An Exact Approach for Relating Recovering Surgical Patient Workload to the Master Surgical Schedule

No other department influences the workload of a hospital more than the Department of Surgery and in particular, the activities in the operating room. These activities are governed by the master surgical schedule (MSS), which states which patient types receive surgery on which day. In this paper, we describe an analytical approach to project the workload for downstream departments based on this MSS. Specifically, the ward occupancy distributions, patient admission/discharge distributions and the distributions for ongoing interventions/treatments are computed. Recovering after surgery requires the support of multiple departments, such as nursing, physiotherapy, rehabilitation and long-term care. With our model, managers from these departments can determine their workload by aggregating tasks associated with recovering surgical patients. The model, which supported the development of a new MSS at the Netherlands Cancer Institute–Antoni van Leeuwenhoek Hospital, provides the foundation for a decision support tool to relate downstream hospital departments to the operating room.

Accounting for Inpatient Wards when developing Master Surgical Schedules

BACKGROUND: As the demand for health care services increases, the need to improve patient flow between departments has likewise increased. Understanding how the master surgical schedule (MSS) affects the inpatient wards and exploiting this relationship can lead to a decrease in surgery cancellations, a more balanced workload, and an improvement in resource utilization. We modeled this relationship and used the model to evaluate and select a new MSS for a hospital. METHODS: An operational research model was used in combination with staff input to develop a new MSS. A series of MSSs were proposed by staff, evaluated by the model, and then scrutinized by staff. Through iterative modifications of the MSS proposals (i.e., the assigned operating time of specialties), insight is obtained into the number, type, and timing of ward admissions, and how these affect ward occupancy. RESULTS: After evaluating and discussing a number of proposals, a new MSS was chosen that was acceptable to operating room staff and that balanced the ward occupancy. After implementing the new MSS, a review of the bed-use statistics showed it was achieving a balanced ward occupancy. The model described in this article gave the hospital the ability to quantify the concerns of multiple departments, thereby providing a platform from which a new MSS could be negotiated. CONCLUSION: The model, used in combination with staff input, supported an otherwise subjective discussion with quantitative analysis. The work in this article, and in particular the model, is readily repeatable in other hospitals and relies only on readily available data.

Master surgery scheduling with consideration of multiple downstream units

We consider a master surgery scheduling (MSS) problem in which block operating room (OR) time is assigned to different surgical specialties. While many MSS approaches in the literature consider only the impact of the MSS on operating theater and operating staff, we enlarge the scope to downstream resources, such as the intensive care unit (ICU) and the general wards required by the patients once they leave the OR. We first propose a stochastic analytical approach, which calculates for a given MSS the exact demand distribution for the downstream resources. We then discuss measures to define downstream costs resulting from the MSS and propose exact and heuristic algorithms to minimize these costs.

Efficiency evaluation for pooling resources in health care

Hospitals traditionally segregate resources into centralized functional departments such as diagnostic departments, ambulatory care centers, and nursing wards. In recent years this organizational model has been challenged by the idea that higher quality of care and efficiency in service delivery can be achieved when services are organized around patient groups. Examples include specialized clinics for breast cancer patients and clinical pathways for diabetes patients. Hospitals are struggling with the question of whether to become more centralized to achieve economies of scale or more decentralized to achieve economies of focus. In this paper we examine service and patient group characteristics to study the conditions where a centralized model is more efficient, and conversely, where a decentralized model is more efficient. This relationship is examined analytically with a queuing model to determine the most influential factors and then with simulation to fine-tune the results. The tradeoffs between economies of scale and economies of focus measured by these models are used to derive general management guidelines.

A review on the relation between simulation and improvement in hospitals

BackgroundSimulation applications on operations management in hospitals are frequently published and claim to support decision-making on operations management subjects. However, the reported implementation rates of recommendations are low and the actual impact of the changes recommended by the modeler has hardly been examined. This paper examines: 1) the execution rate of simulation study recommendations, 2) the research methods used to evaluate implementation of recommendations, 3) factors contributing to implementation, and 4) the differences regarding implementation between literature and practice.ResultsAltogether 16 hospitals executed the recommendations (at least partially). Implementation results were hardly reported upon; 1 study described a before-and-after design, 2 a partial before and after design. Factors that help implementation were grouped according to 1) technical quality, of which data availability, validation/verification with historic data/expert opinion, and the development of the conceptual model were mentioned most frequently 2) process quality, with client involvement and 3) outcome quality with, presentation of results. The survey response rate of traceable authors was 61%, 18 authors implemented the results at least partially. Among these responses, evaluation methods were relatively better with 3 time series designs and 2 before-and-after designs.ConclusionsAlthough underreported in literature, implementation of recommendations seems limited; this review provides recommendations on project design, implementation conditions and evaluation methods to increase implementation.MethodsA literature review in PubMed and Business Source Elite on stochastic simulation applications on operations management in individual hospitals published between 1997 and 2008. From those reporting implementation, cross references were added. In total, 89 papers were included. A scoring list was used for data extraction. Two reviewers evaluated each paper separately; in case of discrepancies, they jointly determined the scores. The findings were validated with a survey to the original authors.

Optimizing the strategic patient mix combining queueing theory and dynamic programming

In this paper we address the decision of choosing a patient mix for a hospital that leads to the most beneficial treatment case mix. We illustrate how capacity, case mix and patient mix decisions are interrelated and how understanding this complex relationship is crucial for achieving the maximum benefit from the fee-for-service financing system. Although studies to determine the case mix that is of maximum benefit exist in the literature, the hospital actions necessary to realize this case mix have seen less attention. We model the hospital as an M/G/~ queueing system to evaluate the impact of accepting certain patient types. Using this queueing model to generate the parameters, an optimization problem is formulated. We propose two methods for solving the optimization problem. The first is exact but requires an integer linear programming solver whereas the second is an approximation algorithm relying only on dynamic programming. The model is applied in the department of surgery at a Dutch hospital. The model determines which patient types result in the desired growth in the preferred surgical treatment areas. The case study highlights the impact of striving for a certain case mix without providing a sufficiently balanced supply of resources. In the case study we show how the desired case mix can be better achieved by investing in certain capacity.

Analytical models to determine room requirements in outpatient clinics

Outpatient clinics traditionally organize processes such that the doctor remains in a consultation room while patients visit for consultation, we call this the Patient-to-Doctor policy (PtD-policy). A different approach is the Doctor-to-Patient policy (DtP-policy), whereby the doctor travels between multiple consultation rooms, in which patients prepare for their consultation. In the latter approach, the doctor saves time by consulting fully prepared patients. We use a queueing theoretic and a discrete-event simulation approach to provide generic models that enable performance evaluations of the two policies for different parameter settings. These models can be used by managers of outpatient clinics to compare the two policies and choose a particular policy when redesigning the patient process. We use the models to analytically show that the DtP-policy is superior to the PtD-policy under the condition that the doctor’s travel time between rooms is lower than the patient’s preparation time. In addition, to calculate the required number of consultation rooms in the DtP-policy, we provide an expression for the fraction of consultations that are in immediate succession; or, in other words, the fraction of time the next patient is prepared and ready, immediately after a doctor finishes a consultation. We apply our methods for a range of distributions and parameters and to a case study in a medium-sized general hospital that inspired this research.

Offload zones to mitigate emergency medical services (EMS) offload delay in the emergency department: a process map and hazard analysis

UNLABELLED Introduction Offload delay is a prolonged interval between ambulance arrival in the emergency department (ED) and transfer of patient care, typically occurring when EDs are crowded. The offload zone (OZ), which manages ambulance patients waiting for an ED bed, has been implemented to mitigate the impact of ED crowding on ambulance availability. Little is known about the safety or efficiency. The study objectives were to process map the OZ and conduct a hazard analysis to identify steps that could compromise patient safety or process efficiency. METHODS A Health Care Failure Mode and Effect Analysis was conducted. Failure modes (FM) were identified. For each FM, a probability to occur and severity of impact on patient safety and process efficiency was determined, and a hazard score (probability X severity) was calculated. For any hazard score considered high risk, root causes were identified, and mitigations were sought. RESULTS The OZ consists of six major processes: 1) patient transported by ambulance, 2) arrival to the ED, 3) transfer of patient care, 4) patient assessment in OZ, 5) patient care in OZ, and 6) patient transfer out of OZ; 78 FM were identified, of which 28 (35.9%) were deemed high risk and classified as impact on patient safety (n=7/28, 25.0%), process efficiency (n=10/28, 35.7%), or both (n=11/28, 39.3%). Seventeen mitigations were suggested. CONCLUSION This process map and hazard analysis is a first step in understanding the safety and efficiency of the OZ. The results from this study will inform current policy and practice, and future work to reduce offload delay.
