Clima de segurança do paciente: percepção dos profissionais de enfermagem

OBJETIVO: Evaluar la percepcion del clima de seguridad de los profesionales de enfermeria que actuan en las clinicas medicas y quirurgicas de un Hospital de Ensenanza. METODOS: Estudio transversal, descriptivo y con abordaje cuantitativo. Fue utilizado el Safety Attitudes Questionnaire (SAQ) - Short Form 2006, traducido para la lengua portuguesa. RESULTADOS: La percepcion del clima de seguridad de los profesionales vario conforme el genero, la clinica, la categoria profesional y el tiempo de actuacion. La satisfaccion en el trabajo fue demostrada por todos los profesionales, con scores arriba de 75, en cuanto que el dominio Percepcion de la Gerencia presento valores mas bajos. CONCLUSION: La satisfaccion del profesional, el dialogo y el soporte al equipo por parte de la administracion son esenciales para la garantia de la seguridad del paciente. Conocer la percepcion de los profesionales de enfermeria sobre el clima de seguridad contribuye para la mejoria del cuidado en salud y para la reduccion de los riesgos del paciente.OBJECTIVE: To evaluate the perception of the safety climate of nursing professionals working in the medical and surgical clinics of a teaching hospital. METHODS: A cross-sectional, descriptive study using a quantitative approach. We used the Safety Attitudes Questionnaire (SAQ) - Short Form 2006, translated into the Portuguese language. RESULTS: The perception of the safety climate of the professionals varied according to gender, clinic, professional category and time of work. Job satisfaction was demonstrated by all professionals, with scores above 75, while the domain of Perception of Management presented much lower values. CONCLUSION: Professional satisfaction, dialogue and support for the team on the part of the administration are essential to ensuring patient safety. To know the perception of nursing professionals about the climate of safety contributes to the improvement of health care and to the reduction of risks to the patient.

Prevalência de interações medicamentosas em unidades de terapia intensiva no Brasil

OBJECTIVE: To determine the prevalence of drug interactions in intensive care units and to analyze the clinical significance of interactions identified. METHODS: A multicenter, retrospective and cross sectional study conducted with 1124 patients in the seven intensive care units of teaching hospitals in Brazil. Information on drugs administered at 24 hours and 120 hours of hospitalization was obtained from the prescriptions. RESULTS: Within 24 hours, 70.6% of patients had at least one drug interaction; the number at 24h was 2299, at 120 h it was 2619. Midazolam, fentanyl, phenytoin and omeprazole were the drugs with higher frequency of drug interactions. CONCLUSION: In this sample, moderate and severe drug interactions were more prevalent. In light of these findings, all actions of health professionals who provide care to these patients must be integrated in order to identify and prevent possible drug events.

Tecnologias educativas para promoção do (auto) cuidado de mulheres no pós-parto

OBJECTIVE to evaluate national and international literature regarding the use of educational technologies to encourage self care in postpartum women. METHOD an integrative review of the literature. The articles were collected from the CINAHL, SCOPUS, PubMed, SciELO, LILACS and Cochrane databases; the time period for the articles referred to January/2004 to July/2014; the languages used in the articles were Portuguese, English, Spanish and French; the articles were selected from the following descriptors: postpartum care period, educational technology, nursing and self care. Twenty-seven articles were selected for analysis Results: based on the information found, the scales, counseling and home visits were among the most recommended educational technologies. CONCLUSION the technologies promote communication, but are sometimes dependent on computer and internet access, which hinder their use by low-income women.

Prevalência e significância clínica de interações fármaco-nutrição enteral em Unidades de Terapia Intensiva

Estudo multicentrico, transversal e retrospectivo, que objetivou determinar a prevalencia de interacoes farmaco-nutricao enteral (NE) em Unidades de Terapia Intensiva (UTI) de sete hospitais de ensino do Brasil, e analisar a significância clinica das mesmas. Foram coletadas informacoes sobre medicamentos e NE administrados com 24 horas e 120 horas de internacao. Para identificacao das interacoes farmaco-NE empregou-se o software Drug Reax®. Foram investigados 1.124 prontuarios. Destes, 320 pacientes, com 24 horas de internacao, estavam em uso de NE, sendo que 20 (6,3%) apresentaram interacao farmaco-NE. Dos 504 pacientes, com 120 horas de internacao, 39 (7,7%) apresentaram interacao farmaco-NE. As potenciais interacoes farmaco-NE mais frequentes foram: fenitoina-NE, levotiroxina-NE e varfarina-NE. As interacoes farmaco-NE podem interferir na qualidade e no custo da assistencia prestada em UTI e, desta forma, e essencial que a equipe de saude tenha conhecimento sobre as mesmas.This is a multicenter, cross-sectional retrospective study, which aimed to determine the prevalence of interactions drug-enteral nutrition (EN) in Intensive Care Units (ICU) of seven teaching hospitals in Brazil, and to analyze the clinical significance of them. Information on medications and EN administered with 24 hours and 120 hours of hospitalization were collected. For identification of drug-EN interaction was employed software Drug Reax®. It was investigated 1.124 records. Of these, 320 patients, with 24 hours of hospitalization, were on EN, and 20 (6.3%) had drug-EN interaction. Of the 504 patients with 120 hours of hospitalization, 39 (7.7%) had drug-EN interaction. The most frequent drug-EN interactions of clinical significance were phenytoin-EN, warfarin-EN and levothyroxine-EN. Drug-EN interactions may interfere with the quality and cost effectiveness of care in ICU, so it is essential that the health team has knowledge about them.

Safety culture: perception of health professionals in a mental hospital

OBJECTIVE To evaluate the culture of patient safety in a mental health service. METHOD Cross-sectional study conducted with health professionals in a mental hospital, by applying the Safety Attitudes Questionnaire (SAQ). Descriptive and inferential analyses were performed. RESULTS One-hundred and three professionals participated in the study, with female predominance (64.1%) and time of performance equal to or greater than 21 years (32.4%). The nursing professionals of technical level and with statutory work regime were the most participative, 54.4% and 52% respectively. The total score was 69 points. The domain that reached the highest score was Job satisfaction (80 points) and the lowest was Working conditions (57 points). The statutory professionals and those with longer professional experience obtained better scores in the perception of safety culture. CONCLUSION The result of safety culture was below the recommended, indicating the need for strengthening this construct in mental health hospitals.

Evaluación de la cultura de seguridad en hospitales públicos en Brasil

Objetivo: evaluar la cultura de seguridad en tres hospitales publicos. Metodo: estudio transversal realizado en tres hospitales publicos brasilenos, desarrollado con profesionales de la salud aplicando el Safety Attitudes Questionnaire (SAQ). Fueron considerados positivos puntajes mayores o iguales a 75. Resultados: participaron del estudio 573 profesionales, incluyendo tecnicos y auxiliares de enfermeria 292 (51%), enfermeros 105 (18,3%), medicos 59 (10,3%), y otros profesionales 117 (20,4%). El promedio del SAQ vario entre 65 a 69 en los tres hospitales. Sin embargo, en los dominios, Satisfaccion en el Trabajo presento mayor puntaje y lo opuesto fue observado en el dominio Percepcion de la Administracion. Los profesionales tercerizados presentaron mejor percepcion de la cultura de seguridad que los profesionales estatutarios. Los profesionales de nivel superior presentaron mejor percepcion de los factores estresores que los profesionales de nivel medio. Conclusion: el nivel de cultura de seguridad encontrado se encuentra debajo de lo ideal. Las acciones administrativas son consideradas como el principal contribuyente para la fragilidad de la cultura, entretanto los profesionales se demostraron satisfechos con el trabajo.
