Conservative treatment for complete dislocation of the acromioclavicular joint by plaster spica. - Second report.

In this report, the conservative treatment for the complete dislocation of acromioclavicular joint by plaster spica is discussed.Sixteen patients were treated, 15 being men and one woman. The average age was 32 years (range, 19-43). We applied plaster spica with 90 degrees arm abduction to immobilize the scapula and reduce the distal end of the displaced clavicle using elastic band. Thus managed, the articular surface of the acromioclavicular joint contacts each other closely. The alignment of reduced acromioclavicular joint must be confirmed by X-ray for 4 to 6 weeks. 13 cases completed treatment in this manner but 3 cases gave up treatment halfway owing to severe pain or failure of reduction. Follow-up periods averaged 1 year and 7 months.Pain relief was generaly satisfactory but anatomical reduction was obtained in only 6 of the 13 completed cases. The relations between reduction and the shape of the acromioclavicular joint were discussed in this series according to Urists classification. 8 of the 13 cases were type II (Vertical) or type V (Incongruent & No contact) and the other 5 were type I (Overriding) or type IV (Incongruent & Overriding). 6 cases which gained anatomical reduction in this series were all classified into type II and type V.Anatomical reduction seemed to be important for the improvement of function and the relief of symptoms. The results were not so good in older patients because of residual contracture.

From the Operated Cases of Spinal Cord Tumor

We have reported the end-result of 23 cases of the spinal cord tumor which were operated on our clinic from 1960 to 1969. The result was as follow:Exeellent: 5, Good: 1, Fair: 7, Unchanged: 5, Worse: 1, Death: 4. 14 cases were neurinoma. We have reported 6 interesting cases from these cases.Case I which had 4 neurinomas into the different spinal cord.Case II which had 6 neurinomas in the different nerves include the spinal cord neurinoma.Case III which had disc henna and spinal cord tumor.Case IV haemoangioma which could not take out.Case V cholesteatoma.Case VI which had spinal cord cancer without spinal bone metastase.We have found the good result in the operated cases of spinal cord tumor within a half year from leg complaint had appeared.

Follow-up Studies of the Patients with Low Back Pain treated by Lumbago-band

For a means of the therapy of low back pain, the lumbagoband is prescribed.During the past two years (1966-1967), 456 patients with low back pain were treated by putting on the lumbago-band. For the purpose of investigating the effects of these lumbagobands, questionnaires were sent out and answers gathered from 223 patients. Here we report and discuss about these results.

Arthroplasty for the Rheumatoid Arthritis of the Elbow. -3 cases report-

It is told that the results of arthroplasty for severe rheumatoid arthritis of the elbow are usually satisfactory. Three patients with good ligaments and muscles were operated by means of J K. Membrane Arthroplasty in our clinic for these 3 years. Two patients have satisfactory result in the range of motion and stability after 1 year and 2 years of surgery.The other has gained the ankylosis again.
