Age-related reduction in 99Tcm-DTPA alveolar-capillary clearance in normal humans.

Age is known to reduce the efficacy of body organs and systems, even in the absence of disease. The alveolar-capillary clearance (ACC) rate is representative of the alveolar-capillary barriers functional state. We studied 29 healthy non-smokers, who were selected after clinical and radiographic evaluation. The patients were divided into three groups based on age: Group I, < or = 30 years (n = 10); Group II, 31-55 years (n = 9); Group III, > or = 56 years (n = 10). Each patient inhaled 750 MBq 99Tcm-diethylenetriamine pentaacetate (99Tcm-DTPA) aerosol generated by a Venticis nebulizer (particles with a mean diameter of 1.1 microns) for 5 min. Forty frames of 30 s duration each were acquired and the ACC rates for the right and left lungs determined using a computer program. The mean ACC rates for the three groups were as follows: Group I, 1.31% min-1; Group II, 1.08% min-1; Group III, 0.76% min-1. The differences between Groups I and III (P < 0.001) and Groups II and III (P = 0.03) were shown to be significant. There was no significant difference between Groups I and II. Possible explanations for an age-related reduction in ACC rates include a reduction in the internal alveolar surface, the closure of the small airways, a reduction in the lung blood capillaries and a reduction in cardiac output. We conclude that there appears to be an age-related reduction in ACC rates in healthy non-smokers, even in the absence of clinically and radiographically detectable lung disease. However, larger studies are required.

The safest and most efficient way to diagnose congenital hypothyroidism (CH) is through screening programs using serum thyroid stimulating hormone (TSH). CH occurs in one in 1:2,300-1:5,000 births but higher rates are found in iodine-deficient areas. Iodine was measured in the urine of 141 newborns (87 males and 54 females) from Ribeirao Preto to complement the screening program developed by the School of Medicine of Ribeirao Preto (FMRP-USP) and Federal University of Sao Paulo (UNIFESP). TSH values did not disclose any case of CH, although we have seen an elevated ratio (0.96%) of calls for retests. The iodine urinary levels ranged from 2.1 to 194 microg/l (mean 58.3+/-36.2 microg/l). No differences between the levels of urinary iodine and gender or gestational ages were observed. A negative correlation between urinary iodine and TSH of blood from umbilical cord was found (r= -0.20, p= 0.02).

Valor preditivo do exame clínico, cintilografia, ultra-sonografia, citologia aspirativa e tiroglobulina sérica no nódulo tiroideano único atóxico: estudo prospectivo de 110 pacientes tratados cirurgicamente

Thyroid nodules are common in medical practice. To establish the predictive values of diagnostic tests we studied prospectively the results obtained from physical examination, aspiration cytology, scintigraphy, ultra-sonography and pre-operative serum thyroglobulin in 110 patients operated and compared them with the results obtained from surgical pathology specimens. We found 124 lesions in the 110 patients: 106 (85%) benign and 18 (15%) malignant. The benign were 90 simple goiters (73%), 8 follicular adenomas (6%), 4 thyroglossal cysts (3%) and 4 other rare lesions; the malignant were 16 (13%) primary thyroid carcinomas (9 papillary, 3 follicular, 3 undifferentiated and 1 medullary) and 2 secondary thyroid carcinomas. Aspiration cytology presented the best results (sensitivity: 94%, specificity: 97%), compared with scintigraphy (sensitivity: 89%, specificity: 21%), ultra-sonography (sensitivities from 60 to 100%, specificities from 25 to 69%) and pre-operative serum thyroglobulin (very low sensitivity and high specificity). Few clinical signs and symptoms also presented high specifity, but low sensitivity, as hoarseness, dysphagia, new lesion, cervical adenomegaly and previous local irradiation. We conclude that aspiration cytology is the best available method to diagnose thyroid nodules.

Hypothyroidism Affects Pulsatile LH Secretion in Pubertal Orchiectomized Rats

The present study evaluated the effect of hypothyroidism on the pituitary-testicular axis in rats rendered hypothyroid on the beginning of puberty. Rats were treated with propylthiouracil for 8 weeks and killed for determination of hormonal status and body parameters. For determination of pulsatile LH secretion other animals were orchiectomized two weeks before sampling. Analysis of the results led us to conclude that although the absolute weight of sex organs tended to decrease in hypothyroid animals, the relative weights were equal or higher than control, suggesting that the development of these organs were not affected by hypothyroidism; the androgenic activity of hypothyroid rat testes were preserved; basal plasma levels of pituitary hormones were similar in control and hypothyroid groups; the pulsatile LH secretion showed a decrease in the number of pulses, nadir mean and total LH secretion in hypothyroid animals. Our results demonstrate that although hypothalamic-pituitary axis of hypothyroid pubertal rats displays an abnormal pulsation LH release, no evidences of abnormalities in the reproductive system functions were found.
