Normatividade e plasticidade: algumas considerações sobre a clínica psicanalítica com pacientes neurológicos

Ultrapassados os limites de uma tecnica fundamentalmente interpretativa, a psicanalise deixa de se restringir ao âmbito do sofrimento neurotico e passa a intervir em outros campos da patologia do mental, quer nos quadros de origem psicogenica, quer nos quadros em que alteracoes de natureza orgânica engendram sofrimentos subjetivos – como acontece com muitos pacientes portadores de lesoes cerebrais. No entanto, tais modalidades de intervencao exigem investigacao teorica e tecnica, ja que as exigencias deste tipo de clinica diferem em pontos consideravelmente importantes das encontradas na clinica psicanalitica classica. O principal objetivo deste artigo e fornecer subsidios – por meio da articulacao entre as formulacoes winnicottianas sobre dependencia, confianca no ambiente, sentimento de continuidade espaco-temporal, e o conceito de normatividade postulado por Canguilhem – para uma abordagem teorica e pratica que oriente a clinica psicanalitica com pacientes portadores de disturbios neurologicos. A realizacao desta tarefa exige, primeiramente, uma investigacao dos conceitos de plasticidade cerebral, epigenese e normatividade. De posse destes conceitos, finalmente, tecermos algumas consideracoes sobre a clinica psicanalitica com pacientes neurologicos. Do traumático ao trauma: a lógica do presente permanente

A partir do atendimento psicanalitico de pacientes portadores de lesoes cerebrais, foi possivel formular a hipotese de que, em muitos casos, o quadro psicopatologico que se instala apos a percepcao e a experiencia das sequelas de adoecimentos neurologicos pode ser localizado em algum ponto entre a neurose traumatica e as patologias narcisico-identitarias. Para fundamentar e ilustrar o desenvolvimento teorico dessa hipotese, apresentaremos um caso clinico no qual a compulsao a repeticao e convocada como mecanismo de defesa: a experiencia subjetiva de adoecimento neurologico e vivida como um golpe traumatico que divide a vida entre um antes e um depois. A consequencia disso e que os recursos para a elaboracao psiquica sao roubados, criando ou intensificando zonas psiquicas nao integradas. Desse modo, e instalada uma temporalidade baseada em um presente permanente, isto e, num tempo que nao fica para tras e tampouco fornece uma abertura para o que esta por vir.

El pré-discursivo en la teoría de Lacan

The aim of this article is to determine the influence of the prediscursive field in Lacanian theory. To do so, we used a schematic division of the teaching of Lacans theories, as elaborated by Miller, in three specific moments. In the first ten seminars, the Structuralist perspective of Freudian texts is the main theme. In the second moment, which started in 1964, a movement can be observed: while the notion of structure lost the central position, the lived experience became more relevant. The end of the second moment was announced in the early 1970s. The seminar Encore marked the beginning of the last moment, and consolidated a special place to the pre-discursive field. The clinical consequence of this period is an emphasis on the body and the experiences before speech acquisition, which provided an alternative for the structural model.
