Apexification of immature teeth with calcium hydroxide or mineral trioxide aggregate: systematic review and meta-analysis

OBJECTIVE The aim of this study was to conduct a quantitative systematic review, including published data, comparing the efficacy of mineral trioxide aggregate and calcium hydroxide as material used for the endodontic management of immature teeth. STUDY DESIGN Relevant studies published through November 2009 were identified through literature searches using Pubmed (Medline) and Scopus databases. Controlled trials in which calcium hydroxide versus mineral trioxide aggregate were used for the apexification of immature permanent teeth were selected for this study. The evaluation included clinical outcome and apical barrier formation. The principal measure of treatment effect was risk difference. The overall effect was tested by using Z score. Heterogeneity was tested by using the χ(2) statistic and I square (I(2)). A fixed-effect model was used when the studies in the subgroup were sufficiently similar. A random-effects model was used in the summary analysis when there was heterogeneity between the subgroups. RESULTS Based on reduction of relative risk with 95% confidence intervals we found that the rate of clinical success (P = .29) and apical barrier formation (P = .76) of the 2 interventions had no perceivable discrepancy. Regarding success and apical barrier formation, either calcium hydroxide or mineral trioxide aggregate may be used for the apexification of immature teeth.

The impact of clothing style on bone mineral density among post menopausal women in Morocco: a case-control study

BackgroundThe clothing style is an important factor that influences vitamin D production and thus bone mineral density. We performed a case-control study in order to evaluate the effect of veil wearing (concealing clothing) on bone mineral density in Moroccan post menopausal women.MethodsThe cases were osteoporotic women whose disease was assessed by bone mineral density measurement. Each patient was matched with a non osteoporotic woman for age, and body mass index. All our patients were without secondary causes or medications that might affect bone density. The veil was defined as a concealing clothing which covered most of the body including the arms, the legs and the head. This definition is this of the usual Moroccan traditional clothing style.Results178 post menopausal osteoporotic patients and 178 controls were studied. The mean age of the cases and the controls was 63.2 years (SD 7) and the mean body mass index was 32.1 (SD 8). The results of crude Odds Ratios analyses indicated that wearing a veil was associated with a high risk of osteoporosis: OR 2.29 (95% CI, 1.38–3.82). Multiparity or a history of familial peripheral osteoporotic fractures had also a significant effect on increasing the osteoporosis risk (ORs: 1.87 (95% CI, 1.05–3.49) and 2.01 (95% CI, 1.20–3.38)). After a multiple regression analysis, wearing the veil and a history of familial osteoporotic fractures remained the both independent factors that increased the osteoporosis risk (ORs: 2.20 (95% CI, 1.22–3.9) and 2.19 (95% CI, 1.12–4.29) respectively).Conclusionour study suggested that in Moroccan post menopausal women, wearing a traditional concealing clothing covering arms, legs and head increased the risk of osteoporosis. Further studies are required to evaluate the clinical impact of the above findings and to clarify the status of vitamin D among veiled women in Morocco.

Neurological manifestations of Behçet's disease: evaluation of 40 patients treated by cyclophosphamide.

INTRODUCTION Neurological manifestations in Behçets disease represent between 4 to 49% of systemic manifestations and remain, in the long term, the leading cause of morbidity and mortality. METHODS Retrospective series of 40 severe Neurobehçet cases fulfilling the International Study Group criteria for Behçets disease were consecutively recruited over a period from June 2004 to December 2010. All patients had clinical and ophthalmologic examinations; they underwent laboratory and imaging investigations. They received corticosteroids and cyclophosphamide as initial bolus of 600 mg/m(2) of BSA in the 1st, 2nd, 4th, 6th and 8th day followed by a bolus of 600 mg/m(2) BSA every 2 months for 2 years. Antithrombotic therapy was given to patients with cerebral deep venous thrombosis. Patient follow-up and tolerance to treatment were analyzed. RESULTS The average age at diagnosis was 34±13 years, with a sex-ratio of 1.78. The clinical presentation was dominated by the meningoencephalitis in 48.8% of cases, cerebral deep venous thrombosis in 43.6% of cases and myelopathy in 7.7% of cases. The 40 patients receiving cyclophosphamide bolus, despite two aggravated cases, evolved positively with clinical improvement and good tolerance. CONCLUSION The demographic and clinical aspects of our series are similar to those reported in the literature. In contrast to previously reported cases of a poor prognosis in severe neurobehçets disease, our study suggests that immediate and aggressive treatment by cyclophosphamide may ameliorate the prognosis. However, a multicenter study is needed to confirm the possible efficacy of cyclophosphamide and further assess the long-term tolerance.

Prevalence of apical periodontitis and factors associated with the periradicular status

Abstract Aim. The present study aimed at assessing the prevalence of apical periodontitis and identifying the factors associated with apical periodontitis, particularly the influence of the quality of root canal fillings and coronal restorations on the periradicular status. Methodology. The study population consisted of adult patients, men and women, seeking routine dental care at the Endodontic Clinic in the school teaching hospital between September 2006 and July 2008. For each patient complete oral examination and periapical radiographs were made for pathological teeth. All teeth were assessed individually. AP was diagnosed according to defined criteria, other variables were root fillings, coronal fillings, trauma and caries. The odds ratio and 95% confidence interval were used to calculate the risk of apical periodontitis related to tooth-specific risk indicators on the presence of apical periodontitis. A multiple logistic regression model to control for confounders and to determine their independent association with apical periodontitis was used also. Results. AP was detected on teeth (63.79%). The prevalence of apical periodontitis in root canal-treated teeth was 39.5%. Of the dental variables caries, trauma, inadequate root fillings and inadequate restoration were significantly associated with AP. Conclusion. The prevalence of AP in this study group was higher than in other populations. The probability of AP increased significantly after root canal treatment and coronal filling and was closely associated with the quality of the filling.

Erosive Arthropathy in systemic sclerosis

BackgroundTo determine radiological features of arthropathy in systemic sclerosis patients with polyarthritis.ResultsForty one women and 5 men were included in this study. The mean age was 41 + 14,2 years, the mean duration of disease was 10,5+ 6,5 years. Thirty seven patients (80%) had radiological abnormalities including joint space narrowing (37%) and erosions (43%). At presentation, the prevalence of radiological foot abnormalities was lower than that of hands (26% vs 79%, p < 0,001). There was no significant difference between patients with (n = 24) and without erosive arthropathy (Joint space narrowing and/or erosion) (n = 22) in terms of cutaneous subtype, organ involvement, calcinosis presence of rheumatoid factor, ANA, Anti-topoisomerase antibodies.ConclusionThis study showed an high frequency of erosive arthropathy in our morroccan SSc patients with clinical synovitis.

Multiple sclerosis: Clinical characteristics and disability progression in Moroccan children

BACKGROUND 5% of multiple sclerosis have their onset in childhood. OBJECTIVES We aim to show the clinical features and follow-up of Moroccan pediatric MS patients. METHODS 25 pediatric MS records were analyzed between 2005 and 2013 at the Neurology B department of Rabat Ibn sina University Hospital and Medical School Group of Neurogenetic Disorder of Rabat Mohammed V University Souissi. RESULTS The median duration of follow up was 4 years [1.56]. The median age at disease onset was 14 years. Clinical symptoms were described. At the last evaluation, the median EDSS score was 3.0 [0-7.5]. The median time between disease onset and diagnosis was 2 years [1-9] and the median time from MS onset to second neurological episode was 1 year [0.9-3] with an average of 2 relapses in the first 2 years. The median time to reach EDSS 3.0 was 4.5 years [2-17] and the median time to go from EDSS 3.0 to 6.0 was 4 years [2.5-6]. The majority of cases at the last follow-up were; relapsing-remitting (17 patients: 68%), although 8 patients (32%) developed secondary progressive MS after median disease duration of 5 years [4-19]. None of the patients had a primary progressive MS. CONCLUSION Time EDSS 3.0 and to secondary progression was shorter in our cohort than previously reported in other series.

Knowledge, attitudes and self-reported practices toward children oral health among mother’s attending maternal and child’s units, Salé, Morocco

BackgroundThe occurrence of severe dental caries is particularly prevalent and harmful in children. A better understanding of parental factors that may be indicators of children’s risk of developing dental caries is important for the development of preventive measures.This study was conducted to assess knowledge, attitudes, and practices (KAP) of mothers in Salé, Morocco regarding oral health and their predictors.MethodsA cross-sectional KAP study was conducted of Mother and Child units in Salé, Morocco. Mothers attending the selected units from November 2014 to 29 January 2015 were recruited. Data were collected using a semi-structured questionnaire, administered by face-to-face interviews, to record socio-demographic factors and KAPs. The main outcome measures included knowledge about oral health diseases and preventive measures, and attitudes and practices related to oral health prevention measures and dental care. KAPs scores were then recoded based on responses and scores were determined for each KAP domain. Linear regression analysis was conducted to assess predictors of KAP scores.ResultsAmong 502 mothers included, 140 (27.8%) were illiterate and 285 (60.9%) were aware that fluoride has a beneficial effect in caries prevention. Mothers’ own practices about dental care were statistically related to their children’s use of dental care services (p < 0.001). Multiple linear regression analysis revealed that the knowledge score was associated with mother’s age (β = 0.05; 95% CI; p < 0.001), education level, and median income (β = 0.38; p = 0.04). Significant predictors of oral health-related practices were mother’s education level and children’s health status.ConclusionsLimited KAP scores were observed among the studied population. A great emphasis on oral health education and some risk factor modifications are recommended.
