Accessory muscles: Anatomy, symptoms, and radiologic evaluation

A wide array of supernumerary and accessory musculature has been described in the anatomic, surgical, and radiology literature. In the vast majority of cases, accessory muscles are asymptomatic and represent incidental findings at surgery or imaging. In some cases, however, accessory muscles may produce clinical symptoms. These symptoms may be related to a palpable swelling or may be the result of mass effect on neurovascular structures, typically in fibro-osseous tunnels. In cases in which an obvious cause for such symptoms is not evident, recognition and careful evaluation of accessory muscles may aid in diagnosis and treatment.

Acute Traumatic Posterior Shoulder Dislocation: MR Findings

PURPOSE To retrospectively evaluate the appearance of lesions of osseous and soft-tissue structures of the glenohumeral joint on magnetic resonance (MR) images after first-time traumatic posterior shoulder dislocation. MATERIALS AND METHODS The study was institutional review board approved and HIPAA compliant, as appropriate, for the four institutions at which the involved patients were treated. Informed patient consent was obtained, were applicable. Thirty-six male patients (age range, 15-80 years; mean age, 40.2 years) with clinically documented first-time traumatic posterior shoulder dislocation were examined with MR arthrography (18 patients) or conventional shoulder MR imaging (18 patients). Causes of posterior shoulder dislocation were electric shock in one patient, seizure in one patient, and trauma in 34 patients. Hill-Sachs lesions, rotator cuff tears, biceps tendon abnormalities, posterior labrocapsular complex lesions, humeral head translation, and osseous glenoid version angle were evaluated. Spearman rank correlation and Student t test analyses were performed. RESULTS In 31 (86%) of the 36 patients, a reverse Hill-Sachs lesion was found. Eleven (31%) patients had a reverse osseous Bankart lesion. Twelve full-thickness rotator cuff tears were seen in seven (19%) patients: four supraspinatus tendon, three infraspinatus tendon, and five subscapularis tendon tears. Six (17%) patients had biceps tendon abnormalities. Posterior labrocapsular complex tears were identified in 21 (58%) patients: 10 (48%) with posterior labral sleeve avulsions and 11 (52%) with reverse Bankart lesions. Twenty-seven (75%) patients had a retroverted scaphoglenoid angle (mean, 4.5 degrees ). The mean humeral translation distance relative to the osseous glenoid fossa was -4.8 mm; in 33 (92%) patients, this distance was translated posteriorly. CONCLUSION The MR appearance of traumatic posterior shoulder dislocation was characterized by reverse Hill-Sachs lesions in 86% of patients and posterocaudal labrocapsular lesions in nearly 60% of patients. Full-thickness rotator cuff tears were seen in approximately 20% of patients.

Lipofibromatous Hamartoma of the Upper Extremity: A Review of the Radiologic Findings for 15 Patients

OBJECTIVE The purpose of this study was to analyze the radiologic characteristics of lipofibromatous hamartomas affecting upper limb peripheral nerves. CONCLUSION Although there are pathognomonic features that characterize lipofibromatous hamartoma on MRI, the range of appearances is broad. Sonography appears to show equally characteristic features and may be a useful tool for assessing this condition.

Investigation of the static and dynamic musculotendinous architecture of supraspinatus

To date, the architecture of supraspinatus (SP) and its relation to joint position has not been investigated. The purpose of this study was to quantify the dynamic architecture of the distinct regions of SP using ultrasound (US). Seventeen subjects (8 M/9 F), mean age 36.4 ± 12.7 years, without tendon pathology were recruited. The SP was scanned in relaxed and contracted states. For the contracted state, SP was scanned with the shoulder actively abducted to 60° and the glenohumeral joint in neutral rotation; 80° external rotation; 80° internal rotation. Fiber bundle length (FBL) and pennation angle (PA) of distinct regions, and muscle thickness were computed. Measurements of the posterior region were limited because of acromion shadowing. Parameters between regions and changes between relaxed and contracted states were analyzed using paired t‐tests and repeated measures ANOVA (P < 0.05). On contraction in the anterior region, mean percentage of FBL shortening ranged between 9% and 21%. However, in the posterior region, shortening of approximately 2% only occurred in two of the three positions; lengthening of approximately 2.5% occurred in internal rotation. For the anterior region, the mean PA increased the least in the externally rotated position, and the mean PA of the middle part was smaller than the deep part for all states. Findings suggest changes in the architecture are not uniform throughout the muscle and joint position may play an important role in force production. The US protocol may serve as an outcome measure of adaptive changes of muscle function following surgery, training, and rehabilitation. Clin. Anat. 23:48–55, 2010.

Musculotendinous Architecture of Pathological Supraspinatus: A Pilot In Vivo Ultrasonography Study

Architectural changes associated with tendon tears of the supraspinatus muscle (SP) have not been thoroughly investigated in vivo with the muscle in relaxed and contracted states. The purpose of this study was to quantify the geometric properties within the distinct regions of SP in subjects with full‐thickness tendon tears using an ultrasound protocol previously developed in our laboratory, and to compare findings with age/gender matched normal controls. Twelve SP from eight participants (6 male/2 female), mean age 57 ± 6.0 years, were investigated. Muscle geometric properties of the anterior region (middle and deep parts) and posterior region (deep part) were measured using image analysis software. Along with whole muscle thickness, fiber bundle length (FBL) and pennation angle (PA) were computed for architecturally distinct regions and/or parts. Pathologic SP was categorized according to the extent of the tear in the tendon (with or without retraction). In the anterior region, mean FBL of the pathologic SP was similar with normal controls; however, mean PA was significantly smaller in pathologic SP with retraction compared with normal controls, in the contracted state (P < 0.05). Mean FBL in the posterior region in both relaxed and contracted states was significantly shorter in the pathologic SP with retraction compared with normal controls (P < 0.05). Findings suggest FBL changes associated with tendon pathology vary between the distinct regions, and PA changes are related to whether there is retraction of the tendon. The ultrasound protocol may provide important information on architectural changes that may assist in decision making and surgical planning. Clin. Anat., 2013.

Diagnostic accuracy of an iPhone DICOM viewer for the interpretation of magnetic resonance imaging of the knee.

Objective:To evaluate the diagnostic performance of viewing magnetic resonance (MR) images on a handheld mobile device compared with a conventional radiology workstation for the diagnosis of intra-articular knee pathology. Design:Prospective comparison study. Setting:Tertiary care center. Patients:Fifty consecutive subjects who had MR imaging of the knee followed by knee arthroscopy were prospectively evaluated. Interventions:Two musculoskeletal radiologists independently reviewed each MR study using 2 different viewers: the OsiriX DICOM viewer software on an Apple iPhone 3GS device and eFilm Workstation software on a conventional picture archiving and communications system workstation. Main Outcome Measures:Sensitivity and specificity of the iPhone and workstation interpretations was performed using knee arthroscopy as the reference standard. Intraobserver concordance and agreement between the iPhone and workstation interpretations were determined. Results:There was no statistically significant difference between the 2 devices for each paired comparison of diagnostic performance. For the iPhone interpretations, sensitivity ranged from 77% (13 of 17) for the lateral meniscus to 100% (17 of 17) for the anterior cruciate ligament. Specificity ranged from 74% (14 of 19) for cartilage to 100% (50 of 50) for the posterior cruciate ligament. There was a very high level of interobserver and intraobserver agreement between devices and readers. The iPhone reads took longer than the corresponding workstation reads, with a significant mean difference between the iPhone and workstation reads of 3.98 minutes (P < 0.001). Conclusions:The diagnostic performance of interpreting MR images on a handheld mobile device for the assessment of intra-articular knee pathology is similar to that of a conventional radiology workstation, however, requires a longer viewing time. Clinical Relevance:Timely and accurate interpretation of complex medical images using mobile device solutions could result in new workflow efficiencies and ultimately improve patient care.

Build Better Bones With Exercise: Protocol for a Feasibility Study of a Multicenter Randomized Controlled Trial of 12 Months of Home Exercise in Women With a Vertebral Fracture

Background Our goal is to conduct a multicenter randomized controlled trial (RCT) to investigate whether exercise can reduce incident fractures compared with no intervention among women aged ≥65 years with a vertebral fracture. Objectives This pilot study will determine the feasibility of recruitment, retention, and adherence for the proposed trial. Design The proposed RCT will be a pilot feasibility study with 1:1 randomization to exercise or attentional control groups. Setting Five Canadian sites (1 community hospital partnered with an academic center and 4 academic hospitals or centers affiliated with an academic center) and 2 Australian centers (1 academic hospital and 1 center for community primary care, geriatric, and rehabilitation services). Participants One hundred sixty women aged ≥65 years with vertebral fracture at 5 Canadian and 2 Australian centers will be recruited. Intervention The Build Better Bones With Exercise (B3E) intervention includes exercise and behavioral counseling, delivered by a physical therapist in 6 home visits over 8 months, and monthly calls; participants are to exercise ≥3 times weekly. Controls will receive equal attention. Measurements Primary outcomes will include recruitment, retention, and adherence. Adherence to exercise will be assessed via calendar diary. Secondary outcomes will include physical function (lower extremity strength, mobility, and balance), posture, and falls. Additional secondary outcomes will include quality of life, pain, fall self-efficacy, behavior change variables, intervention cost, fractures, and adverse events. Analyses of feasibility objectives will be descriptive or based on estimates with 95% confidence intervals, where feasibility will be assessed relative to a priori criteria. Differences in secondary outcomes will be evaluated in intention-to-treat analyses via independent Student t tests, chi-square tests, or logistic regression. The Bonferroni method will be used to adjust the level of significance for secondary outcomes so the overall alpha level is .05. Limitations No assessment of bone mineral density will be conducted. The proposed definitive trial will require a large sample size. Conclusions The viability of a large-scale exercise trial in women with vertebral fractures will be evaluated, as well as the effects of a home exercise program on important secondary outcomes.

Low prevalence of unexpected popliteal DVT detected on routine MRI assessment of the knee.

OBJECTIVE The purpose of this study was to document the appearance and determine the prevalence of findings suspicious for popliteal vein thrombosis on magnetic resonance (MR) assessment of the knee joint. METHODS A total of 2888 MR examinations were retrospectively reviewed and classified as illustrating either normal appearing popliteal vein or findings suspicious for popliteal vein thrombosis. RESULTS A total of 2879 MR studies were assessed as having a normal appearing popliteal vein. Nine studies illustrated findings suspicious for popliteal vein thrombosis. CONCLUSION Although the prevalence of MR findings is low (0.3%), our findings reiterate the need to interrogate the popliteal vein for evidence of thrombosis.

Spontaneous resolution of quadrilateral space syndrome: a case report.

ABSTRACTA case of quadrilateral space syndrome is presented, where a large near-circumferential glenoid labrum tear led to a paralabral cyst that dissected into the quadrilateral space and caused a compressive neuropathy of the axillary nerve. This led to a 6-mo history of left shoulder pain, parasthesias, marked weakness to abduction, and marked denervation in both the deltoid and teres minor on electro-diagnostics. This is a presentation of interest as it is the only case report in the literature, to the authors’ knowledge, where spontaneous resolution of entrapment occurred. This normally requires intervention for definitive management. It resolved through nonsurgical management with pregabalin, oxycodon, and naproxen medications, leading to good functional return, as well as pain and presumed muscle edema dissipation, while awaiting interventional consultation. It also illustrates that quadrilateral space syndrome is a difficult clinical diagnosis owing to the nonspecific symptom presentation, as well as weakness.

Ewing’s sarcoma of the patella

Ewing’s sarcoma is a relatively rare malignancy, occurring mainly between 4 and 25 years of age. It usually arises from the pelvis, followed by the femur, tibia, and remainder of both the long bones of the extremities and flat bones of the axial skeleton. To the best of our knowledge, Ewing’s sarcoma of the patella has never been reported previously. Patellar tumors occur infrequently and represent an uncommon etiology of anterior knee pain. We describe the rare case of a 41-year-old man who presented with a 3–4 month history of escalating right anterior knee pain and swelling. Imaging demonstrated an aggressive patellar tumor with an adjacent soft tissue mass. The diagnosis of Ewing’s sarcoma was confirmed by pathology. Physicians should be aware of atypical locations for Ewing’s sarcoma and, conversely, of rare tumors arising in the patella and accounting for anterior knee pain. Early recognition of such malignancies allows prompt initiation of treatment, hence improving prognosis.
