Field-based fitness assessment in young people: the ALPHA health-related fitness test battery for children and adolescents

The present study summarises the work developed by the ALPHA (Assessing Levels of Physical Activity) study and describes the procedures followed to select the tests included in the ALPHA health-related fitness test battery for children and adolescents. The authors reviewed physical fitness and health in youth findings from cross-sectional studies. The authors also performed three systematic reviews dealing with (1) the predictive validity of health-related fitness, (2) the criterion validity of field-based fitness tests and (3) the reliability of field-based fitness tests in youth. The authors also carried out 11-methodological studies to determine the criterion validity and the reliability of several field-based fitness tests for youth. Finally, the authors performed a study in the school setting to examine the reliability, feasibility and safety of the selected tests. The selected fitness tests were (1) the 20 m shuttle run test to assess cardiorespiratory fitness; (2) the handgrip strength and (3) standing broad jump to assess musculoskeletal fitness, and (4) body mass index, (5) skinfold thickness and (5) waist circumference to assess body composition. When there are time limits, the authors propose the high-priority ALPHA health-related fitness test battery, which comprises all the evidence-based fitness tests except the measurement of the skinfold thickness. The time required to administer this battery to a group of 20 youth by one physical education teacher is less than 2 h. In conclusion, the ALPHA fitness tests battery is valid, reliable, feasible and safe for the assessment of health-related physical fitness in children and adolescents to be used for health monitoring purposes at population level.

Influence of socioeconomic factors on fitness and fatness in Spanish adolescents: The AVENA study

OBJECTIVE This study was to examine the influence of socioeconomic status on fitness and fatness in Spanish adolescents. SUBJECTS AND METHODS A total of 1 795 adolescents (833 boys) aged 12.5-18.5 years from the AVENA study participated in this study. The following physical fitness components were assessed: muscular strength (standing long jump), speed-agility (4 × 10 m shuttle run) and cardiorespiratory fitness (20 m shuttle run). Total and central body fat was assessed using skinfold thickness and waist circumference, respectively. Both parents reported their educational and professional level. RESULTS Girls with higher paternal educational level and boys with higher either maternal or paternal professional level had higher muscular strength levels (all P<0.05). Speed-agility and cardiorespiratory fitness were not associated with parental educational or professional levels in boys. In contrast, girls with higher paternal educational level or higher parental professional level had higher levels in both fitness components (all P<0.05). A higher maternal educational level was associated with lower total and central body fat in boys (all P<0.05), but not in girls. We observed a small effect size (Cohens d∼0.2) in most of these associations. CONCLUSIONS Our results suggest modest associations of high socioeconomic status with better fitness and fatness levels in Spanish adolescents. These associations greatly depend on the socioeconomic status and fitness parameters studied.

Criterion-Related Validity of Sit-and-Reach and Modified Sit-and-Reach Test for Estimating Hamstring Flexibility in Children and Adolescents Aged 6-17 Years

The purpose of the present study was to examine the criterion-related validity of the sit-and-reach test (SRT) and the modified sit-and-reach test (MSRT) for estimating hamstring flexibility in children and adolescents as well as to determine whether the MSRT is more valid than the SRT. A total of 87 (45 boys and 42 girls) children (6-12 years old) and adolescents (13-17 years old) performed the SRT and the MSRT. Hamstring flexibility was measured with goniometry through the passive straight-leg raise test. Regression analysis was performed to study the association of SRT and MSRT with hamstring flexibility (criterion measure). The SRT was associated with hamstring flexibility in both children (beta=1.089, R (2)=0.281, p=0.001) and adolescents (beta=0.690, R (2)=0.333, p=0.004). The MSRT was also associated with hamstring flexibility in both children (beta=1.296, R (2)=0.298, p<0.001) and adolescents (beta=0.588, R (2)=0.243, p=0.027). It is concluded that the criterion-related validity of the SRT and the MSRT for estimating hamstring flexibility is weak. The present data do not support that the MSRT is a more valid method than the SRT in children and adolescents.

Active commuting to school in children and adolescents: An opportunity to increase physical activity and fitness

Aims: The purpose was to describe the patterns of commuting to school in young people and to examine its associations with physical activity (PA) and cardiorespiratory fitness. Methods: The sample comprised 2271 Estonian and Swedish children and adolescents (1218 females) aged 9—10 years and 15—16 years. Data were collected in 1998/99. Mode of commuting to and from school was assessed by questionnaire. Time spent (min/day) in PA and average PA (counts/min) was measured by accelerometry. Cardiorespiratory fitness was assessed by means of a maximal cycle ergometer test. Results: Sixty-one percent of the participants reported active commuting to school (ACS). Estonian youth showed lower levels of ACS than Swedish (odds ratio, 0.64; 95% confidence interval, 0.53—0.76) and girls reported lower levels than boys (0.74; 0.62—0.88). ACS boys showed higher PA levels than non-ACS boys for moderate, vigorous, MVPA, and average PA levels (all p ≤ 0.01). Participants who cycled to school had higher cardiorespiratory fitness than walkers or passive travellers (p < 0.001). Conclusions: Nearly two-thirds of the participants actively commuted to school. ACS may provide an opportunity to increase levels of daily PA, especially in boys, and cardiorespiratory fitness, especially if cycling. Public health strategies should develop and test ACS patterns to get more evidence and promote bike-friendly environments.

Active commuting to school and cognitive performance in adolescents: the AVENA study.

OBJECTIVE To examine the associations between active commuting to school and cognitive performance in adolescents. DESIGN Cross-sectional study. SETTING Five cities (Granada, Madrid, Murcia, Santander, and Zaragoza) in Spain. PARTICIPANTS A total of 1700 adolescents (892 girls) aged 13 to 18.5 years. MAIN EXPOSURES Mode and duration of transportation to school and participation in extracurricular physical activity were self-reported. MAIN OUTCOME MEASURES Cognitive performance (verbal, numeric, and reasoning abilities and an overall score) was measured by the Spanish version of the SRA Test of Educational Ability. RESULTS Active commuting to school was associated with better cognitive performance (all P < .05) in girls but not in boys, independent of potential confounders including participation in extracurricular physical activity. In addition, adolescent girls who spent more than 15 minutes actively commuting to school had better scores in 3 of the 4 cognitive performance variables (all P < .05) than those who spent less time actively commuting to school (≤15 minutes) as well as better scores in all of the cognitive performance variables (all P < .001) than girls inactively commuting. CONCLUSION Active commuting to school and its duration may positively influence cognitive performance in adolescent girls.

Socio-economic factors and active commuting to school in urban Spanish adolescents: the AVENA study

BACKGROUND This study aimed: (i) to describe the patterns of commuting to school in urban Spanish adolescents; and (ii) to examine the associations between active commuting to school (ACS) and socio-economic factors. METHODS From the AVENA Study, 2183 adolescents (1142 females) aged 13-18.5 years were gathered. Mode and time of transportation to school were self-reported by the adolescents. Parental education level (primary, secondary or university degree), parental professional level (managerial, skilled worker or unskilled worker/unemployed) and the type of school (public or private) were self-reported by the parents. The relationships between ACS and socio-economic factors were analysed by binary logistic regression. RESULTS Nearly <65% of the adolescents reported ACS and 83% of them spent <15 min travelling to school. In male adolescents, maternal primary education level showed an odds ratio (OR) of 1.55 (95% confidence interval, 1.12-2.15), with respect to mothers with a university degree. In female adolescents, mothers with a primary education level showed an OR of 0.68 (0.50-0.92), with respect to mothers with a university degree. Low maternal professional level showed an OR of 1.70 (1.29-2.24), with respect to high maternal professional levels. Students attending public schools showed an OR of 3.47 (2.46-4.90), with respect to students from private schools. CONCLUSION Most of the adolescents actively commuted to school, yet the time spent commuting was low. Socio-economic level seems to be inversely related to the ACS in adolescents. Type of school and maternal educational level were the main predictors of ACS.

Sedentary behaviours and socio-economic status in Spanish adolescents: the AVENA study

BACKGROUND This study aimed to describe the influence of socio-economic status (SES) on the prevalence sedentary behaviours among Spanish adolescents. METHODS Cross-sectional data from Spanish adolescents from the Alimentación y Valoración del Estado Nutricional de los Adolescentes (AVENA) Study (2002). A national representative sample of 1776 adolescents aged 13-18.5 years provided information about time spent watching television (TV), playing with computer or videogames and studying. Parental education and occupation were assessed as SES. Participants were categorized by gender, age, parental education and occupation. Logistic regression models were used. RESULTS No gender differences were found for TV viewing. For computer and videogames use (weekdays), more boys played >3 h/day (P < 0.001), whereas a higher percentage of girls reported studying >3 h/day (P < 0.001). Among boys, parental education and occupation were inversely associated with TV viewing, parental occupation directly associated with study and maternal education inversely with computer and videogames use during weekdays (all P < 0.05). For girls, parental occupation was inversely associated with TV viewing. CONCLUSION Spanish adolescents presented different sedentary patterns according to age, gender and SES. Boys reported more time engaged in electronic games, whereas girls reported more time studying. Parental occupation had more influence than parental education on the time spent in sedentary behaviours.

Physical fitness in rural and urban children and adolescents from Spain

OBJECTIVE We aimed to examine the differences in fitness components between rural and urban Spanish children and adolescents. DESIGN/METHOD A sample of 2569 school children (1068) and adolescents (1501) from urban and rural areas from the region of Aragón (Spain). Physical fitness was assessed by 7 tests: 20 m shuttle run, speed shuttle run, sit and reach, standing long jump, handgrip strength, bent arm hang and sit-ups in 30s (s). Differences (in fitness and anthropometry) by place of residence were examined by one-way analysis of covariance. RESULTS Rural young people had higher cardiorespiratory fitness, but lower speed-agility and flexibility than urban young people (all p<0.001; mean differences=1.5 mL/kg per min, 0.4s and 0.9 cm respectively). Regarding muscular fitness, rural young people had a better performance in handgrip strength (mean difference=0.8 kg), bent arm hang (mean difference=2.3s), and a lower performance in sit-ups in 30s (mean difference=0.9s) (all p<0.001), compared to their urban peers. Rural young people had a lower body mass, body mass index (BMI) and sum of skinfolds (all p<0.05; mean differences=1.1 kg, 0.3 kg/m(2) and 4.9 mm respectively) than their urban peers. Effect size was small-medium (Cohens d=0.1-0.3). CONCLUSION Rural Spanish children and adolescents had overall a healthier profile than their urban peers in terms of cardiorespiratory fitness, upper- and lower-limb muscular fitness and adiposity, while they performed worse in speed-agility and flexibility. The place of residence should be taken into account when implementing effective interventions to promote physical activity and health.

A cross-sectional study of demographic, environmental and parental barriers to active school travel among children in the United States

BackgroundPromoting daily routine physical activities, such as active travel to school, may have important health implications. Practitioners and policy makers must understand the variety of factors that influence whether or not a child uses active school travel. Several reviews have identified both inhibitors and promoters of active school travel, but few studies have combined these putative characteristics in one analysis. The purpose of this study is to examine associations between elementary school children’s active school travel and variables hypothesized as correlates (demographics, physical environment, perceived barriers and norms).MethodsThe current project uses the dataset from the National Evaluation of Walk to School (WTS) Project, which includes data from 4th and 5th grade children and their parents from 18 schools across the US. Measures included monthly child report of mode of school travel during the previous week (n = 10,809) and perceived barriers and social norms around active school travel by parents (n = 1,007) and children (n = 1,219). Generalized linear mixed models (GLMM) with log-link functions were used to assess bivariate and multivariate associations between hypothesized correlates and frequency of active school travel, assuming random school effect and controlling for the distance to school.ResultsThe final model showed that the most relevant significant predictors of active school travel were parent’s perceived barriers, specifically child resistance (Estimate = −0.438, p < 0.0001) and safety and weather (Estimate = −0.0245, p < 0.001), as well as the school’s percentage of Hispanic students (Estimate = 0.0059, p < 0.001), after adjusting for distance and including time within school cluster as a random effect.ConclusionsParental concerns may be impacting children’s use of active school travel, and therefore, future interventions to promote active school travel should more actively engage parents and address these concerns. Programs like the Walk to School program, which are organized by the schools and can engage community resources such as public safety officials, could help overcome many of these perceived barriers to active transport.

Hip flexibility is the main determinant of the back-saver sit-and-reach test in adolescents

Abstract Although flexibility field tests are commonly used in research, sport, and school settings, there is no conclusive evidence about what they actually assess. The first aim of this study was to assess the contributions of the main joints involved in the back-saver sit-and-reach test using angular kinematic analysis. The second aim was to measure the inter-method agreement between the back-saver sit-and-reach test and the sit-and-reach test. A total of 138 adolescents (57 females, 81 males) aged 14.5 ± 1.7 years performed the back-saver sit-and-reach test and the sit-and-reach test. Hip, lumbar, and thoracic angles were assessed by angular kinematic analysis while the participants were performing the back-saver sit-and-reach test. Stepwise linear regression models and the Bland-Altman method were used. The hip angle independently explained 42% (P < 0.001) of the variance in the back-saver sit-and-reach test, the lumbar angle explained an additional 30% (P < 0.001) of the variance, and the thoracic angle an additional 4% (P < 0.001). The inter-method mean difference between back-saver sit-and-reach (BSSR) and sit-and-reach (SR) measures (BSSR – SR) was 0.41 cm (P = 0.21). The results suggest that hip flexibility is the main determinant of the back-saver sit-and-reach test score in adolescents, followed by lumbar flexibility. The back-saver sit-and-reach test can therefore be considered an appropriate and valid test for assessing hip and low-back flexibility in this age group. The back-saver sit-and-reach and sit-and-reach tests provide comparable values.
