Antisera to atrial natriuretic factor reduces urinary sodium excretion and increases plasma renin activity in rats

Although the presence of atrial natriuretic factor in the blood has been demonstrated by radioimmunoassay, its biological activity and physiological significance has not been elucidated. Using specific antiserum against atrial natriuretic factor, we investigated the effect of passive immunization in rats. A significant reduction of urine output and urinary sodium excretion lasted for about 30 min after intravenous administration of antiserum. The effects were more pronounced in rats pretreated with deoxycorticosterone acetate and saline. Plasma renin activity was increased after the administration of antiserum. No significant effects on the urinary sodium excretion was observed following injection of normal rabbit serum. The results of this study provide evidence indicating that endogenous atrial natriuretic factor plays an important role in the regulation of urinary water and sodium excretion and plasma renin activity.

Effect of human growth hormone-releasing hormone on GH secretion in Cushing's syndrome and non-endocrine disease patients treated with glucocorticoids.

The GH response to 100 micrograms human growth hormone-releasing hormone (hGRH) given intravenously was evaluated in eleven patients with Cushings syndrome who had been ill for more than one year and in six patients with non-endocrine diseases who were treated with glucocorticoid for one to twelve weeks. Extremely low to no response of plasma GH to hGRH injection was noted in all seven patients with Cushings disease and in four patients with Cushings syndrome due to an adrenal adenoma or carcinoma. In contrast, all six patients with non-endocrine diseases who were treated with glucocorticoid showed normal GH responses to hGRH. These results suggest that the diminished hGRH-induced GH secretion in patients with Cushings syndrome might be caused by the prolonged period of hypercortisolemia.

Development of a system for passive and active CAVH

当施設においてcontinuous arteriovenous hemofiltration (CAVH) を31症例について施行した. これらの症例を分類し, なおかつ臨床経験をもとにCAVHの問題点について検討を行った.その結果は濾液・補充液バランス装置を用いて積極的に溶質除去を行うactive CAVH (mildなHF) が31症例 (52%), 通常のCAVH (passive CAVH) が11例 (35%) となり, passiveとactive CAVHの併用症例が4例 (13%) であった. 血液ポンプの使用をした症例は全体の71%となった. またこの治療が長時間に及ぶため夜間の管理, フィルター内における血液凝固が血液回路にまでおよびblood accessの閉塞が惹起されることも多い. われわれはこれらの問題を解消するためpassiveおよびactive CAVH用システムを開発した. システムとしては血液駆動部, 濾液・補充液バランス部とblood acess確保部により構成されている. Passiveおよびactive CAVHに使用した濾過器はAmicon社製Diafilter-20, Gambro社製FH-101, 旭メディカル社製PAN-50, 60Pとニプロ社製UF-50である. 濾過器の性能を濾過係数 (LP) とふるい係数 (SC) で表わしたところLP 22.6±8.6ml/hr/m/mmHgでSCは尿素0.98±0.02, クレアチニン0.94±0.06, イヌリン0.65±0.12でアルブミンは0であった. 体外回路の血液充填量は24mlである.治療時間数は1日-47日で平均の治療時間は8.3日であった. 濾過についてはpassive CAVHの平均濾過量は350ml/hrとなり, active CAVHの置換液量は350-1,000ml/hrであった. 抗凝固薬は全体の65%がヘパリンを用い投薬量は8U/hr/kgとした.われわれはactive CAVHに必要なシステムを開発したが, 本システムはpassive CAVHに切り換えが可能であり, CAVHの特長を最大に生かした装置といえる.

Basic and clinical studies have been made by the authors of biopsy of the small intestine. Recently, however, medium-chain triglyceride (MCT)which has a feature of being absorbed even in the absence of bile and pancreatic lipase in the intestinal tract has been shown to be effective for the patients of hypopeptic and hypoabsorptive syndrome. In the present paper are described the results of the morphological examinat ion and correlations to the functions of small intestine (particularly to MCT) revealed by biopsy of the small intestine which was performed before and after the adminis t ra t ion of 2 g / k g g r ams of MCT daily for 5--7 days to patients who had persistently shown such symptoms as dilatation of the abdomen, soft stool and diarrhea for more than 1 month following gastrectomy.
