Multislice spiral computed tomography angiography of mesenteric arteries

The introduction of multislice technology has improved and expanded the clinical applications of computed tomography angiography (CTA), and it is now possible to detect subtle vascular abnormalities. We optimised a high resolution CTA protocol to study the anatomy of mesenteric arteries with a multislice spiral CT scanner. We showed that multislice CTA produces images of high quality, with excellent anatomical visualisation of the main mesenteric trunks, and small collateral vessels. CTA may facilitate the diagnosis of vascular diseases, such as chronic mesenteric ischaemia, as well as acute disorders such as gastrointestinal bleeding.

Utility of T2-weighted short-tau inversion recovery (STIR) sequences in cardiac MRI: an overview of clinical applications in ischaemic and non-ischaemic heart disease

T2-weighted short-tau inversion recovery (T2w-STIR) imaging is the best approach for oedema-weighted cardiac magnetic resonance imaging (MRI), as it suppresses the signal from flowing blood and from fat and enhances sensitivity to tissue fluid. The purpose of this pictorial review is to illustrate the clinical use and application of this technique in various ischaemic and non-ischaemic conditions. In ischaemic heart disease, T2w-STIR represents the technique of choice for detecting oedema in patients with acute myocardial infarction (MI), allowing discrimination of acute and chronic injuries. Myocardial haemorrhage may also be depicted as a region of signal abnormality characterised by a central hypointense core with a peripheral hyperintense rim, presumably reflecting the presence of intracellular methaemoglobin within the necrotic area. In the acute setting, elevated T2 relaxation times in association with regional contractile dysfunction but no signs of delayed enhancement may also signify a reversible ischaemic injury without necrosis. In acute myocarditis, the distribution pattern of T2w hyperintensity may be focal in approximately 30% of patients or diffuse in the remaining 70%, and myocardial oedema may be the only marker of disease. Tissue oedema may also be observed in various other conditions, such as primary cardiomyopathies (CMP), storage disease, pulmonary hypertension and cardiac transplant rejection. T2w-STIR represents an appealing and versatile technique that can be applied in a wide variety of ischaemic and non-ischaemic conditions, allowing detection of segmental or global increase of myocardial free water content, reflecting an acute myocardial injury.RiassuntoIn risonanza magnetica cardiaca, le sequenze short tau inversion recovery (STIR) pesate in T2 rappresentano la tecnica ideale per l’identificazione dell’edema tissutale grazie all’applicazione di un triplo impulso di presaturazione che consente di combinare la soppressione del segnale dei fluidi in movimento e del tessuto adiposo. Scopo di questa pictorial review è di illustrare l’utilità clinica e lo spettro applicazioni di questa sequenza nella patologia cardiaca ischemica e non ischemica. Nella cardiopatia ischemica, l’edema tissutale rappresenta un marker di danno miocardico acuto e definisce l’area di miocardio a rischio che contiene sia tessuto necrotico che segmenti miocardici non contrattili ma vitali. L’emorragia rappresenta invece la conseguenza della terapia riperfusiva con stravaso di eritrociti nell’interstizio e può essere identificata in STIR T2 come un’alterazione caratterizzata da un core centrale ipointenso, che riflette la presenza di metaemoglobina nel contesto dell’area di necrosi. Nelle miocarditi acute, la distribuzione del danno miocardico può essere focale nel 30% dei pazienti o diffuso nel restante 70% e l’edema può costituire l’unico segno di patologia. L’edema tissutale può essere inoltre osservato in numerose altre cardiopatie non ischemiche, come varie cardiomiopatie primitive e secondarie, pericarditi acute, l’ipertensione polmonare ed il rigetto di trapianto cardiaco. In aggiunta alle tradizionali sequenze morfo-funzionali, le sequenze STIR T2 costituiscono un’interessante e versatile tecnica di imaging che può trovare applicazione in una vasta gamma di cardiopatie ischemiche e non, consentendo di identificare un incremento focale o diffuso del contenuto di acqua libera intramiocardica e fornendo molto spesso informazioni di rilevante impatto clinico.

Transrectal ultrasound color Doppler in the evaluation of recurrence of anal canal cancer

OBJECTIVEnaim was to obtain elements for a differential diagnosis between post-radiation fibrosis and residual tumor or local relapse in anal canal cancer through detection of presence/absence of intralesional blood flow. Transrectal ultrasound and color Doppler were compared.nnnMETHODSn43 patients underwent transrectal ultrasound sonography and color Doppler before and after therapy to assess intralesional blood flow and flow pattern (spotty and linear signals). All diagnostic imaging results were compared to histological analysis. Specificity was submitted to statistical analysis using McNemar test.nnnRESULTSnbefore therapy 34 lesions (79%) showed color signal; no signal in 9 (21%), which were excluded from our analysis. Eighteen of the 34 patients considered, presented complete response to therapy, 14 partial response and two no response. After therapy, signal disappeared in 17 patients (94%); one false-positive (6%) presented spotty signals; 16 of 34 patients presented changed color signal. Color Doppler showed higher specificity than grey scale transrectal ultrasound in the differentiation of fibrosis from tumor. Response was confirmed by histological examination, considered gold standard. McNemar test demonstrated the significance of color Doppler (P < 0.0001).nnnCONCLUSIONncolor Doppler considerably increases transrectal ultrasound specificity in differentiating tumor relapse from fibrosis in anal canal cancer.

2D and 3D T2-weighted MR sequences for the assessment of neurovascular bundle changes after nerve-sparing radical retropubic prostatectomy with erectile function correlation

The aim of this study was to assess the capability of a 3D isotropic MRI T2-weighted sequence (3D T2 ISO) in the depiction of changes of neurovascular bundles (NVBs) after bilateral nerve-sparing radical retropubic prostatectomy (RRP). Furthermore, our aim was also to introduce a new MRI classification score of the NVB alteration patterns using the International Index Erectile Function Five-Item (IIEF-5) score as standard of reference. Fifty-three consecutive patients were postoperatively submitted to two MR examinations, including both 2D TSE T2-weighted (2D T2) and 3D T2 ISO sequences. Image findings were scored using a relative five-point classification and correlated with the postoperative IIEF-5 score. Radiologists attributed 13.2% of patients to class 0, 11.3% to class I, 34% to class II, 24.5% to class III, and 16.9% to class IV. With 3D T2 ISO images, the same radiologists determined 43.3% class 0, 32% class I, 11.4% class II, 7.5% class III, and 5.7% class IV. In all cases, the correlation and regression analysis between the 3D T2 ISO and IIEF-5 score resulted in higher coefficients values. The 3D sequence correlated most closely with patients’ grading of erectile function.

64-detector row CT cystography with virtual cystoscopy in the detection of bladder carcinoma: preliminary experience in selected patients

PurposeThis study was conducted to evaluate the role of computed tomography cystography (CTC) and virtual cystoscopy (VC) with 64-slice CT in diagnosing bladder lesions using flexible cystoscopy as the reference standard.Materials and methodsTwenty-eight patients with suspected bladder cancer and ten patients who had undergone transurethral resection of the bladder were studied by CTC and VC in both the supine and prone positions after distending the bladder with air. The patient population was divided into three groups based on lesion size at flexible cystoscopy. Results of the CT study were compared with those of flexible cystoscopy.ResultsFlexible cystoscopy depicted 88 bladder lesions in the 38 patients examined. Sensitivity and specificity values of CTC and VC alone were constantly lower than those of the combined-approach (group 1: 93.10% and 92.31%; group 2: 100% and 100%; group 3: 100% and 100%, respectively). Receiver operating characteristic (ROC) curve analysis showed that the combined approach decreases the lower dimensional threshold for lesion detection (1.4 mm).ConclusionsVC and CTC with 64-slice CT are promising diagnostic techniques for bladder cancers measuring 1–5 mm. Further studies on larger patient populations are required to validate our results.RiassuntoObiettivoValutare il ruolo della cistografia con TC (CTC) e della cistoscopia virtuale (CV) utilizzando una TC multidetettore a 64 strati (TCMD-64) nell’identificazione delle lesioni vescicali e confrontare i risultati ottenuti con la cistoscopia convenzionale.Materiali e metodiVentotto pazienti con sospetta neoplasia vescicale e 10 pazienti in follow-up dopo resezione trans-uretrale della vescica (TURB) sono stati sottoposti ad esame CTC e CV. I pazienti sono stati esaminati in posizione prona e supina previa distensione vescicale con aria e suddivisi in base alle dimensioni delle lesioni. I risultati dell’esame TC sono stati confrontati con quelli della cistoscopia convenzionale.RisultatiLa cistoscopia convenzionale ha visualizzato correttamente un totale di 88 lesioni fra i 38 soggetti esaminati. I valori di sensibilità e specificità delle singole metodiche CTC e CV sono stati costantemente inferiori rispetto a quelli riportati dalla valutazione combinata (gruppo I: 93,10%/92,31%; gruppo II: 100%/100%; gruppo III: 100%/100%). L’analisi ROC del primo gruppo ha individuato un limite dimensionale inferiore della metodica di 1,4 mm.ConclusioniLa CV e la CTC con TCMD-64 sono tecniche proponibili nell’identificazione delle neoplasie vescicali di dimensioni comprese tra 1 e 5 mm. Ulteriori studi necessitano di popolazioni più ampie.

Role of magnetic resonance spectroscopic imaging ([1H]MRSI) and dynamic contrast-enhanced MRI (DCE-MRI) in identifying prostate cancer foci in patients with negative biopsy and high levels of prostate-specific antigen (PSA)

PurposeThe purpose of this study was to evaluate the role of magnetic resonance spectroscopic imaging (MRSI) and dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) in detecting tumour foci in patients with elevated prostate-specific antigen (PSA) and negative transrectal ultrasonography (TRUS)-guided biopsy.Materials and methodsThis prospective randomised trial was conducted on 150 patients who underwent [1H]MRSI and DCE-MRI and targeted biopsies of suspicious areas on MRI associated with random biopsies.ResultsAfter the second biopsy, the diagnosis of prostate adenocarcinoma was made in 64/150 cases. On a perpatient basis, MRSI had 82.8% sensitivity, 91.8% specificity, 88.3% positive predictive value (PPV), 87.8% negative predictive value (NPV) and 85.7% diagnostic accuracy. The sensitivity, specificity, PPV, NPV and accuracy for DCE-MRI was 76.5%, 89.5%, 84.5%, 83.7% and 82%, respectively. The combination of MRSI and DCE-MRI yielded 93.7% sensitivity, 90.7% specificity, 88.2% PPV, 95.1% NPV and 90.9% accuracy in detecting prostate carcinoma.ConclusionsThe combined study with [1H]MRSI and DCE-MRI showed promising results in guiding the biopsy of cancer foci in patients with an initial negative TRUS-guided biopsy.RiassuntoObiettivoScopo del nostro lavoro è stato valutare il ruolo della risonanza magnetica (RM) con spettroscopia (MRSI) e studio dinamico (DCEMR) nell’individuazione di foci tumorali in pazienti con elevati valori di antigene prostatico specifico (PSA) e biopsia prostatica guidata tramite TRUS (trans-rectal-ultrasound)-guidata negativa.Materiali e metodiLo studio è stato di tipo prospettico randomizzato. Tutti sono stati sottoposti ad esame di 1H-MRSI e DCEMR ed a prelievi mirati nelle zone sospette alla RM, associate a biopsie random.RisultatiDopo la seconda biopsia, la diagnosi di adenocarcinoma prostatico è stata effettuata in 64/150 casi. Nella nostra popolazione, su una base patient by patient, l’MRSI ha mostrato i seguenti valori: sensibilità 82,8%; specificità 91,8%; valore predittivo positivo (PPV) 88,3%; valore predittivo negativo (NPV) 87,8%; accuratezza 85,7%. La DCEMR ha mostrato i seguenti valori: sensibilità 76,5%; specificità 89,5%; PPV 84,5%; NPV 83,7%; accuratezza 82%.
