A radiographic in vivo study was undertaken to investigate the root canal systems of the maxillary first and second premolars. This study reveals that the frequency of two-canaled maxillary first premolars compared favorably with previous studies. However, multiple canal systems were found with greater frequency in maxillary second premolars than had previously been reported.

This article reports on the incidence of success following single-visit endodontic therapy. Non-surgical endodontic therapy was performed on 210 teeth in 167 patients who were rapidly deployed during the initial stages of Operation Desert Shield. Upon their return, 97 patients were recalled for follow-up evaluation. This study evaluated 102 teeth. The recall rate was 58% with a success rate of 89%.

A clinical in vivo study was undertaken dealing with the root canal systems of mandibular central and lateral incisors and canines and their comparison to endodontically treated teeth. The results are presented individually and then summarized as anterior mandibular teeth collectively. The study revealed that the frequency of two canals in mandibular central and lateral incisors compared favorably with current in vitro studies. A discrepancy was noted with anterior canines; they contained in vivo a lower percentage of two canals than has been previously reported in current in vitro studies. This study suggested that access preparation be reevaluated to facilitate the search, location, and obturation of two canals in mandibular anterior teeth, which may frequently escape detection.

Adjuncts to posterior endodontic surgery include fiberoptics, high-torque handpieces, and optical magnification. These current technologies allow the clinician to expand and amplify his surgical techniques. The patient can now have the benefit of treatment options not available previously.

A case report of a maxillary first molar is presented in which a drainage tube from a decompression procedure was removed 2 days following placement. The oral surgeon who removed the tube scheduled the patient for surgical enucleation of the large periapical lesion. The surgical appointment was subsequently cancelled and routine endodontic follow-up showed continued osseous repair at 3, 6, and 12 months. The decompression of a large periapical lesion is a conservative alternative to surgical enucleation.

A case report of generalized argyria found on routine examination and confirmed by history has been reviewed. The condition has been present for the last 16 years and has gone unchanged. A review of argyria is presented, and the importance of other systemic disease entities in patients with skin discoloration is mentioned.

Two case reports are presented to illustrate the effectiveness of cold in localizing the offending tooth in difficult diagnostic situations where painful pulpitis is present. A review of the literature reveals that there are several proposed theories concerning the mechanisms by which cold relieves the painful symptoms present in some cases of irreversible pulpitis. The hydrodynamic theory seems to provide the most feasible explanation of the events that occur.

An in vitro teaching aid is described as an adjunct in the planning of palatal root surgery on maxillary molars. Study casts are utilized in the preparation of a surgical stent, flap design and for studying the palatal vault in cross section, estimating the proximity of the palatal root to cortical bone.

Often management of unusual cases will require some deviation from standard treatment techniques. The purpose of this article is to describe a case involving extensive internal root resorption. A technique for obturation is described which is a departure from conventional methods presently in use.

A case of cuspid gigantism is reported. A technique used to diagnose and complete the root canal therapy is discussed. Modifications of conventional techniques and materials are shown to be effective in managing this case. Routine use of the bitewing film on long cuspids is not recommended. It is suggested only in extreme cases of unusual anatomic features that lend themselves specifically to this technique. It should be kept in mind that such circumstances are uncommon.
