Sport-Related Maxillo-Facial Fractures.

AbstractSports and exercise are important causes of maxillofacial injuries. Different types of sports might differ in frequency and type of fractures. The aim of the present study was to explore the possible relation between the types of sport practiced and the frequency and nature of the facial bone fractures of patients presenting in an oral and maxillofacial surgery department of a Dutch university center. This study is based on an analysis of patient records containing maxillofacial fractures sustained between January 1, 2000 and April 1, 2014 at the Vrije Universiteit University Medical Center (VUmc) in Amsterdam, The Netherlands. The present study comprised data from 108 patients with 128 maxillofacial fractures. Seventy-nine percent of the patients were male and 21% were female. The patients ranged in age from 10 to 64 years old with a mean age of 30.6 ±12.0. The highest incidence of sport-related maxillofacial fractures occurred in individuals between the ages of 20 and 29. The most common sport-related fractures were zygoma complex fractures, followed by mandible fractures. Soccer and hockey were the most prominent causes of sport-related maxillofacial trauma in the present study. Coronoid process fractures were only observed in soccer players and not in other sports groups. Mandible angle fractures were relatively more frequent in rugby than in other sports. The results of this study suggest a relation between type of sport and the nature and frequency of the fractures it causes.

Dental trauma in association with maxillofacial fractures: an epidemiological study

AIM The aim of this study was to retrospectively investigate the incidence and associated factors of dental trauma in patients with maxillofacial fractures at the VU Medical Center in Amsterdam. MATERIAL AND METHODS Data from 707 patients who were treated surgically for maxillofacial fractures were evaluated. The data were collected retrospectively from patient files and other available databases. The data collected included date of fracture, age, gender, type of fracture, and injured teeth. RESULT Of the total 707 patients, 164 patients (23.2%) presented dental injuries associated with facial fractures. Mandibular condylar fractures, mandibular parasymphyseal fractures, Le Fort fractures, and mandibular body fractures were found to be significantly more associated with dental injury. Zygomatic arch or zygomatic complex fractures were significantly less associated with dental injury. Women had a significant higher risk of facial fractures with dental injuries than men. The maxilla demonstrated the highest incidence of injured teeth. The most affected teeth were the maxillary incisors (33.1%), followed by mandible incisors (13.6%), mandible molars (12.8%), and maxillary premolars (12.6%). CONCLUSION Our findings show a higher risk of dental injury among patients with a mandibular condylar fracture and mandibular parasymphyseal fracture but a lower risk of dental injury among patients with a zygomatic arch or zygomatic complex fracture. On average, patients had more than three injured teeth, with most of the injured teeth being in the upper jaw. The maxillary incisors, followed by the mandible incisors, were the most injured teeth.

The potential of the stem cells composite hydrogel wound dressings for promoting wound healing and skin regeneration: In vitro and in vivo evaluation

In this study, the wound healing properties of the gelatin-based hydrogel (GBH) wound dressing combined with adipose-derived stem cells (ADSCs) were investigated using the mouse and porcine models. The analytical results showed that the ADSCs harvested from the porcine significantly increased cell growth and promoted cell differentiation (adipogenesis and osteogenesis) in comparison to the ADSCs harvested from the mouse in vitro. Moreover, the in vivo results also indicated that the GBH wound dressing combined with ADSCs and its culture medium could potentially accelerate wound healing in the mouse and porcine models. The ADSCs presented a possibility of recovery from wounds and injuries through skin regeneration. Therefore, both in vitro and in vivo results demonstrated that the ADSCs can potentially be an effective clinical treatment through the GBH wound dressing, which is a promising evidence-based complementary and alternative medicine for skin regeneration.

Utilization of pedicled buccal fat pads for coverage of the lateral relaxing wound: A review of literature and a case series of 15 patients

Background The buccal fat pad (BFP) is an encapsulated mass originated from a specific fat tissue in various volume throughout the life of each person and BFP has been used in various surgeries as a source of useful graft material due to its easy accessibility and rich vascularization. Case Report This report describes fifteen patients who were treated with buccal fat pads (BFP) as a pedicled graft for lateral relaxing wound closure in primary cleft palate surgery. A review of relevant literature is also presented. Results All patients had a mean follow-up of 3.7 weeks with a minimum follow-up time of three weeks and a maximum follow-up of four weeks. All patients who had an uneventful immediate postoperative period showed signs of BFP epithelialization characterized by a yellowish tissue beginning in the first week and ending within 3–4 weeks after surgery. Conclusions The use of BFP for the small to medium-sized defects reconstruction in palatoplasty is a safe and reliable method with fast healing. Even older patients who would not be able to tolerate time-consuming flap reconstruction procedures had good results. Key words:Cleft palate, buccal fat pads, lateral relaxing wound.

The Epidemiology, treatment, and complication of ameloblastoma in East-Indonesia: 6 years retrospective study

Background Ameloblastoma is a neoplasm classified as a benign epithelial odontogenic tumor of the jaws, grow slowly and are locally invasive. The aim of the present study was to investigate the incidence, treatment, and complication of patients with ameloblastoma in East-Indonesia during six years retrospective study. Material and Methods This retrospective study included 84 patients who were diagnosed with ameloblastoma from 2011 to 2016. There were 56 patients with treatment data available. Data from each patient, including gender, age, histologic type, the size of the tumor, radiologic form, tumor location, type of treatment, and complication were reviewed and analyzed retrospectively. Results Fourteen patients were diagnosed with unicystic ameloblastoma (25%), thirty two patients with multicystic follicular ameloblastoma (57%) and ten patients with an unspecified multicystic ameloblastoma (18%). A total of about 35 patients were treated conservatively (62.5%) and 21 patients were treated radically (37.5%). Swelling was present as a pre-operative complication in all 56 cases (100%). There were no complaints concerning speech. Conclusions The majority findings of the histologic type were multicystic ameloblastoma and their location were in the mandible. Most ameloblastoma were treated conservatively and reconstructions were made with only titanium plates and not bone graft. Key words:Ameloblastoma, epidemiology, east Indonesia.

Assessing the need for a protocol in monitoring weight loss and nutritional status in orthognathic surgery based on patients experiences

Background To investigate retrospectively the orthognathic surgery (OGS) patients experience in weight loss and the influence of gender, age, duration of the surgical procedure, length of hospital stay, location of surgery and use of intermaxillary fixation (IMF) or without IMF on postoperative weight loss. Material and Methods A total of 4487 patients treated by OGS where all patients visited the outpatient clinic one, three and six weeks after the surgical procedure. After six weeks, patients filled out a questionnaire in which weight loss was addressed. The patients were asked to give an estimate of their experiences weight loss. The population was first divided in two groups weight loss and no weight loss. Results In the weight loss group there is no significant difference in weight loss between patients with IMF and patients without IMF. In the weight loss group there were significantly more females then males. Further, in the subgroup IMF the operation time was significantly longer compared with the subgroup without IMF. The other parameters including age and hospital stay were not different in the groups. Conclusions IMF in orthognathic treatment does not result in a difference self-reported loss of body weight compared to patients without IMF. Treatment protocols should include pre- and post-operative dietician consultations and possible indications for medical nutrition and vitamins. The differences use of panoramic photo and CBCT to evaluation of the gnatoplasty surgery outcome

Gnatoschisis is the most common congenital craniofacial abnormality patients with Cleft Lip and/or Palate (CL&/P). Gnatoschisis cases underwent gnatoplasty. Panoramic radiograph is one means to establish a diagnosis and treatment plan with two-dimension photograph (2D). Cone-Beam Computed Tomography (CBCT) is one means of supporting the maxillofacial to get three-dimension photograph (3D). The objective of this study is to determine the difference in the results of the use of panoramic photos and CBCT in evaluation before and after gnatoplasty surgery. In conclusion, CBCT results more accuracy to evaluation gnatoplasty surgery, however the radiation exposed should be considered. In other hand, panoramic has lower radiation exposure.

Osteomielitis supuratif kronis pada mandibula edentulus Chronic suppurative osteomyelitis on edentulous mandible

Osteomyelitis is an infection of the jaw that extends to the jaw bone, which is the spongy, bone marrow, cortex, and periosteum. Infection occurs in the calcified bone when fluid in the medullary cavity or under the periosteum interfere with the blood supply. In this case, it was reported an elderly patient with a diagnosis of chronic osteomyelitis of mandible edentulous dextra. The treatment were sequesterectomy and extraction the radix of tooth 43 by general anesthesia, and combinating with antibiotics. It was concluded that patient of chronic suppurative osteomyelitis of the mandible edentulus could be diagnosed through clinical and radiological examination, and successfully treated with sequesterectomy and extracting the radix under general anesthesia. So the improvement of systemic condition, food nutrition, vitamin therapy, accelerate the healing process. Treatment of focal infection as soon as possible to avoid more severe complications. Key word: chronic osteomyelitis, mandible, edentulous ABSTRAK Osteomielitis rahang adalah suatu infeksi yang meluas pada tulang rahang, yang mengenai spongiosa, sumsum tulang, korteks, dan periosteum. Infeksi terjadi pada bagian tulang yang terkalsifikasi ketika cairan dalam rongga medula atau di bawah periosteum mengganggu suplai darah. Dalam kasus ini dilaporkan seorang pasien yang berusia lanjut dengan diagnosis osteomielitis kronis pada mandibula edentulus kanan. Penanganan yang dilakukan adalah sekuesterektomi dan pencabutan sisa akar gigi 43 dalam kondisi anastesi umum, dan dikombinasi dengan pemberian antibiotik. Disimpulkan bahwa penderita osteomielitis supuratif kronis pada mandibula edentulus dapat didiagnosis melalui pemeriksaan klinis dan radiologi, dan sukses dirawat dengan pilihan perawatan sekuesterektomi, dan pencabutan sisa akar di bawah keadaan anastesi umum. Jadi perbaikan keadaan umum, nutrisi makanan, terapi vitamin, mempercepat proses kesembuhan. Penanganan fokal infeksi sesegera mungkin menghindari komplikasi yang lebih berat. E-mail: leliya/[email protected] PENDAHULUAN Osteomielitis rahang adalah suatu infeksi yang sifatnya ekstensifpada tulang rahang,yangmengenai spongiosa, sumsum tulang, korteks,dan periosteum. Infeksi terjadi padabagian tulang yang terkalsifikasi ketika cairan dalam rongga medula atau di bawah periosteum mengganggu suplai darah. Tulang yang terinfeksi menjadi nekrosis ketika terjadi iskemia. Perubahan pertahanan host yang mendasar terdapat pada mayoritas pasien yang mengalami ostemielitis pada rahang. Kondisi-kondisi yang mempengaruhi persarafan tulangmenjadikan pasien rentan terhadap berlangsungnya ostemielitis.Kondisi tersebut, antara lain sinar radiasi,osteoporosis,osteopetrosis,penyakit tulang Paget, dan tumor ganas tulang. Penatalaksanaan osteomielitis supuratif kronis pada mandibula edentulus membutuhkan kerja sama yang intensif dengan bagian medis lainnya. Selain itu, tingkat keberhasilan perawatan juga ditentukan oleh kondisi sistemikpenderitasecara umum.Berikut ini dilaporkan penatalaksanaan kasus osteomielitis supuratif kronis yang terjadi pada mandibula yang edentulus. KASUS Pasien laki-laki yang berusia 60 tahun, datang ke RS Hikmah, Makassar dengan keluhan sering keluar darah pada dagu sebelah kanan yang disertai nanah dan sudah berlangsung selama 1 tahun.Tidak adakeluhanrasanyeri pada daerah tersebut.Keadaan umum pasien, antara lain demam (-), alergi (-), rinitis (-), asma (-), batuk (-), riwayat pembedahan sebelumnya (-), merokok (+), edentulus rahang atas dan rahang bawah (+). Keadaan umum lainnya dari pasien baik dengan kesadaran kompos mentis, tekanan darah 140/80 mmHg,nadi68x/menit,pernapasan16x/menit,teratur, dengan suhu afebris. Pada pemeriksaan fisis, pada daerah ekstraoral terdapatfistuladanpusyangkeluarpada daerah dagu kanan (gambar 1A,B).Sedangkan pada pemeriksaan intraoral tampak adanya edentulus pada daerah

Penatalaksanaan impaksi gigi kaninus rahang atas Management of impacted maxillary canine

Impacted canines can be detected in early age. The clinicians might be able to prevent them by means of proper clinical diagnosis, radiographic evaluation and timely interceptive treatment. Various surgical and orthodontic techniques can be used to recover impacted maxillary canines. Surgical technique used to manage impacted canines including gingivectomy, apically positional flap, and closed eruption technique. The aim of this review is to provide the management of impacted maxillary canines. Combination between surgical and orthodontic techniques are the effective method to manage impacted maxillary canines with good result. Impaksi gigi kaninus dapat dideteksi pada usia muda. Dokter gigi dapat mencegahnya dengan membuat diagnosis klinis yang tepat, evaluasi radiografis dan perawatan pencegahan. Berbagai teknik bedah dan ortodontik dapat digunakan untuk merawat impaksi gigi kaninus RA. Teknik bedah yang digunakan untuk penanganan impaksi gigi kaninus rahang, atas meliputi gingivektomi, flap apikal, dan teknik erupsi tertutup. Tujuan penulisan ini yaitu untuk menentukan penatalaksanaan impaksi gigi kaninus RA. Kombinasi antara teknik bedah dan ortodonsik merupakan metode yang efektif digunakan untuk merawat impaksi gigi kaninus rahang agar diperoleh hasil yang lebih baik.

Penatalaksanaan disgnati kelas II skeletal dengan bilateral sagital split osteotomy

This case reported a female patient aged 30 years was treated with orthognathy surgery in Hasan Sadikin Hospital, Bandung. Based on clinical and radiological examinations, model study and photography, diagnosis of skeletal dysgnathy class II was established. The orthodontic treatment is a preparation before surgical operation to achieve stability in optimal dental interdigitation. In operation, BSSO, chinplasty, and V-Y plastic upper lip as well as intermaxillary fixation (IMF) ligation was carried out at the dentofacial position class I. At day-16, radiography was carried out to evaluate the surgical outcome and the bone position. At day-21, the opening of jaws was 1.5 cm; and at day-42, the patient was reconsulted to orthodontist, and to physical medicine and rehabilitation In month-3, occlusion, facial profile and harmony, and jaw functions showed the satisfying outcome, though for the interdigitation stabilization, the rubber elastics was still installed.
