved healing of experimental defects in the canine mandible by grafts of decalcified allogenic bone.

Abstract Standardized surgical defects were made on the buccal surface of canine mandibles. Autologous and decalcified allogenic grafts of canine bone were placed in these defects and allowed to heal for a period of from 2 to 8 weeks. Histologic study revealed that the decalcified allogenic bone grafts underwent complete resorption which was accompanied by new bone formation. The autologous grafts maintained their viability, were not resorbed, and were accepted by the host tissues. Bone regeneration appeared to be somewhat more rapid in decalcified allogenic grafts as compared to autologous grafts. These results suggest that decalcified allogenic bone grafts should be studied in more detail, for they could be of use in the treatment of bony defects in the facial bones.

Effects of soft dietary consistency and protein deprivation on the periodontium of the dog: A preliminary report

Abstract Manifestations of periodontal disease produced in dogs by a combined regimen of protein deprivation (with alternate repletion) and soft dietary consistency have been described. Clinical, roentgenographic, and histologic aspects of the observed lesions were presented. The noteworthy features of the oral tissue response were extensive calculus deposition, associated gingival inflammation and hyperplasia, periodontal pocket formation, and severe resorption of alveolar and supporting bone. The significance of the foregoing observations was discussed in relation to the ecology of human periodontal disease.

Abstract The ecologic, clinical, microbiologic, histologic, and therapeutic aspects of noma (gangrenous stomatitis) have been reviewed. A case report has been presented, with a discussion of clinical, roentgenographic, microbiologic, and therapeutic aspects, which emphasizes the significant contributions of the pooled knowledge of a hospital team in bringing about the successful resolution of a case of noma superimposed upon existing physical debilitation and malnutrition.

Visualization of cellular dynamics associated with orthodontic tooth movement

ORTHODONTIC tooth movement appears to be dependent upon the presence Of osteoblasts and osteoclasts within the periodontal ligament and adjacent bone.l-’ A force applied to a tooth creates areas of pressure and tension. In areas of pressure, bone resorption, in association with the appearance of osteoclasts, is stimulated. In areas of tension, osteoblasts arise and form osteoid, which is subsequently calcified. The bone remodeling process allows the tooth to move and thus maintain the physiologic width of the periodontal ligament.lm7 It has been presumed that osteoblasts and osteoclasts a,re derived from the proliferation and differentiation of undifferentiated mesenchymal cells, since increased mitotic activity of mesenchymal cells is observed in the supporting tissues of a tooth subjected to an orthodontic force.*, 3l 7 In fact, however, the histogenesis of these cells is in doubt. On the other hand, cellular proliferation can be documented by the use of radioactive precursors of DNA, because DNA is duplicated during cell division. The appearance of radioactivity within cell nuclei indicates the presence of “new” DNA which had been synthesized in an early phase of the process of cell division.8 Therefore, using a DNA precursor, tritiated thymidine, we investigated the role of cellular proliferation during orthodontic tooth movement, with emphasis on the formation of osteoclasts and osteoblasts.

Desquamative gingivitis and its response to topical triamcinolone therapy.

Abstract A concept of desquamative gingivitis has been expressed. The condition probably is a reflection, clinically, of a variety of etiologic factors—both local and somatic in origin; in the majority of cases these influences cannot be elicited with clarity, and it is necessary to direct therapy to local alleviation of the gingival manifestations. The topical administration of triamcinolone acetate in Orabase constituted an effective form of therapy. The histopathologic characteristics of desquamative gingivitis include the detachment of epithelium, either intra- or subepithelially, with a variably severe connective tissue inflammatory reaction; it is difficult to collate this response to a specific etiologic agent.

Renal-cell carcinoma metastatic to the mandible and gingiva

A number of malignant tumors are capable of metastasizing to the oral tissues. These include adenocarcinomas derived from such areas as the lung,l* 2 thyroid,3 breast,4 rectum,5 uteruqG prostate,i and kidney.8-‘0 Robbins and Stockdale’l describe the probable pathways for metastasis of these lesions to the structures of the head, with a marked propensity for secondary localization in the mandible. Tumor emboli apparently enter the prevertebral plexus of veins (Batson) to reach the spinal column and skull. Invasion of the renal vein and the lymphatic system may also occur. Two qualities of the cancer cell tend to enhance the frequency of metastasis by embolization. These are (1) lack of cohesiveness of cancer cells to each otherI and (2) stickiness-the tendency of cancer cells to cling to a new surfa.ce.f” Normal cells, particularly epithelial, are noted for their resista.ncc to mechanical separation and their desire to seek contact.14 The intravasation of t)umor cells is possibly a function of (1) a structural opening in the vessel wall, whether it be a true defect or the normally weak approximation of lymphatic endothelium, (2 j a break in the vessel’s surrounding connective tissue. and/or (3 1 a prolonged or transient rise in extra.vasc,ular pressure.15 Tnt,ra.vasation may be further facilitat,ed by repeated trauma to the primary lesion; iatrogenic manipulation may also be responsible.
