Problem  The presence of immunosuppressive cells within the endometrium and decidua is crucial for establishing maternal immune tolerance against fetal antigens. We decided to evaluate the subpopulations of Treg cells and B7H4 macrophages in eutopic endometrium typified by Arias Stella reaction during the development of Fallopian tube pregnancy as well as in decidua at the time of spontaneous abortion (SA), and to compare these findings to those observed in the endometrium during the secretory cycle phase of healthy women.

Do We Pay Enough Attention to Culture Conditions in Context of Perinatal Outcome after In Vitro Fertilization? Up-to-Date Literature Review

Adverse perinatal outcomes in singleton IVF pregnancies have been most often explained by parental underlying diseases and so far laboratory conditions during embryo culture are still not explored well. The following review discusses the current state of knowledge on the influence of IVF laboratory procedures on the possible perinatal outcome. The role of improved media for human embryo culture is unquestionable. Addition of certain components to culture media and their effect on embryo survival and implantation rates have been taken into consideration recently and studied on animal model. Impact of media on perinatal outcome in IVF offspring has also been studied. It has been discovered that epigenetic changes and neonatal birth weight are probably associated with the use of specific culture media, as is the relation between placental size and its influence on perinatal outcome. There are still questions in the discussion about duration of embryo culture (cleavage stage versus blastocyst transfer). Some of the IVF methods, such as in vitro maturation of oocytes and freezing/thawing procedures, also require well-powered randomized controlled trials in order to define their exact impact on perinatal outcome. Constant further research is needed to assess the impact of laboratory environment on fetal and postnatal development.

P282 Cervical length measurement in 22–24 weeks of gestation as a prediction factor of preterm delivery in singleton pregnancies

7.1%. 68 Case notes were identified, but 42 notes were obtained and reviewed. 5 notes were excluded as cervix was not fully dilated. Most women were between 20–30 years and were at term. Most women had BMI < 30kg/m and in 11 women BMI was not documented. 13 women were induced and 19 women were augmented. Second stage ranged from 30 minutes to 6 hours. Cervical dilatation, station and position were documented in 94%, 78% and 62% of women respectively. Kiwi Ventouse was the most commonly used instrument and the operators were experienced Middle grades and consultants. The reasons for failing a trial Instrumental were no descent (47%), Cup pop off (21.05%) and fetal bradycardia (21.05%). High fetal head (37%), no descent (16%) and not stated (21%) remain the reasons for not attempting Instrumental Delivery at full dilatation The complications noted in this audit were PPH (2), Angle extension (2), Scar dehiscence (1) and Apgar <2 (2). Conclusions: In the audit period, the caesarean section rate was 32.9%, second stage section was 8.9%, Instrumental Vaginal Delivery rate 7.6% (RCOG 10–15%) and the failed instrumental delivery rate was 10.2% (Literature 5.6%). With the accrual of experience in Instrumental delivery, the incidence of second stage caesarean sections and its inherent complications could be reduced. Recommendations were made to improve the level of documentation and to develop a database for instrumental delivery and second stage sections.

The maternal weight gain in first trimester was related with fetal ultrasonographic discordancy than other trimesters (correlation coefficient r = 0.200, p = 0.026; r = 0.229, p = 0.22), while it showed a higher relationship with total fetal ultrasonographic weight gain in second trimester than other trimesters in both groups (group A; r = 0.387, p = 0.000, group B; r = 0.425, p = 0.127). Conclusions: In twin pregnancies, maternal weight change in first trimester has shown more effect on fetal discordancy than in any other gestational times. Fetal growth, however, has been found to be closely associated with maternal weight gain in second trimester.
