We present the development of the user interfaces of a multi-device digital coaching service that provides tailored feedback to users concerning their physical activity level and medication intake. We present the outcomes of a survey study of user preferences regarding the situation, device and timing of feedback they receive from their personal attentive digital coach. There are clear preferences among the subjects for different types of messages on different devices. Results were implemented in a first prototype. We present the results of a user evaluations with a real version of the digital health coach and we compare them with the results of the survey study.

Self monitoring, personal goal-setting and coaching, education and social support are strategies to help patients with chronic conditions in their daily care. Various tools have been developed, e.g., mobile digital coaching systems connected with wearable sensors, serious games and patient web portals to personal health records, that aim to support patients with chronic conditions and their caregivers in realizing the ideal of self-management. We describe a platform that integrates these tools to support young patients in diabetes self-management through educational game playing, monitoring and motivational feedback. We describe the design of the platform referring to principles from healthcare, persuasive system design and serious game design. The virtual coach is a game guide that can also provide personalized feedback about the user’s daily care related activities which have value for making progress in the game world. User evaluations with patients under pediatric supervision revealed that the use of mobile technology in combination with web-based elements is feasible but some assumptions made about how users would connect to the platform were not satisfied in reality, resulting in less than optimal user experiences. We discuss challenges with suggestions for further development of integrated pervasive coaching and gamification platforms in medical practice.

Since the introduction of the idea of the software interface agent the question recurs whether these agents should be personified and graphically visualized in the interface. In this chapter we look at the use of virtual humans in the interface of healthy lifestyle coaching systems. Based on theory of persuasive communication we analyse the impact that the use of graphical interface agents may have on user experience and on the efficacy of this type of persuasive systems. We argue that research on the impact of a virtual human interface on the efficacy of these systems requires longitudinal field studies in addition to the controlled short-term user evaluations in the field of human computer interaction (HCI). We introduce Kristina, a mobile personal coaching system that monitors its user’s physical activity and that presents feedback messages to the user. We present results of field trials (N = 60, 7 weeks) in which we compare two interface conditions on a smartphone. In one condition feedback messages are presented by a virtual animated human, in the other condition they are displayed on the screen in text. Results of the field trials show that user motivation, use context and the type of device on which the feedback message is received influence the perception of the presentation format of feedback messages and the effect on compliance to the coaching regime.
