Chitotriosidase and soluble IL‐2 receptor: Comparison of two markers of sarcoidosis severity

Background. Sarcoidosis is a multisystemic granulomatous disease with an unpredictable clinical course characterized by accumulation of activated proliferating T lymphocytes and mononuclear phagocytes in affected organs. Aims and methods. The aims of this study were to describe the clinical, radiological and immunological features of a population of sarcoidosis patients followed at the Sarcoidosis Regional Centre in Siena and to analyse chitotriosidase and sIL‐2R concentrations in serum of these patients in order to understand their potential as disease markers. Results. Chitotriosidase and sIL‐2R concentrations in serum of sarcoidosis patients were found to be significantly higher than in healthy controls (p<0.01) and a positive correlation between the two markers was documented for the first time. Moreover, chitotriosidase and sIL‐2R were expressed differently in different radiographic stages of the disease. Conclusion. Chitotriosidase and sIL‐2R are two markers of sarcoidosis of different origin, the values of which show a correlation in these patients; they are easily detectable in serum and could be useful clinical markers of progression.

Calgranulin B (S100A9) Levels in Bronchoalveolar Lavage Fluid of Patients with Interstitial Lung Diseases

Calgranulins are small calcium-binding proteins with several immunological functions involved in inflammatory processes. Calgranulin A is reported to be mainly associated with acute inflammation while calgranulin B seems to play a role in chronic inflammatory disorders. In this study we used a proteomic approach to analyse calgranulin B expression in bronchoalveolar lavage (BAL) from a group of patients with different interstitial lung diseases. Two dimensional electrophoresis analysis of BAL was performed in 11 idiopathic pulmonary fibrosis patients, nine sarcoidosis patients, 11 with systemic sclerosis patients and five healthy controls. Significantly higher (p < 0.001) calgranulin B percentage volumes were observed in BAL from IPF patients than controls and other ILD patients. This result sustains the hypothesis that calgranulin B could be involved in chronic lung diseases, probably through increased expression and enhanced activation of alveolar polymorphonuclear cells related to idiopathic pulmonary fibrosis. Quantitative analysis by an easier method applied to a larger population will be necessary to determine whether calgranulin B could be a good marker of disease severity.

Posidonia oceanica as a biomonitor of trace elements in the Gulf of Naples: temporal trends by lepidochronology.

Levels of Hg, Cd, Pb, Cu, Zn, Fe, and Mn were assessed by using Posidonia oceanica as a bioindicator in the Gulf of Naples (southern Italy). Lepidochronology, which enables retroactive dating of scales and rhizomes of this seagrass, was combined with atomic spectrometry to assess temporal trends of trace elements in dated scales and rhizomes over a 10-year period. Lepidochronology occasionally has been used to monitor Hg in dated scales, but never has been used to determine concentrations of other trace elements in dated rhizomes. Data were compared between various structural parts of seagrass and between sampling sites. Concentrations of some elements found in dated scales or rhizomes showed a similar trend in most of the sites; increasing for Hg and Cu and decreasing for Pb, Fe, and Zn. Contaminant levels indicated by seagrass meadows varied from metal to metal. Concentrations of Hg, Cd, and Pb fell within the ranges measured in other areas considered to have low levels of heavy metal pollution, whereas Cu, Mn, and Zn had higher concentrations only in some years. However, on the basis of other studies, none of the sampling sites are considered to have high contamination levels.

Organotropism of persistent organic pollutants and heavy metals in the Greenland shark Somniosus microcephalus in NE Greenland.

The Greenland shark Somniosus microcephalus is an opportunistic feeder, a top predator, and a very long-lived species. The brain, liver, red and white muscle, gonads, fat, skin, pancreas, and spleen of Greenland sharks from NE Greenland fjords were analysed for PCBs, PCDDs/DFs, PBDEs; DDT isomers; HCH isomers; dieldrin; endrin; HCB; Cd, Hg, Pb, and Se. PCBs (2.01-103 ng/g wet wt) and PBDEs (7.9-3050 pg/g wet wt) were detected in most of the samples. PCDDs/DFs showed high values when detected. DDTs, HCB and HCHs were only detected in some tissues. The ΣTEQ was 5.76 pg/g in muscle. Cadmium mainly accumulated in the pancreas and liver (19.6 and 10.7 mg/kg dry wt, respectively); mercury in red muscle (4.10-6.91 mg/kg dry wt); selenium in the pancreas (3.57 mg/kg dry wt) and spleen (1.95 mg/kg dry wt); lead in the skin (0.358 mg/kgd ry wt). The selenium-mercury ratio in the liver was also evaluated.

Cadmium, lead, and mercury levels in feathers of small passerine birds: Noninvasive sampling strategy

Bird feathers have been widely used as a nondestructive biological material for monitoring heavy metals. Sources of metals taken up by feathers include diet (metals are incorporated during feather formation), preening, and direct contact with metals in water, air, dust, and plants. In the literature, data regarding the origin of trace elements in feathers are not univocal. Only in the vast literature concerning mercury (as methyl mercury) has endogenous origin been determined. In the present study, we investigate cadmium, lead, and mercury levels in feathers of prey of Falco eleonorae in relation to the ecological characteristics (molt, habitat, and contamination by soil) of the different species. Cluster analysis identified two main groups of species. Differences and correlations within and between groups identified by cluster analysis were then checked by nonparametric statistical analysis. The results showed that mercury levels had a pattern significantly different from those of cadmium and lead, which in turn showed a significant positive correlation, suggesting different origins. Nests of F. eleonorae proved to be a good source for feathers of small trans-Saharan passerines collected by a noninvasive method. They provided abundant feathers of the various species in a relatively small area--in this case, the falcon colony on the Isle of San Pietro, Sardinia, Italy.

Pleiotropic effects of hexavalent chromium (CrVI) in Mytilus galloprovincialis digestive gland.

Hexavalent Chromium Cr(VI) is an important contaminant considered as a model oxidative toxicant released from both domestic and industrial effluents, and represents the predominant chemical form of the metal in aquatic ecosystems. On the other hand, in mammals the reduced form Cr(III) is considered an essential microelement, involved in regulation of lipid and carbohydrate metabolism; moreover, recent evidence suggests that Cr may have endocrine effects. In this work, the effects of Cr(VI) were investigated in the digestive gland of the marine bivalve Mytilus galloprovincialis. Mussels were exposed to 0.1-1-10-100 μg Cr(VI) L(-1) animal(-1) for 96 h. At 100 μg L(-1), a large increase in total Cr tissue content was observed; in these conditions, the lysosomal membranes were completely destabilized, whereas other lysosomal biomarkers (neutral lipids-NL and lipofuscin-LF), as well as different enzyme activities and gene expression were unaffected, this indicating severe stress conditions in the tissue. On the other hand, at lower concentrations, changes in other histochemical, biochemical and molecular endpoints were observed. In particular, at both 1 and 10 μg L(-1), lysosomal destabilization was associated with significant NL and LF accumulation; however, no changes in catalase and GSH transferase (GST) activities were observed. At the same concentrations, GSSG reductase (GSR) activity was significantly increased, this probably reflecting the recycling of GSSG produced in the GSH-mediated intracellular reduction of Cr(VI). Increased activities of the key glycolytic enzymes PFK (phosphofructokinase) and PK (pyruvate kinase) were also observed, indicating that Cr(VI) could affect carbohydrate metabolism. Cr(VI) induced downregulation or no effects on the expression of metallothioneins MT10 and MT20, except for an increase in MT20 transcription in males. Moreover, significant up-regulation of the Mytilus estrogen receptor MeER2 and serotonin receptor (5-HTR) were observed in both sexes. The results demonstrate that exposure to Cr(VI) in the low ppb range did not result in strong toxicity or oxidative stress conditions in mussel digestive gland. On the other hand, our data support the hypothesis that low concentrations of the metal can exert pleiotropic effects on mussel physiology, from modulation of lipid and carbohydrate metabolism, to effects on the expression of estrogen-responsive genes.

Analysis of Trace Elements in Bronchoalveolar Lavage of Patients with Diffuse Lung Diseases

Airborne trace elements are implicated in the etio-pathogenesis of a large number of pulmonary diseases. The aim of this study was to evaluate the reliability and effectiveness of direct determination of Cd, Cr, Cu, Fe, Mn, Ni, Pb, V, and Zn concentrations in bronchoalveolar lavage (BAL) samples from patients with sarcoidosis, idiopathic pulmonary fibrosis, and Langerhans cell histiocytosis and healthy (smoking and non-smoking) controls. A total of 44 individuals were recruited among sarcoidosis, idiopathic pulmonary fibrosis, and Langerhans cell histiocytosis patients and healthy (smoking and non-smoking) controls. Average Mn concentrations in BAL from patients were 45% lower than in controls (p < 0.01) and remarkable decreases in average concentrations of Cr, Ni and Zn were also found in BAL from patients with idiopathic pulmonary fibrosis and Langerhans cell histiocytosis. As these diseases are characterized by the enhanced activation of certain immunomodulatory cells and by generation of free radicals, the depressed Mn, Zn, Cr and Ni concentrations in BAL from patients may be due to oxidative stress. These preliminary results indicate that assessment of the elemental composition of BAL is a promising approach to study the pathogenesis of diffuse lung diseases and Langerhans cell histiocytosis.

Calgranulin B (S100A9/MRP14): A Key Molecule in Idiopathic Pulmonary Fibrosis?

Calgranulin B is a small calcium-binding protein with several immunological functions mainly involved in chronic inflammation and cancer. It can participate in recruitment of neutrophils and leukocytes in inflamed tissue, oxidant/antioxidant balance, adhesion of neutrophils to fibronectin, and regulation of apoptosis. In a previous proteomic study, we found that calgranulin B was up-regulated in the bronchoalveolar lavage (BAL) of patients with idiopathic pulmonary fibrosis (IPF) with respect to controls and patients with other interstitial lung diseases. The aims of this study are to compare calgranulin B concentrations in BAL of patients with IPF and sarcoidosis and controls by a quantitative method, to look for correlations with clinical data, and to evaluate calgranulin B expression in lung tissue of IPF patients by immunohistochemistry. A modification of a commercial ELISA was used to determine calgranulin B concentrations in BAL of 16 patients with IPF (a group of patients in which we previously performed proteomic analysis), 17 patients with sarcoidosis, and 7 controls. The immunohistochemistry was done in a subgroup of patients with IPF and a control group of lung transplant donors. Calgranulin B concentrations were significantly higher in patients with IPF than controls (p < 0.01); they were inversely correlated with FVC and DLCO values and directly correlated with neutrophil and eosinophil percentages in BAL. Immunohistochemistry revealed a patchy distribution of calgranulin B, predominantly around areas of fibrotic remodeling. Calgranulin B may be a trigger molecule involved in the evolution and progression of IPF, being overexpressed in BAL of patients with IPF with severe functional deterioration and in the peribronchiolar area bordering zones of honeycombing.

Serum Analysis of Coagulation Factors in IPF and NSIP

Recent literature and our previous proteomic findings prompted us to study the coagulation system in idiopathic pulmonary fibrosis (IPF), the pathogenesis of which remains unclear. The aim of this study was to compare coagulation factors in idiopathic pulmonary fibrosis and idiopathic nonspecific interstitial pneumonia (NSIP) patients and healthy controls. Thirty-three IPF patients (23 acute exacerbation and 10 stable IPF patients), 7 NSIP patients, and 44 controls were enrolled. Concentrations of D-dimer, homocysteine, functional protein C, protein C antigen, free and total protein S antigen and activity, fibrinogen and factor VIIIc were analyzed in serum of patients and controls. The lupus anticoagulant (LAC) test was also performed. Factor VIIIc levels were significantly higher in acute exacerbation IPF patients than controls (p = 0.0001) and in stable IPF patients than controls (p = 0.002). Factor VIIIc levels were higher and PT levels were lower in acute exacerbation IPF patients who died after exacerbation than in patients who survived (p = 0.04 and p = 0.003, respectively). D-dimer, fibrinogen, and homocysteine levels were also significantly higher in IPF patients than controls (p < 0.01). Protein C activity was increased in acute exacerbation IPF patients than controls (p = 0.005). The LAC test was positive in seven IPF patients and negative in controls. Procoagulant status was demonstrated in IPF patients (mainly in acute exacerbation/IPF) than controls and NSIP patients, probably due to endothelial activation and microvascular injury. These preliminary results are of interest because of their potential implications in the pathogenesis and treatment of this disease.

Heavy metals in flamingos (Phoenicopterus ruber) from Italian wetlands: the problem of ingestion of lead shot.

Waterbirds are exposed to many contaminants, including lead from ingestion of shot and fishing sinkers. Lead poisoning had never been reported in flamingos wintering in Italian wetlands. Our investigation stems from a case of four flamingos found dead in Tuscany in 2002 with numerous lead shot in their gizzards. We therefore considered other specimens found dead in different Italian wetlands. Many lead shot found in gizzards and lead tissue concentrations confirmed the hypothesis of lead poisoning in two of the seven specimens analysed: concentrations in liver, kidney, and bone were 361.3, 265.09, and 43.31mug/g d.w., respectively. Lead organotropism was typical of acute poisoning. Cadmium and mercury were also determined, and found to be in line with what little data are available on this species in the literature. Although Italy recently endorsed the African-Eurasian Waterbird Agreement (AEWA) prohibiting use of lead shot for hunting in wetlands, our results reveal a first case of lead shot poisoning in flamingos wintering in Italian wetlands. This evidence sounds a further warning of the problem of spent lead shot in countries where hunting in wetlands is not strictly regulated.
