Relationship Between Hypermethylated MGMT Gene and Osteosarcoma Necrosis Rate After Chemotherapy

To investigate the relativity of MGMT(O-6-methylguanine-DNA methyltransferase) gene methylation from patients with protein expression and osteosarcoma necrosis rate after chemotherapy. Fifty-one oteosarcoma tissues were collected, Methylation of MGMT gene promoter was detected by methylation-specific PCR method, and protein expression of MGMT was examined by immunohistochemistry procedure, the relationship between methylated MGMT gene expression and patients response to chemotherapy was analyzed. The positive ratio of methylation MGMT gene promoter in 51 patients was 23.5% (12 in 51). Negative percentage of protein expression of MGMT was 27.5% (14 in 51). It seemed that methylation of MGMT gene in osteosarcoma tissues had no evident relationship with the patient’s age, sexuality, and the size and type of neoplasms, etc. The necrosis rates of methylated MGMT of osteosarcoma (tumor grade from I to IV) were 0 (0/51), 3.9% (2/51), 5.9% (3/51), 13.7% (7/51), respectively. In contrast, the necrosis rates of unmethylated MGMT of osteosarcoma (tumor grade from I to IV) were 45.1% (23/51), 25.5% (13/51), 3.9% (2/51), 2.0% (1/51), respectively. It suggest that methylated and unmethylated MGMT gene of osteosarcoma have significant difference in protein expression. The unmethylated MGMT gene has higher positive protein expression (u = −4.92, P < 0.001). Methylation of MGMT gene has higher tumor necrosis rate in osteosarcoma patients. Methylation in MGMT promoter may be important for judging the effect of chemotherapy in Osteosarcoma patients.

Four case reports on pelvic tumors with deep venous thromboses as main symptoms and literature review.

To probe into the reasons for misdiagnoses of pelvic tumor as deep venous thromboses as well as the diagnostic methods and effective treatments on pelvic tumor. Four case reports on misdiagnosing pelvic tumor as deep venous thromboses and further analysis on the causes of misdiagnosis, diagnosis, and treatment with the literature study. The four cases were misdiagnosed as pelvic tumor, which actually were fibroneuroma, myxo.fluidity liposarcoma, moderately differentiated squamous cell carcinoma, and synovial sarcoma, respectively. The tumor in first case was completely removed, and the tumor in other three cases, which were malignant tumors, were resected when the tumors shrank with clear boundary and less blood supply after applied with 3. cycles of intra.arterial chemotherapy via an implanted pumpies. Pelvic tumor usually show up or is misdiagnosed as deep venous thromboses for its untypical clinical manifestation, so it should be on the alert for pelvic tumor when deep venous thromboses occurs. Tumor resection is preferred for benign tumor, and intra.arterial intervention chemotherapy should be applied first for malignant tumor followed by surgery.

Clinical report of intra-arterial interventional chemotherapy for synovial sarcoma on limbs

OBJECTIVE The efficiency of implantable intra-arterial interventional chemotherapy before the operation for synovial sarcoma on limbs was evaluated. MATERIALS AND METHODS Drug delivery device (chemotherapy pumps) was implanted percutaneously for 36 patients, and the interventional chemotherapy was conducted for 3-4 cycles. The surgical resection was carried out after obvious improvement was observed by clinical and imaging examination. The evaluation standard for the efficiency of chemotherapy was tumor cell necrosis rate obtained from postoperative pathological test. RESULTS The response rate (RR) of pain alleviation was 91% (32/36), and the RR of the changing of lesion imaging was 83% (30/36). Thirty cases received extensive tumor excision, and six cases received amputation because of poor response to tumor chemotherapy and local carcinelcosis. The postoperative pathological test showed 81% moderate and severe chemotherapy response (29/36). The average follow-up time was 39 months. There were three cases presenting with local tumor recurrence, and seven deaths due to pulmonary metastasis. CONCLUSION Intra-arterial interventional chemotherapy before operation for synovial sarcoma can effectively control pain. After chemotherapy, the imaging performance significantly changed, and tumor cells were necrotized extensively. The pathological changes were obvious.

Clinical and imaging analysis of subclinical hemophilia combined with coxarthrosis: case report and literature review

IntroductionThe incidence of subclinical hemophilic arthritis is low, with this case reports and literature review, we hope clinicians could pay more attention to the diagnosis of subclinical hemophilic arthritis and prevent the misdiagnosis and mistreatment.Case presentationWe analyzed the imaging feature, and therapy of a subclinical hemophilia case with hip lesion by reporting its clinical manifestations, imaging features and therapy procedure, and reviewing literatures.ResultsHemophilia is a sex-linked recessive hereditary hemorrhagic disease, and the pathogenesis of subclinic hemophilia is concealed, which often involves joint lesion. The imaging of joint lesion of hemophilia cases is typical, which facilitates the differential diagnosis with other joint diseases. The current treatment is mainly supplementary or replacement therapy.ConclusionHemophilic arthritis cases, especially the mild or subclinical cases without family history and with an older age of onset, are not common. The disease should be further understood, and early diagnosis and treatment is crucial to prevent the progressive development of joint diseases.

Lower Limb Myxoid Liposarcoma with Distant Metastasis: Report of Two Cases and a Literature Review

Introduction: Liposarcoma, mainly involving the deep soft tissue of limbs and torso, especially lower limbs and retroperitoneum, is the second most common malignant tumour derived from primitive mesenchymal topped by malignant fibrous histiocytoma, accounting for 20% of soft tissue Liposarcoma is divided into five histologic subtypes and three biological types based on WHO classification criteria in 2002: 1) well-differentiated liposarcoma (WDLPS) and dedifferentiated liposarcoma (DDLPS); 2) myxoid liposarcoma (MLS) and round cell liposarcoma (RCLS); 3) pleomorphic liposarcoma (PLS). Among them, MLS is biologically characteristic; especially some patients with extrapulmonary metastases come out with poor prognosis. We hereby retrospectively analyse two MLS cases characterized as extrapulmonary metastases treated in our hospital from June 2007 to October 2015and reviewed related literatures as follows. Methods: We have retrospectively analysed two cases of MLS with distant extrapulmonary metastases and reviewed related literatures. Result: MLS is relatively sensitive to radiotherapy and chemotherapy, but the therapeutic outcome does not correlate with radiochemotherapy, the more the round cells and the malignant the myxoid liposarcoma. About 1/3 MLS patients have distant metastasis, with predilection to soft tissue and bone of other parts of body, while the bone scanning is usually negative. Conclusion: As one subtype of liposarcoma, MLS has many specific biological characteristics, like distant metastasis, and currently regular follow-up examination is needed for timely diagnosis and treatment, but individualized treatment may be the future research direction.

Limb Salvage Strategy by Intra-Arterial Chemotherapy for LocalRecurrent Osteosarcoma in Extremities

Abstract Objective: The feasibility of intra-arterial chemotherapy for local recurrent extremity osteosarcoma with prosthesis replacement. Methods: 9 local recurrent limb osteosarcoma patients with prosthesis replacement who received treatment during the period from April 2010 to June 2013 at our hospital were recruited, including 3 patients with lung metastasis. All patients were treated with intra-arterial chemotherapy via implanted pump at affected limb. Results: The local recurrent neoplasms calcified and formed clear boundary with normal tissues after 3 to 5 cycles intra-arterial intervention chemotherapy, which indicates the osteosarcoma was controlled effectively to facilitate the osteosarcoma excision. No local recurrence happened during the follow-up visit varied from 12 to 38 months. The late result was better in 6 patients without lung metastasis than 3 patients with lung metastasis, 2 of who died and 1 currently survive without tumor. Conclusion: Intra-arterial intervention chemotherapy could effectively curb the progress of local recurrent osteosarcoma to facilitate the subsequent limb salvage treatment, and prolong the lifetime of patients with lung metastasis.
